INTRODUCTION: Low back pain (LBP) represents the primary global cause of years lived with disability, significantly compromising health-related quality of life and imposing a substantial economic burden on healthcare systems. This narrative review conceptualizes LBP as a dynamic life-course process rather than a static diagnostic entity.
METHOD: A comprehensive search of PubMed, Scopus, and Web of Science was conducted using MeSH and free-text terms (e.g., "Low Back Pain," "Aging," spine diseases) for clinical trials, cohorts, and reviews published from inception through to January 2026.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Acute myocardial infarction (AMI) significantly contributes to the progression of heart failure (HF). Standard treatment for HF has long been angiotensin-converting enzyme inhibitors (ACEIs), targeting the renin-angiotensin-aldosterone system (RAAS). Recent developments in HF management introduced sacubitril/valsartan (S/V), a novel angiotensin receptor-neprilysin inhibitor (ARNI), showing promising results in global trials. This study aimed to assess the efficacy of early S/V application compared to angiotensin-converting enzyme inhibitors (ACEIs) in reducing NT-proBNP levels and

INTRODUCTION: Low back pain (LBP) is highly prevalent after lower limb amputation (LLA) and contributes to substantial reductions in quality of life and function. Towards understanding pathophysiological mechanisms underlying LBP after LLA, this article compares lumbar spine pathologies and muscle morphologies between individuals with LBP, with and without LLA.
MATERIALS AND METHODS: We queried electronic medical records of Service members with and without LLA who sought care for LBP at military treatment facilities between January 2002 and May 2020. Two groups with cLBP, one with (n = 15) an

After corrective surgery for adolescent idiopathic scoliosis (AIS), patients can return to sports activities without restrictions. While there have been many reports of long-term disc degeneration between adjacent segments after posterior corrective fusion, the effects of sports activities on adjacent segments after corrective fusion surgery are not well understood. Particularly, cases of acquired spondylolysis after long fusion surgeries for scoliosis are rare. In this report, we present two cases of AIS in which patients continued to play badminton at a high-performance level following poste

BACKGROUND: Pediatric lumbar spondylolysis (LS) is common in junior and senior high school athletes. Lower LS (L4-L5 level) is more common in children, and upper LS (L1-L3 level) is relatively rare; therefore, the pathogenesis of upper LS remains unclear.
PURPOSE: To elucidate the mechanisms of upper LS by identifying and comparing characteristics between upper and lower LS cases.
STUDY DESIGN/SETTING: Retrospective cross-sectional study.
PATIENT SAMPLE: This study included 842 pediatric patients aged ˂18 years diagnosed with single-level acute LS at a single outpatient clinic between 2015

Spondylolysis is defined as a defect or elongation in the pars interarticularis of the lumbar spine, either unilateral or bilateral. Growing children with bilateral spondylolysis may develop spondylolisthesis, i.e., forward slipping of the affected vertebra. The etiology of spondylolysis is regarded as a stress fracture due to repetitive loading associated with a genetic predisposition. Lumbar magnetic resonance imaging (MRI) shows an increased signal intensity before an actual fracture line develops. In low grade spondylolisthesis, two-thirds of children with acute pediatric spondylolysis wil

Lumbar spondylolysis of a single lumbar vertebra with a fracture of the pedicle on the opposite side, as well as fractures of both pedicles and bilateral spondylolysis, have been extensively reported in the literature. These cases are commonly linked to factors such as trauma, sports activities, and spinal surgeries. We report a unique case of a unilateral lumbar spondylolysis with a fracture on the opposite side including the pedicle and lamina. To the best of our knowledge, this specific case has not been previously reported. Additionally, we provide a comprehensive assessment of the existin

CASE: A 14-year-old male athlete presented with a 9-month history of low back pain, worse with hyperextension. Nonoperative management for bilateral L4 spondylolysis had been unsuccessful. The patient underwent a novel magnetic resonance imaging (MRI) that generated a synthetic computed tomography (sCT). MRI/sCT revealed nonunion of the spondylolysis with persistent edema. The patient underwent minimally invasive bilateral L4 robot-navigated intralaminar screw fixation. CT at 3 months demonstrated complete healing.
CONCLUSION: This case is the first to document the use of MRI-generated sCT in

