Disc excision and spine fusion in the management of lumbar disc disease. A minimum ten-year followup.

Seventy-nine percent of 312 patients who underwent lumbar disc surgery were evaluated at least 10 years postoperatively (mean equal to 13.7 years). Residual back and nerve root symptoms and functional impairment were equally as common among the 143 patients who underwent fusion as they were among the 64 patients who did not. Thirty percent of the patients whose spines were fused and 37.7% of those patients whose spines were not fused were considered long-term failures because of persistent symptoms or the need for reoperation. Thirty-seven percent of the fusion patients had persistent graft do

A lumbar disc surgery predictive score card.

A lumbar disc surgery predictive score card or questionnaire has been developed to assess potential candidates for excision of a herniated lumbar disc who have not previously undergone lumbar spine surgery. It is not designed to encompass patients who are being considered for other types of lumbar spine surgery, such as decompressive laminectomy or fusion. In an effort to make the "score card" usable by almost all physicians who are involved in lumbar disc surgery, only studies which have broad acceptance and are generally employed are included. Studies which have less widespread use such as e

[Treatment of cervical spine injuries by an anterior stable spondylodesis with an H-plate (author's transl)].

A stable spondylodesis of the cervical spine was obtained by an osteosynthesis with metal plates. The H-plate of the A-O in connection with autologous bone-chips from the iliac crest or Kieler bone and bone cement was used in five cases and resulted in a solid fusion of the affected cervical segments. The development of this technique and the advantages over other methods are demonstrated. The results of our cases reflect the superiority of this procedure.

[Enlarged operations for atypical lombo-sciatic pain from disk origin (author's transl)].

In certain forms of atypical sciatic pain the surgical treatment of the affection cannot be accomplished exclusively upon the disk. This study was done on 120 patients operated upon during a two year period. The major technics used had an action upon the articular processes and facets with or without removal of the disk. In 60 cases a desensibilisation of the region was equally done. The principal reason of the surgical treatment remains still to permit the decompression of the sick root. In certain circumstances a more important surgical treatment is required: laminectomy is performed in case

Repeat surgery after disc excision.

Sixty patients requiring repeat surgery after lumbar disc excision are reviewed. Thirty-one had had their initial surgery performed by the authors and represent a 5.9% frequency of repeat surgery after initial laminectomy for sciatica. Of the 60 patients, 49 suffered recurrent radicular pain, 20 suffered from back pain, and 8 had other problems (infection, extradural cysts, etc.). In patients with recurrent sciatica, perineural fibrosis and recurrent herniation were the most common problems; those patients with the latter pathologic condition responded most favorably to repeat surgery. Of 520

Failed disk syndrome.

Over half the patients in most pain clinics in the United States are seeking relief from the "failed disk syndrome." Careful initial diagnostic evaluation and aggressive nonoperative therapy are essential to avoid poor results. Causes of continued pain after disk surgery include original pain syndromes not due to a disk, inadequate surgery, development of arthritis, additional disk disorders and nerve root fibrosis. Treatment demands precise diagnosis. Diagnostic injections and nerve blocks are helpful when other techniques fail.

Metastatic disc infection and osteomyelitis of the cervical spine. Surgical treatment.

Two cases of vertebral osteomyelitis and discitis in the cervical spine are presented, both having arisen from hematogenous spread to the spine. Bacteriologic diagnosis was established in 1 patient, but the infection had subsided in the other. Both patients were treated with anterior debridement and fusion, and a supplemental posterior wiring and fusion was performed in the second case to reduce kyphotic angulation. In the second case, both the anterior and posterior surgery were performed while the patient was under the same anesthetic. Satisfactory results were achieved in both cases.

Atlanto-axial arthrodesis by the wedge compression method.

Fifteen patients with atlanto-axial instability (secondary to os odontoideum in three, nonunion of an odontoid fracture in seven, acute odontoid fracture in three, and rheumatoid arthirtis in two) were treated by wedge compression arthrodesis of the atlanto-axial joint. One patient died at home eight weeks after fusion with the cause of death never established. Of the two patients with rheumatoid arthritis (ankylosing spondylitis), one had a non-union and in the other the posterior arch of the atlas fractured and the fusion had to be extended up to the occiput and down to the third cervical ve

Congenital thoracic lordosis.

The rare spinal deformity of congenital thoracic lordosis is illustrated in five patients. Often this deformity is progressive and may cause severe pulmonary deficits and death. In the two patients most recently treated, early spine fusion (one anterior and one posterior) showed promising results--halting of the progression of deformity and even some amelioration of the lordosis. In two patients in whom active attempts were made to correct the lordosis, the complications were severe--one patient died and quadriparesis developed in the other.

Kyphosis in childhood and adolescence.

Kyphosis has become an increasingly important problem to the surgeon interested in the management of significant spinal deformity. As scoliosis has become better understood, the more difficult problems have become apparent and one of the most difficult of these is kyphosis. Kyphosis is the deformity which can produce paraplegia if it progresses to a significant degree and remains untreated. Contrary to scoliosis, in which bracing and posterior fusion alone are usually quite sufficient, kyphosis sometimes responds to bracing and other times it does not. If surgical treatment is necessary it qui

Comparison of the results in patients operated upon for ruptured lumbar discs with and without spinal fusion.

