Uncovering Heterogeneous Health Burden in Axial Spondyloarthritis Through Latent Class Analysis of the Assessment of SpondyloArthritis International Society Health Index.

OBJECTIVE: To identify and characterize unobserved subgroups of patients with axial spondyloarthritis (axSpA) based on response patterns to the Assessment of SpondyloArthritis international Society Health Index (ASAS HI) using latent class analysis (LCA). METHODS: We conducted LCA on 17 dichotomous ASAS HI items in a cohort of 180 patients with axSpA. Models with 2 to 6 classes were evaluated using the Bayesian information criterion (BIC) and Akaike information criterion, with the optimal model selected based on the lowest BIC. Class profiles were defined by conditional item endorsement proba

Systematic review and meta-analysis on comparing treatment approaches of cervical fractures in ankylosing spondylitis to determine improvement in outcomes.

BACKGROUND & OBJECTIVE: Anterior, posterior and combine surgical approach for treatment of cervical fractures in ankylosing spondylitis is debatable in current existing literature. There are recognized advantages and disadvantages associated with anterior, posterior, and combined surgical approaches to the cervical spine when treating fractures in patients with ankylosing spondylitis (AS). Despite growing surgical experience, the optimal approach with respect to neurological recovery, complication rates, and mortality remains unclear, and the choice is largely guided by surgeon expertise and p

Decoding autoimmune diseases through the adaptive immune receptor repertoires.

The adaptive immune receptor repertoire (AIRR), composed of T-cell receptors (TCRs) and B-cell receptors (BCRs), underpins antigen-specific immunity through extensive diversity generated by V(D)J recombination and junctional modification. Advances in high-throughput AIRR sequencing have enabled comprehensive characterization of clonal diversity, gene usage patterns, and antigen-driven selection at unprecedented resolution. In autoimmune diseases, breakdown of immune tolerance permits the persistence and expansion of autoreactive lymphocyte clones, and is associated with characteristic repertoi

Therapeutic Drug Monitoring of Golimumab to Predict Disease Activity and Antidrug Antibodies Status in Patients with Ankylosing Spondylitis.

BACKGROUND AND AIM: Golimumab is a fully human tumor necrosis factor-alpha inhibitor that is highly effective in the management of ankylosing spondylitis (AS). A major limitation of golimumab is nonresponse, resulting from low drug concentrations, which is primarily due to antidrug antibodies (ADAs). The aim of this study was to assess the trough concentration of golimumab and the prevalence of ADA formation in patients with AS. METHODS: An enzyme-linked immunosorbent assay was used to quantify golimumab trough concentrations and ADAs in 59 Iraqi patients with AS; AS disease activity was also

Short-term Efficacy of massage and related treatments for Ankylosing Spondylitis: A Systematic Review and Meta Analysis.

BACKGROUND: Ankylosing spondylitis (AS) is a chronic inflammatory rheumatic disease causing significant pain, stiffness, and functional impairment. Conventional treatments are limited by adverse events and poor efficacy for axial manifestations, leading AS patients to seek complementary therapies such as massage. However, no systematic reviews or meta-analyses have synthesized evidence on massage for AS, leaving its efficacy unconfirmed. METHODS: From inception to July 5, 2025, a comprehensive literature search was conducted using eight electronic databases to identify randomized controlled t

The causal association between Celiac disease and Ankylosing spondylitis: a two-sample Mendelian randomization analysis.

OBJECTIVE: The concept of the gut-joint axis is often mentioned when exploring the pathogenesis of Ankylosing spondylitis (AS). This study aims to explore whether there is a causal association between Celiac disease (CeD) and AS. METHOD: Two-sample Mendelian randomization (MR) analysis was conducted using the CeD dataset (the largest pure CeD phenotype dataset in the IEU OpenGWAS database at the time of data retrieval, n = 24,269) and the AS dataset (strictly defined, from the IEU OpenGWAS database, n = 218,030). Inverse Variance Weighting (IVW) was adopted as the primary MR analysis method,

Association between ankylosing spondylitis and hearing loss: A Mendelian randomization study.

Observational studies have suggested a potential link between ankylosing spondylitis (AS) and hearing loss. However, this association remains controversial due to potential drug-induced ototoxicity from treatments. Therefore, we employed a Mendelian randomization (MR) approach to evaluate the association between AS and hearing loss from a genetic perspective. This MR study utilized summary data from genome-wide association studies on AS and hearing loss, including sensorineural hearing loss (SNHL) and conductive hearing loss (CHL). The primary analysis was conducted using the inverse-variance

Effects of microtrauma on joint structural changes in the Curdlan-injected SKG mouse model of ankylosing spondylitis.

Although microtrauma is considered a potential environmental factor in the development of ankylosing spondylitis (AS), there is a lack of experimental research regarding its precise contribution to disease pathogenesis. To induce physiological microtrauma, the right ankles of SKG mice were immobilized for a week using surgical plaster and a pipette tip to stimulate compensatory mechanical overload in the contralateral limb. Following the removal of the immobilization, the mice were intraperitoneally injected with 3 mg of Curdlan. In this model, the non-immobilized left ankle served as the micr

Magnetic Resonance Imaging in Axial Spondyloarthritis: Beyond the Sacroiliac Joints-Focus on Facet Joint Involvement.

Axial spondyloarthritis (axSpA) is a chronic inflammatory disease of the sacroiliac joints (SIJs) and spine, encompassing non-radiographic axSpA (nr-axSpA) and radiographic axSpA (r-axSpA) or ankylosing spondylitis (AS). Early diagnosis is challenging, particularly in nr-axSpA, due to the absence of radiographic sacroiliitis and overlap with nonspecific back pain. Imaging plays a pivotal role in disease classification and monitoring. To synthesize current evidence on inflammatory and structural facet joint involvement in axSpA, with particular emphasis on MRI characterization, available scorin

Late-onset ankylosing spondylitis presenting with severe flank and abdominal-to-back pain in a 64-year-old man: a case report.

BACKGROUND: Ankylosing spondylitis (AS) is a chronic inflammatory disease that primarily affects the spine and sacroiliac joints and is typically diagnosed in younger adults. However, AS may be overlooked in older patients when symptoms are atypically localized, such as flank or abdominal-to-back pain, rather than classic inflammatory low back pain. CASE DESCRIPTION: We report the case of a 64-year-old man who presented with persistent left-sided abdominal-to-back pain accompanied by markedly elevated inflammatory markers. Initial evaluation focused on abdominal, urinary, infectious, malignan

High Burden and Phenotype-Specific Variability of Pain Polypharmacy in Early Autoimmune Rheumatic Diseases: A 14-Year Analysis.

BACKGROUND: Pain drives disability and medication use in autoimmune rheumatic diseases (ARDs), even when inflammation is controlled. The extent of pain-related polypharmacy across ARDs and chronic overlapping pain conditions (COPCs) remains poorly characterized. We aimed to quantify first-year pain medication burden following ARD diagnosis and examine variations by disease, COPC status, and time. METHODS: Using the Merative MarketScan Commercial Claims database, we identified adults newly diagnosed with rheumatoid arthritis, systemic lupus erythematosus, ankylosing spondylitis, psoriatic arth
