Age at Onset Predicts Motor and Non-motor Severity in Cervical Dystonia.

BACKGROUND: Cervical dystonia (CD) is the most common focal dystonia and shows significant heterogeneity in both motor and non-motor symptoms. Although motor symptoms typically prompt clinical presentation, non-motor features are the principal determinants of quality of life. Previous efforts to classify CD have focused on selected motor and non-motor clinical characteristics, suggesting distinct subtypes with potential prognostic implications. OBJECTIVE: We employed cluster analysis to identify phenotypic subgroups within a large CD population with detailed motor and non-motor assessments.

Successful Treatment of Cervical Dystonia With Temporary Peripheral Nerve Stimulation of the Medial Branch Nerves: A Case Report.

BACKGROUND: Cervical dystonia can be a difficult source of neck pain to treat, with pain from both nociceptive and nonnociceptive sources. Botulinum toxin as the mainstay of treatment carries approximately 10% failure rate in these patients, leaving them with debilitating pain. Peripheral nerve stimulation (PNS) is a proven pain management technique, and until now, PNS has not been reported as a treatment for refractory cervical dystonia. CASE REPORT: We present a case where temporary PNS is successfully used to treat refractory cervical dystonia by targeting the cervical medial branch nerves

The motor endplate band as an optimal target for botulinum toxin in congenital torticollis: evidence from a rabbit model.

BACKGROUND: Botulinum neurotoxin type A (BoNT-A) is an established minimally invasive treatment for congenital muscular torticollis (CMT); however, current injection strategies rely on empirical anatomical landmarks rather than precise functional targets. Accurate localisation of the sternocleidomastoid muscle (SCM) motor endplate band may improve treatment efficacy by optimising toxin delivery. We aimed to investigate the therapeutic efficacy of BoNT-A injection targeting the motor endplate band of the SCM, as localised by acetylcholinesterase (AChE) whole-muscle staining, in the treatment of

Cervical Dystonia Treatment with Injections Supplemented by Transcranial Magnetic Stimulation: The D-TWIST Pilot Randomized Controlled Trial.

BACKGROUND: For some patients with cervical dystonia (CD), botulinum toxin (BoNT) injections last less than 12 weeks. OBJECTIVES: To determine if a novel, accelerated repetitive transcranial magnetic stimulation (rTMS) protocol is a safe and feasible adjunctive treatment with BoNT for CD. METHODS: Patients with CD reporting BoNT benefit lasting ≤9 weeks were enrolled in a randomized double-blind, sham-controlled, crossover study. The Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS), mood, cognitive, and gait measures were collected before, after, and 2-weeks following rTMS/SHAM.

Fibromatosis Colli Presenting as a Left-Sided Cervical Mass: A Case Report.

Fibromatosis colli is a rare, benign condition of infancy characterized by fibrous enlargement of the sternocleidomastoid (SCM) muscle. It typically presents during the first several weeks of life as a painless cervical mass and may be associated with congenital muscular torticollis (CMT). Ultrasonography is the preferred initial imaging modality; however, variable sonographic appearances may complicate recognition. A two-week-old female patient, with an uncomplicated neonatal course, presented for a routine well-child examination and was found to have a new, painless, left-sided cervical mass

How Do I: How and When Do I Inject Suboccipital Muscles in Cervical Dystonia.

Cervical dystonia (CD) is characterized by involuntary, sustained or intermittent contractions of neck muscles, leading to abnormal postures, often associated with pain or tremor. Utilizing the COL-CAP framework, the Caput subtype describes the head motion resulting from the action of muscles on the occipito-atlantal (C0-C1) and atlantoaxial (C1-C2) joints; while the Collis subtype describe the neck motion induced by muscle action on the cervical spine (C2-C7). Co-existence of various Caput and Collis subtype combinations in CD is well known. The suboccipital muscles are significant drivers of

Optimizing treatment for cervical dystonia: botulinum toxin alone or combined with physical therapy?

BACKGROUND: Idiopathic cervical dystonia (ICD) is a chronic movement disorder characterized by involuntary neck muscles contractions, leading to pain, disability, and poor quality of life. While botulinum toxin (BTX) type A is the first-line treatment, its efficacy on non-motor symptoms is limited. Physical therapy (PT) may act synergistically and improve functional outcomes. OBJECTIVE: To evaluate whether BTX with tailored PT yields superior clinical outcomes compared to BTX alone in ICD patients, focusing on motor severity, disability, pain, mood, and quality of life. METHODS: This is a mon

Genotypic and radiological expansion of CONDSIAS.

Neurodegeneration, childhood-onset, stress-induced, with variable ataxia and seizures, is a rare genetic cause of childhood neuroregression and ataxia. A girl in early childhood, born of third-degree consanguinity, presented with paroxysmal torticollis starting around 2 years of age, which worsened subsequently, accompanied by ataxia, titubation, dystonia, dyskinesias, seizures and encephalopathy. The brain MRI initially showed signal abnormalities in the periventricular white matter and bilateral putamina, with restricted diffusion seen in the latter. While the putaminal changes disappeared i

Deep Brain Stimulation of the Internal Pallidum Modulates the Mesencephalic Locomotor Pathway in Improved Cervical Dystonic Patients.

BACKGROUND: Deep brain stimulation of the internal globus-pallidus (GPi-DBS) improves focal dystonia, often incompletely. GPi-DBS modulates pallido-thalamo-cortical pathways; however, involvement of descending pathways, including the mesencephalic locomotor region (MLR), has been proposed. OBJECTIVES: To investigate structural connectivity for predicting outcomes. METHODS: We retrospectively analyzed 21 focal dystonia patients with GPi-DBS. Dystonia was assessed pre- and postoperatively. Contacts were localized, volumes of tissue activated (VTA) modeled, and structural connectivity quantifie

Spinal deformity following surgery for paediatric intramedullary spinal cord tumours.

OBJECTIVE: Spinal deformity (SD) in children with intramedullary spinal cord tumours (IMSCT) can occur at presentation or postoperatively due to neurogenic and mechanical factors on a background of future spinal growth. Deformity is associated with pain, functional decline and reduced quality of life but spinal instrumentation can compromise MRI tumour surveillance. This study aims to identify risk factors for SD amongst children operated for low-grade IMSCT to identify any modifiable risk factors and help guide pre-operative counselling and surveillance. METHODS: Cases of benign IMSCT in pat

Factors influencing botulinum toxin response in cervical dystonia: a two-center GAS-based study.

BACKGROUND: Botulinum toxin (BoNT) is first-line therapy for cervical dystonia (CD), yet treatment response is heterogeneous. Patient-centered outcomes may be better captured by Goal Attainment Scaling (GAS) than by examiner-rated motor scales. We aimed to identify clinical factors influencing BoNT response in CD using GAS. METHODS: We conducted a retrospective two-center study including adults with CD treated with BoNT between January 2011 and August 2025. Treatment response was assessed using GAS T-scores. The primary outcome was achieving GAS ≥ 50 after the first session; the secondary out

Spatial signature of low-frequency network changes accounts for pallidal stimulation outcome in cervical dystonia.

BACKGROUND: Pallidal deep brain stimulation (DBS) has remarkable effects in patients with cervical dystonia. Yet, its neurophysiological mechanisms are not fully resolved to date. Converging evidence suggests that pallidal DBS modulates sensorimotor and cerebellar network activity in dystonia, possibly by disrupting pathologically enhanced low-frequency oscillations in the basal ganglia. Still, anatomical and electrophysiological findings have rarely been linked, and it is unclear whether oscillatory changes occur in the same network identified in neuroimaging studies. METHODS: In this cross-
