[Diagnosis and therapy of the narrow lumbar spinal canal].

In 30%-40% of the patients who are operated on for herniation of lumbar discs, osseous stenosis plays a certain role. However, only in one-third of them are special operative measures such as laminectomy necessary with or without additional lumbar fusion. When spondylodesis is carried out after laminectomy it is often combined with metal implant, which can drastically reduce the time a patient requires perioperative treatment. In younger patients showing typical signs of nerve root compression due to osseous stenosis of lateral recess, only segmental decompression in the form of foraminotomy i

Spinal deformities in patients with muscular dystrophy other than Duchenne. A review of 11 patients having surgical treatment.

Eleven patients diagnosed as having muscular dystrophy and who underwent posterior spinal fusion were reviewed: Becker dystrophy in one, limb girdle in two, facioscapulohumeral in one, myopathia unspecified in one, myotonia dystrophica in two, myotonia congenita in one, and hypotonia congenita in three. There were eight females and three males. The curve pattern was thoracic in four, thoracolumbar in three, double thoracic and thoracolumbar in three, and thoracolumbar lordosis in one. Scoliosis was associated with kyphosis in two, with lumbar lordosis in one, and thoracic lordosis in four pati

[Lumbar stenosis with scoliosis. Symptomatologic study and surgical treatment of 39 cases].

In 39 cases of narrowed lumbar spinal canal associated with a scoliosis, the authors have studied the role played by deviation of the spine and the origin of the narrowing. It seemed that central stenosis with intermittent claudication was more frequent when a scoliosis was present. In more than half of the cases, the level of the compression was at the junction of the curves. After failure of conservative treatment, the surgical procedure performed was the same as in straight spines. The results were satisfactory in 90 p. 100 as regards root involvement and intermittent claudication. However

Halo-clavicle traction.

A case is presented employing a modification of the halo traction technique using clavicle and scapular based pins attached through Hoffmann spacing bars to a standard halo frame. Other techniques that have been employed for fractures and dislocations of the cervical spine are reviewed. The principle advantages are the satisfactory stability it affords and the access the technique provides to the torso. The major problem anticipated of loosening of pins or instability of the apparatus because of the mobile clavicle and scapula did not occur in this patient. The halo-clavicle traction is a need

Surgical treatment of cervical spondylotic myelopathy complicating athetoid cerebral palsy.

Operative treatment was performed in nine patients with cervical spondylotic myelopathy complicating athetoid cerebral palsy. The first two patients were treated by laminectomy, and the other seven by anterior interbody fusion. The symptoms in both the laminectomy patients improved after operation, but became worse again when cervical instability developed; they then had to have an anterior fusion in addition. In six of the seven patients who had primary anterior fusion a halo-cast (or a halo-vest) was used to keep the cervical spine immobile, and good bony fusion was obtained with satisfactor

Anterior stabilization of thoracolumbar injuries.

A new implant system has been designed to rigidly fix the spine anteriorly at a single level above and below the injury site for treatment of severe thoracolumbar fractures. The primary system consists of two rods rigidly linked by vertebral body bridges. These bridges are secured to each vertebra at two points: one posterolaterally on the body with a vertebral body screw and the other more anterolaterally with a vertebral body staple. This implant system has been used in 48 consecutive thoracolumbar injuries. There have been three late failures to attain spine fusion, but in no patient has th

Symptomatic atlantoaxial subluxation in persons with Down syndrome.

In a prospective study 40 of 236 individuals with Down syndrome were found to have atlantoaxial instability. Whereas most of the 40 affected children were asymptomatic, seven patients exhibited various neuropathology. Historical data, neurologic findings, radiologic interpretations, and surgical management of these seven patients are described in detail. In addition, case reports of other individuals with Down syndrome and symptomatic atlantoaxial subluxation from the literature were analyzed. We conclude that early identification of these patients and appropriate surgical management, includin

Posterolateral lumbosacral arthrodesis (P.L.A.). A study of 85 cases.

Posterolateral lumbar and lumbosacral arthrodesis has been used during the last 50 years as a method of posterior arthrodesis in patients who have undergone previous laminectomies or after previous unsuccessful attempts at posterior fusion. The higher percentages of successful fusion has extended the use of this technique to other conditions such as spondylolisthesis and lumbar pain due to segmental instability. The present study is a clinical and radiographic evaluation of the results obtained in a series of 84 patients affected by lumbar or lumbosciatic pain arising from different causes - s

Long-term follow-up evaluation of a few patients with spondylolisthesis treated by excision of the loose lamina with decompression of the nerve roots without spinal fusion.

Long-term follow-up observations were recorded in 52 patients with spondylolisthesis who were treated by excision of the loose lamina with decompression of the nerve roots without spinal fusion. The longest follow-up period was 146 months (average, 71 months). Measured by the method of Taillard, the average postoperative increase in displacement in all of the patients, including children, was 5%. In patients with increasing slippage, results were satisfactory in 90% and failures in 10%, whereas 82.7% of the results were satisfactory and 17.3% were failures in the group that showed no progressi

Anterior cord decompression and spinal stabilization for patients with metastatic lesions of the spine.

Metastatic lesions involving the spine may cause vertebral body collapse, resulting in either spinal instability or neural compression, or both. Progressive destruction of the vertebral body by the tumor may cause increasing spinal instability, leading to a kyphotic deformity and further neural deficit. Anterior decompression allows direct excision of the tumor focus and direct neural decompression. Because of postoperative irradiation, conventional bone grafts are rarely incorporated. Over an 8-year period, 52 patients with spinal instability secondary to metastatic pathological fractures of

Combined neurosurgical-orthopedic approach to the correction of radiculopathy and instability in spondylolysis and spondylolisthesis.

