Factors Associated With Nocturnal Hypoglycemia After Exercise in the Type 1 Diabetes and Exercise Initiative (T1DEXI).

INTRODUCTION: The respiratory tract is one of the main points of entry of foreign substances into the body. Because of its location, the respiratory tract is heavily exposed to harmful agents, such as gases, vapors, or aerosols. AIM: Our objective was to evaluate the symptoms of occupational rhinitis in workers exposed to cotton dust. METHOD: The prospective study population consisted of workers from the "Nova Esperança" Cooperative of Nova Odessa (Sao Paulo), who were studied between September and December 2008. Data were collected through an individually and privately answered questionnair

Morphometric characteristics and intradural connections of the spinal root of accessory nerve in human fetuses.

Intradural connections between the spinal root of accessory nerve (SRAN) and the posterior roots of spinal nerves in the cervical region are clinically significant for torticollis, occipital neuralgia and surgeries of the craniocervical region. However, information on these connections during the fetal period is limited. This study aimed to examine the morphology and morphometry of the SRAN in fetuses and its intradural connections with the posterior roots of spinal nerves in the cervical region. Twenty-nine fetuses (58 sides; 18 to 35 gestational weeks) fixed in 10% formalin, were bilaterally

Botulinum Toxin Type A in Spasticity and Cervical Dystonia: Practical Clinical Recommendations from a Mexican Multidisciplinary Panel of Specialist Injectors.

Cervical dystonia and spasticity are debilitating neuromuscular disorders that substantially impair quality of life. Botulinum toxin type A (BoNT-A) is a cornerstone therapy; however, heterogeneity in dosing, muscle selection, and guidance techniques can limit outcomes, and Mexico-specific practical guidance is limited. We convened a multidisciplinary panel of Mexican rehabilitation and neurology specialists with extensive experience in BoNT-A injection; recommendations were developed through a three-phase process: a targeted literature search, a structured online survey of Mexican rehabilitat

The Key to Early Diagnosis: Retropharyngeal Abscess Masked by Neck Extension Limitation in a Pediatric Patient.

Retropharyngeal abscess (RFA) is a rare but potentially life-threatening deep neck infection in children and may lead to airway obstruction, mediastinitis, vascular complications, and other severe outcomes. In the pediatric population, RFA typically develops as a result of lymph node suppuration in this region secondary to an upper respiratory tract infection. We present the case of an 8-year-old boy who presented to the pediatric emergency department with sore throat, dysphagia, and nasal congestion. Because restricted neck extension raised suspicion for a deep neck infection, the patient was

Nonmotor symptoms of craniocervical dystonia: a multicentre international study-using the Dystonia nonmotor symptoms questionnaire.

Non-motor symptoms (NMS) are increasingly recognized as significant contributors to the overall burden of craniocervical dystonia (CCD), yet they remain underdiagnosed. This international, multi-centre observational study aimed to evaluate the prevalence and burden of NMS in 167 patients with adult-onset idiopathic CCD across Poland, the UK, and Germany, using the validated Dystonia Non-Motor Symptoms Questionnaire (DNMSQUEST). Participants were assessed at the end of their botulinum neurotoxin treatment cycle. The majority of patients (94.6%) reported at least one NMS, and over half experienc

How I do it: pediatric lateral-type posterior fossa ependymoma resection.

BACKGROUND: Lateral-type ependymomas, or cerebellopontine ependymomas, of the posterior fossa are rare tumors extending into the cerebellopontine angle, encasing important neurovascular structures, presenting as a particular surgical challenge. METHOD: We performed a radical resection of lateral-type ependymoma in a 2-year-old patient with anamnesis of torticollis and vomiting. Resection of the tumor was preceded by insertion of external ventricular drainage. CONCLUSION: Optimal treatment improving prognosis requires achieving gross total resection of the tumor.

Pain-Specific Benefits of Higher-Dose Neubotulinum Toxin Type A in Cervical Dystonia: A Multicenter Randomized Double-Blind Crossover Trial.

Pain is a major contributor to disability in cervical dystonia (CD) and may persist despite improvement in abnormal head posture after botulinum toxin treatment. Whether increasing the dose of Neubotulinum toxin type A (Neu-BoNT-A) provides additional analgesic benefit without compromising safety remains uncertain. In this prospective, multicenter, randomized, double-blind, two-period crossover trial, 50 adults with CD were randomized 1:1 to Neu-BoNT-A 100 U followed by 50 U or 50 U followed by 100 U, with each treatment period lasting 12 weeks. The principal between-dose TWSTRS analysis used

A case report of postoperative atlantoaxial rotatory subluxation following craniotomy for pilocytic astrocytoma in a pediatric patient.

BACKGROUND: Pilocytic astrocytoma is a benign, slow-growing pediatric brain tumor, while Grisel's syndrome refers to nontraumatic atlantoaxial subluxation classically associated with infection or head-and-neck surgery. Postoperative atlantoaxial rotatory subluxation following intracranial surgery is rare and poses significant diagnostic and management challenges. CASE DESCRIPTION: We report a 7-year-old girl who presented with progressive headaches, imbalance, and signs of obstructive hydrocephalus. Imaging revealed a large cystic pilocytic astrocytoma involving the left frontotemporo-parieta

Joint mobilization combined with cervical resistance training for pediatric atlantoaxial rotatory subluxation: Protocol for a randomized controlled trial with musculoskeletal ultrasound assessment.

