Risk factors for insufficient segmental lordosis at the fused level following posterior lumbar interbody fusion: Effect of change in segmental lordosis in the intraoperative prone position.

BACKGROUND: A decrease in segmental lordosis (SL) at the fused level (ΔSL < 0) following posterior lumbar interbody fusion (PLIF) has been identified as a contributing factor to early-onset adjacent segment disease. However, achieving sufficient SL can be challenging despite technical strategies such as optimized cage placement. This study aimed to identify risk factors for ΔSL < 0 following PLIF, with a particular focus on intraoperative prone SL. METHODS: A total of 105 patients who underwent primary single-level PLIF at L4-5 for degenerative lumbar spondylolisthesis between 2017 and 2022 w

Neuro-Geometric Graph Transformers with Differentiable Radiographic Geometry for Spinal X-Ray Image Analysis.

Radiographic imaging remains a cornerstone of diagnostic practice. However, accurate interpretation faces challenges from subtle visual signatures, anatomical variability, and inter-observer inconsistency. Conventional deep learning approaches, such as convolutional neural networks and vision transformers, deliver strong predictive performance but often lack anatomical grounding and interpretability, limiting their trustworthiness in imaging applications. To address these challenges, we present SpineNeuroSym, a neuro-geometric imaging framework that unifies geometry-aware learning and symbolic

Functional Outcomes of Intensive Rehabilitation Versus Fusion Surgery in Patients With Low Back Pain Due to Lumbar Degenerative Spine Disease: A Systematic Review and Meta-Analysis.

Lumbar degenerative disease is mostly managed non-operatively, but lumbar spinal fusion has been used for almost a century. It has shown clinical efficacy in decreasing pain and disability scores and allowing patients to return to work. However, spinal fusion may not be clinically effective and may place a financial burden on patients. The alternative to spinal fusion is physical therapy and rehabilitation for lumbar degenerative diseases. This study aims to compare the functional outcomes of intensive rehabilitation and lumbar spine fusion surgery among patients with chronic low back pain fro

The Alignment and Compensation (AC) Classification of High-Grade Spondylolisthesis: Description and Validation of a Novel Biomechanical Classification.

STUDY DESIGN: Cross-sectional reliability study. OBJECTIVE: To propose and validate a novel biomechanical classification for high-grade spondylolisthesis (HGS) that incorporates compensatory mechanisms into the assessment of sagittal alignment patterns. BACKGROUND: Existing classifications of HGS are primarily descriptive, rely on parameters often distorted by sacral endplate dysplasia, offer limited guidance for surgical decision-making and do not completely capture the spectrum of compensation in sagittal alignment. The new classification system defines four alignment types: type 1: propor

3D-Navigation guided sublaminar contralateral unilateral biportal endoscopy (UBE) for migrated foraminal disc in the lumbar spine: A technical note and case series.

BACKGROUND: Migrated lumbar foraminal disc herniation (FDH) poses a great challenge, requiring the facet joint resection and stabilization procedure. We used a contralateral sublaminar unilateral biportal endoscopic (UBE) approach utilizing O-arm navigation to preserve the facet and effectively resect a FDH. METHODS: Through the sublaminar window, resecting the ligamentum flavum, performing a limited medial facetectomy using O-arm navigation, safely exposed the foraminal zone, allowing for the safe FDH resection. RESULTS: Fourteen patients underwent this procedure between 2022 and 2023. Mean

The Prone-Angled Transpsoas Approach: Utilizing Position, Approach, Navigation, and Neuromonitoring in Prone Lateral Surgery.

BACKGROUND AND OBJECTIVES: Lateral lumbar interbody fusion techniques have advanced toward minimally invasive, anatomy-preserving approaches that enhance sagittal alignment and reduce perioperative morbidity. The most common existing approaches-the prone transpsoas and oblique lumbar interbody fusion-offer distinct advantages with opposing limitations. This manuscript describes a novel prone angled transpsoas (PAT) technique that combines the ergonomic and sagittal benefits of prone positioning with a navigated oblique/angled approach and direct lateral working corridor. This technique aims to

High-energy traumatic spondyloptosis at T8-T9 with complete spinal cord injury: a case report.

INTRODUCTION: Spondyloptosis, the most severe form of spondylolisthesis, involves complete (>100%) anterior or posterior displacement of one vertebra over the subjacent segment, resulting in total anatomical dislocation. Typically caused by high-energy trauma, it leads to severe spinal instability, bony fragment intrusion into the canal, and significant neurological deficits. This report presents a representative case of T8-T9 spondyloptosis with complete spinal cord injury [American Spinal Injury Association (ASIA) Impairment Scale Grade A]to analyze its injury features, surgical approach, an

Is flexion-extension imaging necessary for surgical decision-making in degenerative lumbar spondylolisthesis? Supine MRI and upright radiographs offer superior diagnostic value.

INTRODUCTION: Degenerative lumbar spondylolisthesis (DLS) is a major indication for lumbar surgery. Surgical decision-making, particularly regarding the need for fusion, often depends on radiographic detection of segmental instability. Flexion-extension (FE) radiographs remain the reference standard but are limited by radiation exposure, motion-related discomfort, and poor reproducibility, whereas upright-supine (US) imaging - combining upright lateral radiographs with supine MRI - may offer a comparable, low-radiation alternative using routinely acquired images. RESEARCH QUESTION: To compare

ERAS protocol for anterior/oblique lumbar fusion reduces postoperative length of stay without worsening outcomes.

BACKGROUND: Anterior and oblique lumbar interbody fusion combined with enhanced recovery protocols, has gained prominence for optimizing patient outcomes, including reduced postoperative length of stay (PLOS). The approval of anorganic bone graft substitute like P-15/ABM, which reduce complications associated with autologous graft harvesting, further supports these approaches. However, the impact of these strategies on clinical outcomes, particularly in reducing PLOS and complications, remains unclear. This study addresses: Does combining enhanced recovery protocols with anorganic bone graft s

Three-step fluoroscopy-guided percutaneous lumbar pedicle screw placement: a pilot study on technical feasibility, safety, and fluoroscopy reduction.

BACKGROUND: Minimally invasive surgery has gained widespread popularity in clinical practice. Among spinal surgeries, percutaneous pedicle screw placement (PPS) is one of the most widely performed procedures. However, it necessitates high-frequency fluoroscopic guidance to ensure accuracy, resulting in substantial radiation exposure. Therefore, it is crucial to develop a technique that is fast, safe, and minimizes radiation exposure. OBJECTIVE: We aimed to describe a novel three-step fluoroscopy-guided technique for PPS and preliminarily evaluate the technical feasibility, procedural efficien

A Trustable Spine Abnormalities Classification System Using ResNet50 and VGG16 Supported by Explainable Artificial Intelligence.

Deep learning has been applied in various fields and has been proven to provide good results for classification tasks. However, there is limited understanding of a deep learning model's decisions, so deep learning is commonly described as a black box. Applying deep learning for critical applications such as medical diagnostic process introduces trust issues. For the deep learning model to be trusted by the medical practitioners, the methods employed by the deep learning model must be seen to be aligned with the diagnostic process employed by the medical practitioners. Explainable methods such
