Safety of the M72/AS01 Tuberculosis Vaccine in People Living With HIV: A Systematic Review and Meta-Analysis.

BACKGROUND: Tuberculosis (TB) remains a leading cause of morbidity and mortality among people living with HIV (PLHIV). Although M72/AS01 demonstrated efficacy against active TB in HIV-negative adults, its safety in PLHIV is uncertain. As AS01 induces strong innate immune activation, evaluating tolerability in immunocompromised individuals is essential. We assessed the safety of M72/AS01 in PLHIV receiving antiretroviral therapy (ART). METHODS: We systematically searched MEDLINE, Embase, CENTRAL, and Web of Science for randomized controlled trials (RCTs) of PLHIV receiving M72/AS01 or placebo

[Guideline for the diagnosis and treatment of psoriasis arthropathica (psoriatic arthritis) in China (2026 edition)].

Psoriasis arthropathica, also known as psoriatic arthritis (PsA), is a highly heterogeneous systemic inflammatory disease. In addition to involving the skin with characteristic psoriatic lesions such as erythematous scaly plaques, it may also affect the joints, leading to joint pain, swelling, stiffness, and functional impairment, thereby substantially reducing patients' quality of life. To develop clinical practice guidelines that align with international standards for guideline development while reflecting Chinese clinical practice, the China Dermatologist Association, in collaboration with

SAPHO Syndrome Presenting With Severe Inflammatory Back Pain (Sacroiliitis) and Rare Retinol Associated Myopathy in an Iraqi Adolescent Male: A Case Report and Literature Review.

SAPHO syndrome is a rare autoinflammatory disorder characterized by synovitis, acne, pustulosis, hyperostosis, and osteitis. Although musculoskeletal and dermatologic manifestations are well recognized, extra-articular involvement remains uncommon, particularly muscular inflammation. We report the case of a 17-year-old male who presented with severe inflammatory low back pain and extensive nodulocystic acne for which he took escalating doses of isotretinoin. Clinical examination demonstrated positive sacroiliac stress tests, proximal muscle myopathy, and dermatologic evaluation confirmed acne

Lower CALLY index values are associated with higher disease activity in psoriatic arthritis: a retrospective cohort study.

Assessing disease activity in psoriatic arthritis (PsA) remains challenging, and additional biomarkers that can complement conventional inflammatory markers are still needed. The CRP-albumin-lymphocyte (CALLY) index integrates inflammatory, immune, and nutritional components into a single measure. This study examined the relationship between the CALLY index and disease activity in patients with PsA. This retrospective longitudinal cohort study included 150 patients with PsA and 50 age- and sex-matched healthy controls. Blood-derived inflammatory indices, including neutrophil-to-lymphocyte rati

Unveiling the Conundrum: Genital Tuberculosis with Omental Tubercles and Uterine Leiomyoma Masquerading as Malignancy.

Female genital tuberculosis (TB) is an uncommon form of extrapulmonary TB that may closely mimic advanced gynecological malignancy because of overlapping clinical, radiological, and biochemical findings. We report the case of a 70-year-old woman who presented with abdominal distension, pelvic pain, weight loss, elevated CA-125 levels, and omental nodularity suggestive of peritoneal carcinomatosis. Imaging revealed an enlarged uterus with an intramural mass and multiple peritoneal deposits, raising strong suspicion for ovarian or primary peritoneal malignancy. Diagnostic laparoscopy demonstrate

Predicting psoriatic arthritis in new-onset psoriasis: development of multivariable prediction models from an inception cohort study.

BACKGROUND: Psoriatic arthritis is underdiagnosed in people with psoriasis. Delayed psoriatic arthritis diagnosis is associated with worse outcomes, whereas early intervention improves prognosis. Therefore, we aimed to develop and internally validate two sets of prediction models. The first set intended to identify participants with new-onset psoriasis suitable for rheumatologist referral; the second set intended to identify participants with subclinical psoriatic arthritis at high risk of developing clinical psoriatic arthritis. We also aimed to identify psoriatic arthritis predictors. METHO

Why Do Some Therapies Work While Others Fail? Mechanisms of Response and Emerging Treatments in axial Spondyloarthritis: 2026 SPARTAN Annual Meeting Proceedings.