Back pain that is associated with lumbar spine pathology is a growing issue in the athlete population. As an aging population continues to remain active, it is essential for primary care physicians, general orthopaedic surgeons, and spine surgeons alike to understand the nuances of diagnosis and management in the recreational athlete population. This is a unique population due to the increased importance placed on returning to high levels of activity, but, by definition, they enjoy less resources and financial incentive to optimize their rehabilitation and return to sport compared with profess

BACKGROUND AND OBJECTIVE: The classification of lumbar spondylolysis varies, and there is currently no clear consensus on a standardized system. This study examines the morphological characteristics and parameter differences of the L5 vertebra in patients with isolated versus fused spondylolysis using CT measurements. It also proposes a preliminary classification system based on the separation distance at the fracture site and explores its clinical significance.
METHODS: A total of 117 young male patients with L5 spondylolysis related to high-intensity physical activity were enrolled. Patient

STUDY DESIGN: Retrospective case-control study.
OBJECTIVE: To determine risk factors for recurrence of pediatric lumbar spondylolysis at L5 after return to sport in patients who achieved bone union with conservative treatment.
SUMMARY OF BACKGROUND DATA: Pediatric lumbar spondylolysis is a stress fracture commonly observed in adolescent athletes, particularly at the L5 vertebra. Because some patients experience a recurrence of spondylolysis after bone fusion with conservative treatment, identifying risk factors for recurrence may help athletes continue sports activities, maintain performance

OBJECTIVE: Spondylolysis, often progressing to spondylolisthesis, commonly defies conservative treatment in refractory cases, indicating a need for surgery. Robot-assisted techniques may provide a stable and effective minimally invasive approach for the treatment of lumbar spondylolysis. To compare the clinical efficacy between robot-assisted percutaneous screw fixation combined with endoscopic bone graft and conventional open screw fixation with bone graft in the treatment of lumbar spondylolysis.
METHODS: A cohort study involving 43 individuals with lumbar spondylolysis who underwent surgic

Pars interarticularis stress injury and spondylolysis are frequent causes of back pain among young athletes. Accurate diagnosis and grading of these conditions are essential to institute appropriate management strategies in order to achieve optimal outcomes. The aim of management strategies for acute active spondylolysis stress reactions and stress fractures is to achieve bone healing while avoiding recurrence and non-union, with the ultimate goal of returning to sports as determined by symptom resolution. Although various radiological classification systems for spondylolysis have been describ

PURPOSE: CT is considered the gold standard to assess bone morphology, whereas MRI is the imaging modality of choice to assess soft tissue. To reduce the ionising radiation exposure of the patient from CT, new MRI technologies have been developed to obtain images of bone. Two promising methods include MRI-based synthetic CT (sCT) and T1-weighted isotropic MRI.
METHODS: A prospective study including twenty-four adult patients receiving lumbar or sacral spine stabilisation surgery was performed. For each patient, two scans were acquired: a 3D dual echo T1-weighted gradient image, from which a s

PURPOSE: The diagnosis of adolescent lumbar spondylolysis typically requires computed tomography (CT) imaging; however, concerns regarding medical radiation exposure have emerged. With recent advancements in magnetic resonance imaging (MRI), it has become possible to acquire CT-like images using an MR bone imaging approach. This study aimed to investigate whether MR bone imaging can be utilised as a substitute for CT in evaluating the fracture line in patients with adolescent spondylolysis.
METHODS: We included 24 patients (28 levels) diagnosed with adolescent lumbar spondylolysis between Jun

BACKGROUND: Fractures of the thoracic and lumbar spine are uncommon injuries in children and adolescents, but they carry substantial morbidity and a real risk of death. Compared with the adult spine, the pediatric spine has unique anatomic and biomechanical properties that yield different fracture patterns than the adult population. The mechanisms of injury and degree of skeletal maturity constitute important predictors of the outcomes of pediatric thoracic and lumbar fractures; however, there remains a paucity of meaningful epidemiologic data with specific attention to the pediatric spine.

BACKGROUND: Spondylolysis is commonly linked to low back pain in athletes, and the connection between muscle weakness and spondylolysis is unclear. Therefore, this study examined the biomechanics of spondylolysis and influence of muscle weakness by finite element (FE) analysis.
METHODS: A patient's L1-S1 lumbosacral unit was scanned by computed tomography, and generated a three-dimensional pathology-free FE model. Unilateral incomplete, unilateral, and bilateral defect models of the L5 isthmus were created. The mobility of the sacrum was limited to 7.5 N m moment, and a 500 N load was applied
