Automated grading and diagnosis of sacroiliitis on CT images using a 3D convolutional neural network: a multicenter retrospective study.

Radiographic grading of the sacroiliac joints plays a critical role in the differential diagnosis of ankylosing spondylitis (AS) and in guiding the treatment. The aim of this study was to develop an automated 3D convolutional neural network (3D CNN) for grading and diagnosing sacroiliitis on CT images to assist clinicians. This study included CT images from 2,144 participants, comprising healthy controls and patients with suspected ankylosing spondylitis (AS). A V-Net based segmentation model was applied, followed by training a three-dimensional DenseNet-based convolutional neural network (3D

The immature sacroiliac joints on cross-sectional imaging: developmental anatomy and diagnostic pitfalls.

The immature sacroiliac joint (SIJ) has developmental imaging appearances that frequently simulate sacroiliitis in children and adolescents evaluated for juvenile spondyloarthritis, and false-positive interpretation increases when adult MRI and structural criteria are applied. This review integrates developmental anatomy with MRI and CT findings and discusses CT-like MRI and MRI-based synthetic CT as structural adjuncts for cortical assessment in the immature SIJ. We emphasize the most relevant false-positive pathways in daily practice, including peri-physeal fluid-sensitive marrow hyperintens

Advanced MRI Sequences for Structural Lesion Assessment in Sacroiliitis.

Background/Objectives: Assessing structural damage in pediatric sacroiliitis is challenging, necessitating radiation-free alternatives to computed tomography (CT). This study evaluated the diagnostic performance of advanced MRI sequences-3D-MENSA (Multi-Echo in Steady-State Acquisition), 3D-MERGE (Multiple-Echo Recombined Gradient Echo), and Zero Echo Time (ZTE)-against conventional T1-weighted sequences for detecting structural lesions. Low-dose computed tomography (LDCT) served as the reference standard. A secondary objective was to qualitatively assess the visibility of active inflammatory

A Gated Attention-Based Multiple Instance Learning and Test-Time Augmentation Approach for Diagnosing Active Sacroiliitis in Sacroiliac Joint MRI Scans.

Background and Objective: Axial spondyloarthritis (axSpA) is a group of chronic inflammatory diseases that primarily affect the sacroiliac joints. Early diagnosis is crucial for preventing irreversible structural damage. Magnetic Resonance Imaging (MRI) is the gold standard for detecting early inflammatory changes such as sacroiliitis. However, conventional MRI interpretation is inherently subjective and susceptible to both intra- and inter-observer variability. Therefore, artificial intelligence (AI)-driven diagnostic solutions are increasingly being explored. Among them, the Gated Attention

A Probable Case of Postpartum Sacroiliac Joint Tuberculosis: The Diagnostic Value of Imaging in a High-Endemic Setting.

Tuberculous sacroiliitis is a rare form of osteoarticular tuberculosis and is particularly uncommon in the postpartum period. Its presentation is often nonspecific and can mimic more common postpartum musculoskeletal or infectious conditions, leading to delayed diagnosis, especially in high tuberculosis-endemic regions. We report the case of a 24-year-old woman presenting 10 days postpartum with severe, debilitating right-sided lower back pain and markedly elevated inflammatory markers. Initial evaluation revealed retained products of conception (RPOC), which were managed surgically and with b

Disseminated Skeletal Tuberculosis Presenting as Multifocal Axial and Peripheral Joint Disease: A Diagnostic Challenge.

Disseminated skeletal tuberculosis (TB) is an uncommon manifestation of extrapulmonary TB and often presents with non-specific musculoskeletal symptoms, contributing to diagnostic delay and an increased risk of irreversible structural damage. We report a 35-year-old male with diabetes mellitus who presented with two months of progressive nocturnal low back pain with functional limitation, followed by progressive painful swelling and restricted movement of the left elbow, and subsequently developed high-grade fever, weight loss, and constitutional symptoms. The coexistence of axial and peripher

The challenge of bringing advances in sacroiliac joint imaging to the clinic.

PURPOSE OF REVIEW: Recently reported criteria are more stringent for classification of axial spondyloarthritis (axSpA) in the absence of positive imaging. This places increased reliance on the accuracy of sacroiliac joint (SIJ) imaging. This review highlights some of the key challenges to bringing advances in SIJ imaging to clinical practice. RECENT FINDINGS: The first international consensus for an MRI acquisition protocol for sacroiliitis, published in 2024, defines how the sequences should be orientated, and which sequences are required to identify various lesions that may be seen in infla
