Narrow axis: an inherited anomaly of the second cervical vertebra in the rabbit.

Narrow axis, an inherited anomaly resulting in a marked narrowing of the second cervical vertebra, has been observed in strain X/J rabbits. This condition is first recognizable on X rays at 32-33 days gestation. For size comparisons 21 measurements of the first five cervical vertebrae were taken on the skeletons of each of 14 strain X/J animals (7 normal and 7 with narrow axis) and 14 IIIC/J animals for control at two months of age and 27 strain X/J (11 normal and 16 narrow axis) and 14 strain IIIC/J at seven months of age. The primary effect appeared to be a premature fusion of the centrum wi

The mechanical consequence of failure of ossified union in attempted posterior spinal fusion. A canine model.

The mechanical behavior of pseudarthrosis in posterior spinal fusion was investigated. A canine model was developed in which an incompletely ossified posterior fusion mass was consistently produced. The spines were excised, and the motion segments were mechanically tested using a specially developed loading apparatus. Tests were performed to evaluate stiffness of the segments to loading with compression, torsion, and anterioposterior and lateral bending shear stiffness. Changes in other modes of loading were less consistent. The motion characteristics of the pseudarthrosis could not be predict

Unrecognized spinal instability associated with seemingly "simple" cervical compression fractures.

This paper described 27 patients who had seemingly stable compression cervical vertebral fractures. All were treated in cervicothoracic orthoses. None was initially treated in a Halo device or with surgery. Twenty patients were stable and healed without progressive displacement, angulation, or pain. One patient demonstrated an angular deformity, which spontaneously stabilized within six months with anterior bony ankylosis in a kyphotic position. Six patients demonstrated persistent progressive post-injury instability related to posterior ligament rupture. As muscle spasm diminished, the patien

Prospective study of the radiological changes in hands, feet, and cervical spine in adult rheumatoid disease.

Annual radiographs of hands, feet, and cervical spine were taken in 100 patients with rheumatoid arthritis from the first year of disease for a mean follow-up period of 9.5 years. Seventy-six patients developed peripheral erosive disease and 54 developed rheumatoid changes of the cervical spine, of whom 34 (63%) had subluxations. The severity of rheumatoid neck damage correlated strongly with the severity of peripheral erosive disease (p = 0.002). Cervical subluxation was more likely to occur in patients with erosions of the hands and feet which deteriorated progressively with time (p = 0.018)

Congenital atlanto-axial dislocation.

Report about 105 personally operated cases of congenital atlanto-axial dislocation. This fairly common malformation in India occurs three times more in males than in females, 50% show symptoms before the age of 20 years, the other half during the third decade of life. Transient attacks of cervical cord compression occur with progressive motor and posterior column defect, and sometimes unconsciousness and cerebellar involvement are characteristic. The diagnosis is proved by X-rays including tomograms and dynamic studies. Three types of malformation can be differentiated: Group I with assimilati

Lower lumbar anterior discectomy without fusion: a several year follow-up indicating usefulness of this technique in surgery of the lower lumbar spine--a report of one hundred ten cases.

For the surgical treatment of lumbar disc lesions, the posterior approach has often been used. It is not always easy to get good results, because one has to go through the spinal canal with irritation of the dura and the nerve root and cause bleeding. If the correct diagnosis is made by myelography or other methods, it is more logical to remove the disc from the front without disturbing the spinal canal. Under this idea, anterior discectomy has been performed on 110 patients suffering from low back pain and sciatica. The excellent results were obtained in 90.9 per cent.

Extrapharyngeal (anterolateral) approach to the cervical spine.

The extrapharyngeal approach to the anterior cervical spine is a safe, rapid surgical exposure. Other surgical exposures such as the posterior, lateral, and intraoral (transpharyngeal) have inherent limitations that this approach avoids. By going anterior to the sternocleidomastoid muscle and great vessels, the surgical exposure of the anterior cervical spine is wide and the vital structures of the neck are visualized and not injured. We have used this extrapharyngeal approach to treat various disease states of the anterior cervical spine, such as trauma, osteomyelitis, neoplasia, and degenera

Spinal fusions to the sacrum in adults with scoliosis.

Forty-five skeletally mature patients averaging 44.3 years of age had spinal fusions which extended to the sacrum for pain and/or progression of their scoliosis. The primary diagnosis was idiopathic scoliosis in 35 patients, congenital scoliosis in two patients, and paralytic scoliosis in eight patients. A single curve pattern was present in 41 patients and the remainder had double primary curves. Thirty-eight patients had single stage procedures and 35 of these were done posteriorly. There was an evolution in the type of posterior instrumentation used over the 12 years. Thirty-five patients (

Staged correction of neuromuscular scoliosis.

Anterior release and fusion, combined with tong gravity traction and second stage L-rod instrumentation, establishes correctability and accomplishes circumferential arthrodesis of the spine in neuromuscular scoliosis. From March 1979 through April 1982, nine patients with neuromuscular scoliosis, with an average age of 16.4 years, underwent this two-stage surgical procedure. The parameters investigated included correction of scoliotic deformity, correction of pelvic decompensation, and spinal arthrodesis. The preoperative scoliotic curvature measured 81 degrees and the postoperative curve meas

Fifty years survival after cervical fracture and fusion.

A patient who survived for 50 years after two cervical injuries is described. Among other effects he showed remarkable compensatory changes in the vertebrae above and below a cervical fusion operation. He also developed a meningeal pseudocyst following lumbar laminectomy. The history and pathological findings are considered to be instructive.