A comparative study of 259 patients operated on for ruptured lumbar discs, with and without spinal fusion, has been carried out. Simple removal of the herniated nucleus pulposus was performed in 119 patients while 68 were subjected to combined removal of the herniated disc material and lumbo-sacral fusion. During the first postoperative months the results were similar in the two groups, with a satisfactory result in 89 and 88 per cent of the cases respectively. During the following years a significant difference occurred between the groups. Severe pain recurred in 27% of the patients with simp

[The overgrowth of cartilaginous and fibrous structures in Paget's Disease (author's transl)].

The overgrowth of cartilaginous and fibrous tissue of the spine in Paget's disease is described in long-term follow-up studies. On cervical manifestation the disease progressed from one vertebrae to the other with destruction of the intervertebral discs, fusion of the vertebral bodies and intervertebral joints and shortening of the affected area. On the lower thoracic and lumbar spine osteomalacia with collapse of the vertebral bodies and biconvex deformities of the discs were seen in the initial period. Several years later the discs flattened, vaulted and fused in the periphery by dystrophic

Proceedings of the Annual Scientific Meeting of the International Medical Society of Paraplegia held at Stoke Mandeville from 28-30 July 1977 (Part II). Fracture dislocation of the cervical spine: a critique of current management in the United States.

The author has been asked to review the hospital records and X-rays of 12 patients from various regions of the United States who have sustained fracture dislocations of the cervical spine. In no instance was the initial care considered to be appropriate. No patient was significantly improved by treatment although only 17% had apparent complete transverse cord syndromes on admission. Sixty-seven per cent of the patients became worse. In the latter group there were three patients who were admitted with either no neurological deficit or only minimal pyramidal signs. All of these three patients be

Fused kidneys: morphologic study and theory of embryogenesis.

A study was made of 50 fused kidneys; 41 postmortem specimens and 9 living patients' records were reviewed. Three patterns of renal fusion were identified--midline fused horseshoe kidney [15], laterally fused horseshoe kidney [21], and crossed renal ectopia [14]. These morphologic renal patterns were correlated with coexisting abnormalities of the urinary tract and other organ systems. A high incidence of hydronephrosis of varied etiology, vertebral, and anorectal defects was found in the laterally fused horseshoe and crossed renal ectopic groups. A theory of embryogenesis was developed to exp

A postaxial polydactyly-dental-vertebral syndrome.

Three patients with postaxial polydactyly and other abnormalities of the hands and feet, hypoplasia and fusion of the vertebral bodies, and dental abnormalities are reported. Two were sisters born to normal unrelated parents; the other patient was the male offspring of a consanguineous marriage. We suggest that this constellation of abnormalities represents a recessively inherited syndrome.

Meander tail: a recessive mutant located in chromosome 4 of the mouse.

A variable kinked-tail mutant was found in 1974 in a moderately inbred stock of mice at Iowa State University, Ames. It was named meander tail and was shown to be completely recessive. Study 8 alizarin-stained skeletons showed all degrees of ankylosis or fusions of tail vertebrae and occasionally other vertebrae. Extreme examples had great reduction in number of tail vertebrae. Affected mice over 2 weeks old commonly also show some unsteadiness, presumably a pleiotropic effect of the mutant. Less commonly, one or both hind legs showed some paralysis. Linkage tests of the mutant, symbolized mea

Congenital kyphosis.

Congenital kyphosis is an uncommon, but potentially devastating anomaly of the spine. Without treatment, the tendency is for severe, and sometimes catastrophic deformity. It is the most common etiology for spinal cord compression due to spine deformity, excluding tuberculosis. Few patients have nonprogressive benign lesions. Late treatment is difficult and dangerous. Braces and other forms of non-operative treatment do not work. The best treatment is early posterior fusion, before the curve reaches 50 degrees and before age 5 years. For curves over 60 degrees and over age 5, both posterior and

[Scoliosis and spondylolisthesis (author's transl)].

The coincidence of a scoliosis with a lumbosacral spondylolysis or spondylolisthesis has remained largely ignored in the German language literature. After a survey of the foreign literature the pathogenesis of various combination forms is discussed. Primarily with the aid of oblique X-rays of lumbar scoliosis a scheme of classification involving 7 categories is worked out. The two main categories comprise unstable spondylolisthetic scolioses with increasing abnormal posture and scoliotic spondylolistheses. In these cases a lumbar scoliosis probably induces an asymmetric spondylolysis. The sche

The management of thoracic and thoraco-lumbar injuries of the spine with neurological involvement.

The results of treatment of injuries of the thoracic and thoraco-lumbar spine with neurological involvement have been reviewed in a retrospective study of 115 patients, of whom eighty-nine received conservative and twenty-six surgical treatment. Operation was reserved, in general, for patients with irreducible dislocations and incomplete neurological lesions, open reduction and internal fixation being the commonest procedure. Only three patients required a delayed spinal fusion for suspected instability after a period of conservative treatment. On the other hand, ten patients, eight of whom ha

Posterior cervical fusion in chidren.

Six cases of posterior cervical fusion with rib grafts in children are reported. Four of the children had sustained cervical spine injuries in accidents, and two had congenital absence of the odontoid. Three-level fusions (C1-3) were done in four children, and four-level fusions (C1-4) in two. One child died of unrelated causes 3 months after the operation. The other five children have been followed for 5 to 13 years. All are doing well and each has a remarkably supple, stable neck and no neurological deficit.
