Coexistent Ankylosing Spondylitis and Ocular Toxocariasis in a Pediatric Patient Manifesting As Bilateral Panuveitis.

The coexistence of ankylosing spondylitis and ocular toxocariasis in the literature is rare and limited to a few case reports. Typically, such cases present as acute nongranulomatous anterior uveitis with Toxocara IgG seropositivity. A patient manifesting with findings of both ankylosing spondylitis and toxocariasis bilaterally has not been reported previously in the literature. We present a case of coexistent juvenile spondyloarthritis and ocular toxocariasis in a 16-year-old male presenting with generalized pustules, back pain, peripheral polyarthritis, and bilateral panuveitis. Both eyes di

Impact of combining magnetic resonance images and radiographs on radiographic grading of sacroiliitis and classification in axial spondyloarthritis: a reliability study.

To investigate the reliability of sacroiliac radiographic grading based on the modified New York criteria, both through grading pelvic radiographs alone and in combination with sacroiliac magnetic resonance imaging and the influence of alone and multimodal interpretation on the categorization of radiographic and non-radiographic axial spondyloarthritis (r-axSpA and nr-axSpA). Radiographs of the SI joints from patients with axSpA were graded by two rheumatologists independently in two sessions (radiographs only and MRI + radiographs) and followed by consensus sessions with the participation of

Hip Involvement in Pediatric Scurvy: Early Magnetic Imaging Signs.

Background: Scurvy is an uncommon and often underrecognized disease. However, conditions associated with a restrictive and/or selective diet and inadequate absorption still pose a high risk for developing vitamin C deficiency. Musculoskeletal symptoms are among the most characteristic manifestations of scurvy, often requiring radiological investigations. Objective: This study aims to describe the radiological signs of scurvy on pelvic magnetic resonance imaging (MRI) in children presenting with musculoskeletal symptoms and to highlight features that may help differentiate it from other conditi

Expanding the Genetic Framework: Insights into Non-HLA-B27 Contributions to Axial Spondylarthritis.

Background and Objectives: Spondylarthritis is a complex group of inflammatory diseases closely associated with the HLA-B27 antigen. However, the role of non-HLA-B27 alleles in the disease's pathogenesis has gained significant scholarly attention in recent years. Case presentation: This case study presents a 49-year-old male with a history of progressive inflammatory back pain, characterized by morning stiffness and restricted spinal mobility developed over several years. Initially presenting with non-specific symptoms, the patient eventually experienced persistent axial pain and deteriorating

Recognizing the Unexpected: A Rare Cause of Rice Bodies in Brucellosis.

Brucella is a gram-negative zoonotic agent transmitted by consumption of raw milk and infected meat. Among musculoskeletal manifestations, axial involvement such as spondylodiscitis and sacroiliitis is well documented, while peripheral manifestations like tenosynovitis and rice bodies remain underreported. In this case, flexor tenosynovitis and associated rice body involvement due to Brucella, which developed progressive swelling, pain, and restricted movement in the wrist and third finger of the hand after a minor abrasion, were investigated ultrasonographically. As far as we know, this is th

Isotretinoin-Induced Sacroiliitis: New Clinical Insights and Magnetic Resonance Imaging-Based Outcomes in a Real-Life Clinical Setting.

BACKGROUND: Isotretinoin, a frequently prescribed treatment for severe acne vulgaris, is associated with rare but clinically important musculoskeletal adverse effects, notably sacroiliitis. Although previous studies reported an association, comprehensive long-term data on clinical outcomes, imaging progression, and risk of evolution to axial spondyloarthritis (axSpA) remain limited. Basically, previous studies commonly excluded patients with spondyloarthritis-related risk factors such as chronic inflammatory back pain, but this limits the generalization. This study uniquely evaluated the clini

Intra-Abdominal Cystic Lymphangioma in a 30-Year-Old Male: A Case Report and Review of Management Strategies.

BACKGROUND: Although the use of inflammatory markers in diagnosing Brucella-related complications has been the subject of research, studies on osteoarticular disease are insufficient, especially in children. This study aimed to compare inflammatory markers in children diagnosed with brucellosis, distinguishing between those with and without osteoarticular involvement (OI). METHODS: In this retrospective study, patients diagnosed with brucellosis from 1 month to 18 years of age were evaluated. Data collected included age, gender, OI, treatment duration, complete blood count, inflammatory marke

Ultrasound-Guided (USG) Aspiration From Sacroiliac Joint in Tuberculosis Is Rare: An Atypical Case Report From a Developing Country.