The authors give a brief review of the pathologic anatomy, the incidence and distribution, the probable etiologic factors in the development of spondylolysis and spondylisthesis, and a description of the clinical syndrome of this abnormality. It is pointed out that there are two distinct parts to the clinical syndrome: back pain due to instability of the isthmic defect and/or radiculopathy due to direct compression of the lumbar nerve root in its course immediately beneath the osteocartilaginous deformity which is the primary pathologic feature of this disorder. It is suggested that the variab

Acquired spondylolysis after spinal fusion.

Spondylolysis occurring after a spinal fusion is considered to result from operative damage to the pars interarticularis on both sides. Fourteen cases are reported, and compared with the 23 cases which have previously been published. The defects are usually recognised within five years of fusion, and usually occur immediately above the fusion mass. Other contributory causes may be: fatigue fracture from concentration of stress; damage and altered function of the posterior ligament complex; and degenerative disc disease immediately above or below the fusion. Fusion technique is critical, since

[Involvement of the cervical spine in chronic juvenile arthritis. 29 cases followed for more than 5 years].

Analysis of cervical spine involvement is based on data from 29 cases of juvenile chronic arthritis (JCA), (8 systemic and 21 polyarticular forms). Follow-up is a least 5 years and exceeds 10 years in 17 cases. Among these 29 patients, 14 experienced cervical pain. One patient had Arnold neuralgia. No other neurologic complications were recorded. 14 of the 29 patients exhibited one or more roentgenological changes (3 of the 8 systemic patients and 11 of the 21 polyarticular patients). Of these, 4 never had any corresponding clinical symptoms. A chronologic correlation between onset of clinical

Interspinous process segmental spinal instrumentation.

A method for interspinous segmental spinal instrumentation (ISSI) is described and the laboratory testing and early clinical results are reported. The method utilizes a button-wire implant that is passed through the thickest and strongest part of the base of the spinous process. Tension tests show the stress-relieving qualities of the implant improve pullout strength 47% over simple wire fixation of the spinous process. Tests on the scoliosis simulator show that the interspinous instrumented spine resists high compressive loads to failure, comparing favorably with other systems tested. Early c

Acquired spondylolisthesis following scoliosis surgery. A case report.

The association between scoliosis and spondylolisthesis is well documented; however, spondylolisthesis following a posterior spinal fusion with Harrington rod instrumentation for idiopathic scoliosis has not been previously reported. Internal fixation of the spine with metal rods and fusion for scoliosis produces a stress concentration at the immediately adjacent levels. This, combined with other factors that may cause failure of the neural arch, can lead to a traumatic spondylolisthesis. In an 11-year-old boy, localized pain and impending listhesis indicated that the instrumentation should be

Scoliosis: diagnosis and current treatment.

The most common form of scoliosis (lateral curvature of the spine) is adolescent idiopathic scoliosis (AIS). The prevalence of AIS is 10-20 individuals per 1,000 population screened. Progressive AIS affects females more frequently than males. Only 1-2 per 1,000 individuals have progressive AIS and need brace or surgical treatment. Early screening programs for scoliosis and other spinal deformities are instituted in schools in the fifth to ninth grades to achieve early detection of progressive AIS. If detected early, progressive AIS can in most cases be successfully treated by use of an externa

Microsurgical anterior approach to cervical discs. Review of 60 consecutive cases of discectomy without fusion.

During the past six years 60 patients have undergone anterior cervical microsurgical discectomy without exogenous intercorporal grafting using the operating microscope. This alternative method of treatment is a further refinement of previous techniques for the removal of cervical disc protrusions, which fulfils all the criteria of effective surgery. Discectomy provides ample exposure of the neural elements through the disc space. The visualisation of the underlying pathology is adequate through direct light and magnification of the operating microscope. A radical surgery is possible without da

[Instability of the cervical spine caused by arthrosis. An unusual form of degenerative spondylolisthesis].

A case of a patient with spondylolisthesis between C4 and C5 is reported. There were neurological signs of spinal cord involvement and the lesions were essentially those of a posterior degenerative arthrosis with disc degeneration at other levels. This severe instability was cured by an anterior interbody fusion.

Nocardial osteomyelitis of the spine with epidural spinal cord compression--a case report.

A 53-year-old man presented with complaints of back pain and weakness in his lower extremities. Physical examination demonstrated a thoracic myelopathy with a sensory level at T-4. The diagnostic work-up revealed vertebral osteomyelitis of the thoracic spine with epidural compression at T-2-T-4 causing a nearly complete block on myelography. A decompressive laminectomy and debridement were performed, followed by anterior spinal fusion. Nocardia asteroides was cultured from the epidural space. The patient was concurrently treated with sulfonamides. No underlying malignancy or immunosuppression

Paraplegia in unstable thoracolumbar injuries. A study of conservative and operative treatment regarding neurological improvement and rehabilitation.

Thirty-seven patients with unstable thoracolumbar fractures and paraplegia were studied: 11 patients were given exclusively conservative treatment, 14 patients were treated conservatively after laminectomy with or without fusion, and 12 patients were treated with Harrington instrumentation. None of the 12 cases with complete paraparesis at admission to the hospital improved their neurological status. In patients with moderate and severe but not complete paraparesis one month after injury, the neurological improvement continued for several years and in many the neurological restitution was comp
