Fracture of the odontoid process.

A review of 26 cases of odontoid fractures has been carried out. Late diagnosis and consequent delay in treatment were found to be important causes of non-union. Myelopathy in odontoid fractures is of two types. One type is that present immediately after injury, while the other is delayed. In treatment of long-standing cases, only fusion without posterior decompression should be done. If the latter procedure is carried out, it can produce medullospinal haematomyelia with respiratory arrest and tetraplegia. In the performance of atlanto-axial fusion, the technique of bone grafting and the use o

Comparison of disk excision and combined disk excision and spinal fusion for lumbar disk ruptures.

In a series of private patients 77 per cent over-all satisfactory results were obtained from surgical disk excision performed without spinal fusion. Patients in whom spinal fusion combined with discectomy has been recommended by other authors are not much more satisfied than patients with discectomy alone without fusion (81% versus 74% satisfactory). Patients in whom the third party influence prevails were significantly more dissatisfied (only 59% satisfactory). Findings of a definite disk rupture at the time of surgery result in a higher incidence of satisfactory rating than those with degene

Limited intertransverse fusion using iliac grafts.

Fusion of the lower lumbar spine or lumbosacral joint can be achieved by the use of limited intertransverse iliac grats inserted by a lateral approach. The method offers certain special advantages and is no less successful than other techniques in common use.

Congenital deformities in two Clydesdale foals.

The deformities observed in 2 Clydesdale foals are described. Both had abnormal joint positions in the forelimbs and discrepancies in the symmetry of the vertebral column. The changes were only mild in one case but extreme in the other where it was accompanied by torticollis, scoliosis and vertebral fusion. A comparison is made with deformities described in the contracted foal syndrome and some of the developmental implications discussed.

Degenerative spondylolisthesis.

Degenerative spondylolisthesis is one important type of spinal stenosis. The spinal canal stenosis varies in degree. This is the result of anterior slipping of the whole vertebra, usually L4 on L5. The clinical picture is not greatly different from that observed in the other types of lumbar canal stenosis. The surgical management involves decompression of the cauda equina by laminectomy. Often this is associated with arthrectomy to decompress the nerve roots. Extensive excision of the neural arch may result in further spinal instability for which spinal fusion is generally advisable.

Neurofibromatous scoliosis. Natural history and results of treatment in thirty-seven cases.

Of 400 patients with the diagnosis of neurofibromatosis on their hospital records, 141 actually had the disease. The presence of at least two of the following features was considered diagnostic: positive family history; positive biopsy; a minimum of six cafe-au-lait spots, each with a diameter of at least 1.5 centimeters; and multiple subcutaneous neurofibromas. Scoliosis was present in thirty-seven patients (26 per cent), most commonly associated with cafe-au-lait spots (thirty-five patients). In many of the patients with scoliosis there were associated medical and surgical problems. Although

Anterior cervical discectomy with and without interbody bone graft.

Fifty-one patients with symptomatic cervical disc disease refractory to conservative management were allocated at random to one of two treatment groups. The standard anterior approach devised by Cloward was used for 25 patients, and radical discectomy and foraminotomy for the other 26. All patients were followed for 6 months or longer with interview, physical examination, and radiographic evaluation. There was no difference in the success rate between the two groups. The large majority (92%) of patients in both groups were pleased with results of their operation. Because of technical factors r

Late results of cervical disc surgery.

Late results of cervical disc surgery have been reported and statistically studied in 383 cases: 83% were lateral discs, 13% were central spondylosis discs, and 4% central soft discs. Central spondylosis occured at a higher spinal level, and caused cord compression with or without weakness of the hands, but no pain. A posterior approach was used in all lateral discs, and either an anterior or a posterior approach, with or without fusion, for central discs. Preoperative myelography was always done and is recommended postoperatively in central disc surgery to evaluate the results. Our results we

Biomechanical analysis of clinical stability in the cervical spine.

This study was undertaken because there is a dearth of objective information in the literature on the clinical instability of the cervical spine below C2. To our knowledge, it is the first biomechanical investigation designed to analyze clinical stability. We have carried out a quantitative analysis of the behavior of the spine as a function of the systematic destruction of various anatomic elements. Under controlled conditions designed to maintain the biological integrity of the specimens, 17 motion segments from 8 cervical spines were analyzed. The spines were studied with either flexion or

Hematogenous candida osteomyelitis. Report of three cases and review of the literature.

Candida osteomyelitis of the spine and intervertebral disc developed in three patients without evidence of back trauma of overlying cutaneous infection. Two patients were prone to the development of disseminated candidiasis by the use of multiple antibiotics and other predisposing modalities following abdominal surgery. One patient had no identifiable cause for development of the infection. The diagnosis was established in all three cases by x-ray evidence of osteomyelitis and culture from needle aspirate. Two patients had bone scans consistent with infection. Each patient received different t

Intraosseous neurilemmoma of the cervical spine causing paraparesis and treated by resection and grafting.

A neurilemmoma presented as an expanding lesion of the bodies of C6 and C7 vertebrae and caused local neurological signs together with a spastic paraparesis. This tumour was treated by preliminary posterior fusion, followed by its complete removal via an anterior approach and stabilization by anterior spinal fusion. Other cases in the literature are reviewed and discussed.

[Transoral approach for the treatment of atlanto-axial dislocation with the Os odontoideum and obstruction of the bilateral vertebral arteries (author's transl)].

The two approaches are well-known in the surgical treatment of atlanto-axial dislocation, the one is the anterior and the other is the posterior approach. The indication of these methods is still controversial. This report concerns with one case of atlanto-axial dislocation with the separate odontoid and bilateral obstruction of both vertebral arteries, which was successfully treated by transoral decompression and fusion between the bodies of the atlas and axis. The reasons why we chose this method were: (1) the spinal cord decompression was necessary because of the presence of compression of

Cerebro-costo-mandibular syndrome. A new familial developmental disorder.

Three sibs with a hitherto unreported syndrome are described, the main features being mental handicap, palatal defects, micrognathia, and severe costovertebral defects, involving segmentation of most ribs and fusion of their dorsal ends to the vertebral bodies. In addition one infant had hypoplasia of an elbow together with defects of sacrum and coccyx; she and one other sib had minor dental defects. The syndrome is potentially lethal in the neonatal period; one of the sibs has survived. The inheritance is probably autosomal recessive.