Tuberculosis (TB) in the musculoskeletal system is rare, though it is an emerging health concern in many developing countries. Sacroiliac joint (SIJ) TB is uncommon. This report highlights the use of ultrasound-guided (USG) aspiration to diagnose SIJ TB. We report a 23-year-old boy who presented with low back pain and localized tenderness over the SIJ. Initial imaging showed signs of inflammation. USG aspiration was performed successfully to obtain fluid for microbiological analysis, and laboratory investigations confirmed the presence of Mycobacterium tuberculosis. Sacroiliac TB often present

Effect of Nonsteroidal Anti-Inflammatory Drugs on Sacroiliac Joint Inflammation, as Seen on Magnetic Resonance Imaging, in Axial Spondyloarthritis.

OBJECTIVE: Imaging evidence of active sacroiliitis is important for diagnosis, classification, and monitoring of axial spondyloarthritis (axSpA). However, there is no consistent guidance on whether patients should temporarily stop nonsteroidal anti-inflammatory drugs (NSAIDs) before magnetic resonance imaging (MRI). The aim of this study was to determine whether NSAIDs lead to an underestimation of active sacroiliitis, as observed using MRI. METHODS: Adults with axSpA were recruited from rheumatology clinics and undertook NSAID washout for one to two weeks before a sacroiliac joint MRI scan.

Systematic Review on Minimally Invasive Posterior Sacroiliac Joint Fusion.

BACKGROUND: Sacroiliac joint dysfunction contributes to a significant proportion of lower back and leg pain and associated functional deficiencies. Following failure of conservative methods, lateral fusion approaches have previously provided the mainstay for surgical stabilization of the joint. In recent years there are multiple devices available for sacroiliac fusion via posterior or posterolateral oblique approaches, thereby reducing neurovascular complications of the lateral approach. OBJECTIVES: To provide a comprehensive review of the evidence for pain and functional outcomes in patients

Misdiagnosis of sacroiliac joint gout as ankylosing spondylitis: Solving the diagnostic dilemma with dual-energy computed tomography.

Gout rarely affects the axial joints, and sacroiliac joint involvement is exceptionally uncommon.1 This report describes the case of a 30-year-old female with a family history of gout who had recurrent knee swelling and low back pain for 2 years, initially misdiagnosed with ankylosing spondylitis. Laboratory findings showed episodic hyperuricemia and elevated inflammatory markers, while MRI revealed bilateral sacroiliitis and bone island in the right sacroiliac joint. HLA-B27 was negative, and no family history of psoriasis or ankylosing spondylitis was noted. The atypical presentation of infl

CT Angiographic Evaluation of Superior Gluteal Artery Branches in Lateral Sacroiliac Joint Fusion: A Pilot Study to Inform Preoperative Surgical Planning and Anatomic Parameters.

INTRODUCTION: The sacroiliac (SI) joint can be a source of low back pain. Surgical techniques for fusion include lateral transiliac, posterolateral, sacro-alar iliac and posterior interpositional fixations. Although these various trajectories are supported by the literature, the lateral transiliac approach is the most extensively studied. However, this technique has been associated with concern for potential injury to branches of the superior gluteal artery (SGA). Currently, no preoperative workup includes the evaluation of SGA and its branches for location and potential injury during lateral

Using the Deep Learning Algorithm to Determine the Presence of Sacroiliitis from Pelvic Radiographs.

Deep learning (DL) techniques have demonstrated remarkable capabilities in recognizing complex patterns in medical imaging data. In recent years, DL has been increasingly applied in clinical medicine for disease diagnosis and progression prediction. This study aimed to develop and validate a DL model for detecting sacroiliitis using pelvic anteroposterior (AP) radiographs. We retrospectively analyzed 1853 patients with pelvic AP radiographs, including 3706 sacroiliac joints (SIJs). Pelvic AP radiographs served as input data for the DL model development, while the presence or absence of sacroil

FPGA implementation of deep learning architecture for ankylosing spondylitis detection from MRI.

Ankylosing Spondylitis (AS), commonly known as Bechterew's disease, is a complex, potentially disabling disease that develops slowly over time and progresses to radiographic sacroiliitis. The etiology of this disease is poorly understood, making it difficult to diagnose. Therefore, treatment is also delayed. This study aims to diagnose AS with an automated system that classifies axial magnetic resonance imaging (MRI) sequences of AS patients. Recently, the application of deep learning neural networks (DLNNs) for MRI classification has become widespread. The implementation of this process on co