The posterior interspinous fusion in the treatment of quadriplegia.

The purpose of this report is to describe our experience with the posterior interspinous fusion in the treatment of spinal injury with quadriplegia. The charts and roentgenograms of 22 patients treated with this operation by the senior author (RW) from July 1978 to June 1981 were reviewed retrospectively. Follow-up averaged 19.5 months. All injured spines had significant posterior ligamentous damage. There were 14 fracture-subluxations, two unilateral facet dislocations, and six bilateral facet dislocations. The specific operative indications included six unacceptable closed reductions, nine f

Clinical aspects of juvenile and adult ankylosing spondylitis.

Ankylosing spondylitis (AS) is a heterogeneous and systemic rheumatic disorder that is characterized primarily by inflammation of the spine and sacroiliac joints. Consequently, back pain is a frequent presenting complaint although the disease can begin with peripheral arthritis as well as acute anterior uveitis. Unlike men, however, women appear to have milder or atypical AS that may go unrecognized for years. Moreover, the presentation in children rests on the recognition of two distinct subgroups that may be indistinguishable from juvenile chronic polyarthritis. The more frequent subgroup in

Transoral operations for craniospinal malformations.

The transoral approach to the lower third of the clivus and to the ventral aspect of the upper cervical spine is used in craniospinal malformations with or without dislocation as well as in basilar aneurysms, ventrally situated cranio-spinal tumours, fractures of the odontoid process, and in rheumatoid arthritis compressing the spinal cord. In consideration of the literature and ten personal cases the indications and techniques of the transoral approach in craniospinal malformations are discussed. According to our own experiences and those of other authors it is possible to expose the lower cl

[Atloaxoid dislocation in chronic inflammatory rheumatism. Results of 16 occipitoaxoid arthrodeses].

Sixteen cases of spontaneous dislocation of the atlanto-axial joint have been reviewed, fifteen in rheumatoid arthritis and one in ankylosing spondylitis. On four occasions there was some neurological impairment, but in eleven there was only pain and stiffness. The only case of ankylosing spondylitis was operated on after failure of conservative treatment. Three patients died between one and eight months after operation, two of them having had severe neurological impairment before operation. In the thirteen other cases, the average follow up was four years. All had satisfactory fusion after gr

Assessment of bony union after interbody fusion of the lumbar spine using a biplanar radiographic technique.

Assessment of bony union after anterior fusion of the lumbar spine has previously relied on the skilled interpretation of plain radiograph. A biplanar radiographic technique was used to measure small movements between vertebrae and to give a quantitative measure of bony union in 11 patients who had undergone interbody fusion with autogenous bone chips at one level in the lumbar spine. The investigation gave three types of results: bony union, where the fused level showed marked restriction of movement relative to the rest of the lumbar spine; paradoxical movement, where the fused joint showed

Posterior atlanto-axial fusion: a new compression clamp for laminar osteosynthesis.

A new method of dorsal arthrodesis in patients with atlanto-axial instability of different etiology is described. In three patients solid laminar osteosynthesis C1/C2 was achieved by insertion of an autogenous cortico-cancellous bone graft into the interlaminar space and by a bilateral application of the lately developed metal clamp. The technical principles of the clamp guarantee a permanent pressure on the graft and a solid initial stability in the segment. Additional external fixation and immobilization of the patient are not necessary. Complications as known from other procedures of the at

Surgical management of the symptomatic cervical spine in rheumatoid arthritis.

Eleven spinal fusions were performed on 10 patients with symptomatic cervical spines secondary to rheumatoid arthritis. A variety of surgical procedures and various types of postoperative immobilization were used. The average patient age at surgery was 43.2 years, with an average followup of 36.9 months. Six of these patients had instability at C1-2, one each at C4-5, C5-6, and C6-7, and one patient had a complete myelographic block without instability at C5-6. Six of the patients presented with neurologic symptoms - all improved; some dramatically. There were no deaths, four transient complic

Spinal cord injury without radiographic abnormalities in children.

Spinal cord injury in children often occurs without evidence of fracture or dislocation. The mechanisms of neural damage in this syndrome of spinal cord injury without radiographic abnormality (SCIWORA) include flexion, hyperextension, longitudinal distraction, and ischemia. Inherent elasticity of the vertebral column in infants and young children, among other age-related anatomical peculiarities, render the pediatric spine exceedingly vulnerable to deforming forces. The neurological lesions encountered in this syndrome include a high incidence of complete and severe partial cord lesions. Chil

Myelopathy following cervical spine manipulation.

Manipulation of the spine, a controversial mechanotherapeutic procedure, has been taught and practiced widely. Occlusion of vertebral, basillar, and cerebellar arteries with head and neck manipulation is well documented in the literature. However, there is a paucity of references about myelopathy associated with cervical spine manipulation. Three cases of cervical myelopathy following spinal manipulation are presented. All patients noted increase in cervical pain following manipulation, developed significant sensary/motor deficits within 24 hours and became tetraparetic. Two patients were foun

Management of odontoid fractures.

Fifty-one adults with odontoid fractures were treated between 1966 and 1979. Approximately 50% had additional trauma; over half of the patients were younger than 40 years old. Three patients presented with myelopathy: 1 was complete at C-2 and the others recovered. Cervical roentgenograms were initially considered normal in 4 patients. The fracture was through the base of the odontoid process (Type 2) in 49 patients and was into the body of C-2 (Type 3) in 2 patients. Thirty-four patients underwent early posterior cervical fusion, whereas the others were treated initially with external immobil