BACKGROUND: Atlantoaxial rotatory subluxation (AARS) is a common cause of acquired painful torticollis in children. Conservative treatment is widely used, but high-quality randomized evidence for structured muscle-focused rehabilitation is limited. Musculoskeletal ultrasound, especially shear-wave elastography (SWE), can measure cervical muscle stiffness in a non-invasive way. This trial aims to test the efficacy and safety of joint mobilization combined with cervical resistance training in children with AARS. It will also examine whether ultrasound-based measures can detect treatment-related

Cervical dystonia as the presenting symptom of novel calcium voltage-gated channel subunit alpha1A mutation.

A woman in her mid-50s presented with an uncomfortable pulling sensation in the right neck that worsened with movement. This was diagnosed as cervical dystonia and treated symptomatically for several years through trials of botulinum toxin. As the years progressed, her symptoms worsened and evolved with the patient eventually experiencing debilitating ataxia requiring use of a wheelchair for mobility, severe dysphagia requiring the use of a feeding tube for nutrition and dysarthria requiring the need of a family member for translation. Through genetic testing, the patient was diagnosed with sp

Surgical Management of Congenital Muscular Torticollis in a Child Using Z-plasty: A Case Report and Literature Review.

Congenital muscular torticollis (CMT) is characterized by unilateral shortening of the sternocleidomastoid muscle (SCM), producing ipsilateral head tilt and contralateral rotation of the face and chin. It usually manifests during infancy and responds favorably to conservative treatment based on physiotherapy; spontaneous resolution is expected in the majority of cases. However, when diagnosis is delayed and patients present at a later age, conservative approaches tend to yield limited outcomes, neglected presentations remain prevalent in developing countries such as Morocco, where patients oft

Postural Torticollis and Early-Onset Scoliosis in Infants With SMA Identified Through Newborn Screening: Single Institution Experience.

INTRODUCTION/AIMS: In the era of disease modifying treatments (DMT), early-onset scoliosis may occur despite administration to infants with spinal muscular atrophy (SMA). We hypothesize that postural torticollis (Ptort), defined as tilt and/or rotation of the head to the same side due to an assumed asymmetry in cervical muscle strength, could be playing a role in early-onset scoliosis (EOS). The aim of this study was to assess the frequency of Ptort and EOS in infants with SMA. METHODS: Clinical data was collected retrospectively on infants with SMA identified by newborn screening (NBS) seen

Cervicothoracic Tuberculous Spondylitis in a Toddler: A Case Report.

Background and Importance Tuberculous spondylitis (or Pott's disease) is a common form of skeletal tuberculosis. Although primarily treated with tuberculostatic drugs, spinal surgery is necessary in patients with deformities or spinal cord compression. Literature describing surgery strategies in very young children remains scarce. Clinical Presentation A 20-month-old girl presented with torticollis without neurological deficits. Magnetic resonance imaging showed a cavitating mass, with destruction of the C7, T1, and T2 vertebrae. A combined anterior cervical and sternotomy approach was used to

PAH-associated health risk from ultrafine particles: an urban field study.

Reteplase (Retavase) is a plasminogen activator, mimicking endogenous tissue plasminogen activator (t-PA), a serine protease, converting plasminogen to plasmin and thereby precipitating thrombolysis. It is a third-generation recombinant form of t-PA that operates in the presence of fibrin (i.e. it is fibrin specific). Reteplase can be administered as a bolus dose (nonweight-based), rather than an infusion, which promotes rapid and safe administration. The ease of administration of this reteplase dosage regimen (two 10U bolus doses, each over 2 minutes, 30 minutes apart) is conducive to prehosp

Aplasia Cutis Congenita in Bart's Syndrome: A Case Report and Literature Review.

Aplasia cutis congenita (ACC) is a partial or complete absence of skin layers which can be a part of a syndromic disease. Bart's syndrome is a combination of clinical manifestations including ACC, epidermolysis bullosa, blistering in the oral mucosa, nail dystrophy, or congenital absence of nails. Additional anomalies, such as pyloric atresia, a broad nasal root, hypertelorism, and ear abnormalities, may also be present. We aimed to raise awareness about concomitant anomalies with ACC. A 23-year-old female with gravid 3 para 2 gave birth to a male fetus of 36 weeks and 3 days who had multiple

Congenital Muscular Torticollis as a Potential Neonatal Marker of Autism Spectrum Disorder and Other Neurodevelopmental Conditions.

BACKGROUND: Early intervention consistently improves developmental outcomes in children with autism spectrum disorder (ASD) and related conditions, underscoring the importance for timely recognition. Identifying early markers of atypical neurodevelopment is therefore critical. Congenital muscular torticollis (CMT), unilateral shortening of the sternocleidomastoid muscle causing infant head tilt, shares prenatal and perinatal risk factors with ASD and other developmental abnormalities. This overlap suggests that CMT may signal early disruption in neural development, potentially enabling earlier