PURPOSE OF REVIEW: This review summarizes recent advances in the pathogenesis of axial spondyloarthritis (axSpA), examines why TNF, IL-17, and JAK inhibition is effective whereas IL-23 blockade has not demonstrated clinical efficacy, and highlights emerging therapeutic targets. RECENT FINDINGS: TNF, IL-17 A, and IL-17 F are key drivers of inflammation in axSpA and may also contribute to pathological new bone formation. HLA-B27 can present peptides to CD8+ T cells, potentially expanding pathogenic T-cell populations, including TRBV9+ (V9 T cell receptor beta chain) cells, that produce TNF and

Muscle MRI as an Imaging Biomarker of Muscle Damage in Patients With Spinal and Bulbar Muscular Atrophy.

BACKGROUND AND OBJECTIVES: Spinal and bulbar muscular atrophy (SBMA) is a rare neuromuscular disorder without effective treatments available. There is an unmet need to identify biomarkers reflecting underlying disease processes and sufficiently responsive to detect slow changes to enable future clinical trials. Muscle MRI is a responsive measure of muscle volume and fat content, a pathologic hallmark of SBMA that precedes functional changes. We therefore assessed muscle MRI as a potential imaging biomarker for detecting change in SBMA severity. METHODS: This was a noninterventional, longitudi

Elucidating the therapeutic mechanism of Gomisin G from Guomin Gecoction for psoriasis treatment through targeting the PI3K-AKT/JAK3-STAT3 pathway.

ETHNOPHARMACOLOGICAL RELEVANCE: Guomin Decoction (GMD) is a traditional Chinese medicine formula that has been empirically used for the treatment of inflammatory skin disorders, including psoriasis. However, its active constituents and underlying mechanisms remain poorly defined. Gomisin G (GomG), a representative bioactive component of GMD, possesses potent anti-inflammatory properties; yet, its role in psoriasis has not been established. AIM OF THE STUDY: This study aimed to identify the key anti-psoriatic component of GMD and clarify its mechanism of action. MATERIAL AND METHODS: We predi

Very late-onset Krabbe disease with concomitant dementia: case description and a critical review of the literature.

BACKGROUND: Krabbe disease (KD) is a rare autosomal recessive lysosomal storage disorder caused by pathogenic variants in GALC. Despite accounting only for 5% of forms, reports of adult-onset KD cases are increasingly described. METHODS: A female patient manifesting KD after the age of 60 years, presenting with spastic paraplegia and cognitive decline, is described. The scientific literature of KD with onset > 10 years has been extensively reviewed to refine the spectrum of later-onset KD manifestations. RESULTS: Including ours, we identified 84 KD adolescent/adult-onset patients (mean age at

Engineering the future of advanced therapy medicinal products: a bioengineering call to action.

Advanced Therapy Medicinal Products - cell therapies, gene therapies, and tissue-engineered products - are beginning to deliver on the promise of curative medicine: CAR-T therapies double survival in chemotherapy-refractory lymphomas, gene therapies reverse the natural history of spinal muscular atrophy and hemoglobinopathies, and Pluripotent Stem Cell (PSC)-derived islet transplantation renders type 1 diabetic patients insulin-independent. Yet the trajectory from proof-of-concept to equitable, scalable deployment is consistently impeded not only by unresolved biology but also by engineering,

A critical role for α7 nicotinic acetylcholine receptor signaling in myeloid lineage cells in chronic neuropathic and inflammatory pain responses.

Neuroinflammation influences the development of chronic pain, an increasingly prevalent condition impacting the lives of many worldwide. Given the challenges of managing chronic pain, it is essential to explore new neuroimmune targets for the development of safe and effective analgesics. α7 nicotinic acetylcholine receptors (α7 nAChR) have been explored for their role in pain and inflammation. It was hypothesized that α7 nAChR expression within myeloid cells mediates an anti-inflammatory response that mitigates pain states. Two mouse strains were engineered, with deletion of the α7 gene (Chrna

[Mechanism of traditional Chinese medicine manipulation in the treatment of lumbar disc herniation:a new perspective of mechanics and biochemistry].

Lumbar disc herniation (LDH) is a prevalent spinal disorder. As a non-pharmacological therapy in traditional Chinese medicine (TCM), manipulation is widely employed in the conservative management of LDH and has garnered considerable attention due to its notable clinical efficacy. This paper aims to review the mechanism of TCM manipulation in the treatment of LDH, and analyze the existing evidence from the multi-dimensional integration of "biomechanical stimulation, cell signal transduction, system physiological response". At the biomechanical level, TCM manipulation can adjust the spinal force
