Classical rheumatoid arthritis in a patient with Reiter's syndrome.

A 44 year old white male presented with sterile urethritis, anterior uveitis, painless oral ulcers, low back pain, and synovitis of the knees and left ankle. Unilateral sacroiliitis and limited spondylitis further supported a diagnosis of Reiter's syndrome. Serum rheumatoid factor was present. The patient subsequently developed morning stiffness, symmetrical polyarthritis of the hands and wrists, and an olecranon subcutaneous nodule. Histology of the nodule and synovium was consistent with rheumatoid arthritis. Histocompatibility antigen typing established the presence of HLA B27. The occurren

Sacroiliitis in severe psoriasis.

In 100 consecutive patients admitted to the hospital for control of severe psoriasis, 25 had radiologic changes characteristic of psoriatic arthritis. Twenty had erosions and mild sclerosis around the sacroiliac joint; in eight of these, asymptomatic sacroiliac changes were the sole manifestation of musculoskeletal disease. Twelve had vertebral syndesmophytes, 11 had terminal interphalangeal joint changes, and 10 had peripheral joint involvement.

Histocompatibility antigens in inflammatory bowel disease. Their clinical significance and their association with arthropathy with special reference to HLA-B27 (W27).

Histocompatibility (HLA) antigen phenotypes have been studied in 100 patients with ulcerative colitis, 100 with Crohn's disease, and 283 normal controls. In addition the incidence of ankylosing spondylitis, sacroiliitis, and "enteropathic" peripheral arthropathy was determined in the patients with inflammatory bowel disease (IBD). There was no significant difference in antigen frequency between patients and controls. However, the incidence of HLA-B27 was increased in the patients complicated by ankylosing spondylitis and/or sacroiliitis in both ulcerative colitis and Crohn's disease. In contra

HLA-B27 antigen in diagnosis of atypical seronegative inflammatory arthropathy.

Eighteen patients with an inflammatory pauciarticular peripheral arthropathy not typical of any known entity showed an asymmetrical pattern of disease, with a predilection for the lower extremities. Destructive joint changes and deformities were absent (mean follow-up 10 years). Although spondylitis and sacroiliitis were absent on x-rays, HLA-B27 antigen was found in 8 patients (44%). This antigen and similar joint symptoms are also found in other forms of arthritis.

Histocompatibility antigens in psoriatic arthritis.

Histocompatibility (HL-A) antigen typing was performed on 82 patients with psoriatic arthritis. The prevalance of HL-AB27 was increased (28%) compared with that in a control group (7-5%), and this was most marked in those with spinal changes. The prevalence in patients with sacroiliitis and syndesmophytes was 75%, with sacroiliitis alone 78%, but with syndesmophytes and normal sacroiliac joints only 36%. The overall prevalence in psoriatic spondylitis was 60%, with a prevalence of 71% in those fulfilling the New York criteria for anklosing spondylitis. The prevalence was not increased in those

HLA and juvenile chronic polyarthritis.

HLA antigen have been identified in patients with juvenile chronic polyarthritis (J.C.P.) (n = 35). In J.C.P. the incidence of antigen B27 (57%) was found to be higher than in a normal population (n = 1,000). On recent evaluation of the clinical status, serology and x-rays, the patients with juvenile chronic polyarthritis who has been followed up many years, could be subdivided into four groups: Group 1: J.C.P. evolving to ankylosing spondylitis (n = 3); Group 2: J.C.P. with sacroiliitis (n = 17); Group 3: J.C.P. without sacroiliitis (n = 9); Group 4: juvenile rheumatoid arthritis characterise

HL-A antigens and sacroiliitis.

Lymphocyte typing for HL-A B27 is useful under conditions in which data based on history and examination are suggestive but not diagnostic of a seronegative spondyloarthropathy. The presence of HL-A B27 would increase the probability of a patient's having one of these entities, but its absence does not rule out such a diagnosis. Furthermore, tissue-typing allows one to predict the probability that a patient with inflammatory bowel disease will develop AS and the likelihood that the family member of a patient with AS will develop a related disease.

"Ankylosing spondylitis" without sacroiliitis in a woman without the HLA B27 antigen.

An elderly woman with otherwise typical ankylosing spondylitis for 45 years lacked radiologic evidence of sacroiliitis and the HLA B27 antigen. The illness was complicated by renal tuberculosis requiring a left nephrectomy 23 years after the onset of low back pain, and 20 years after an episode of severe iritis. After the eradication of the tuberculosis by surgery and chemotherapy, she has continued to have symptomatic spondylitis. The case seems to be an exception to the rule that sacroiliitis is a sine qua non for ankylosing spondylitis. Women with ankylosing spondylitis tend to have milder

HL-A antigens and sacroiliitis.

The histocompatibility antigen W27 has been found to have a high incidence in ankylosing spondylitis (96%), Reiter syndrome (74%), and other forms of arthritis that may involve the axial skeleton. In patients with Reiter syndrome, there was no correlation between the presence of W27 antigen and the extent or duration of the symptoms. Sacroiliitis was predominantly found in those patients with W27 antigen in all the axial arthropathies. Tissue-typing techniques may be of value in detecting early or atypical disease.

Investigation of sacroiliac disease: Comparative evaluation of radiological and radionuclide techniques.

An attempt has been made to improve diagnostic precision in a group of diseases associated with inflammation of the sacroiliac joints, by using a 99mTechnetium stannous pyrophosphate bone scan. Inflammation of these joints is associated with osteoblastic activity and is reflected by an increase in the uptake of radionuclide, which can be precisely quantitated. The uptake was markedly above the range of normal in patients with active ankylosing spondylitis (AS), and also in a number of patients with possible AS, psoriasis, and Reiter's syndrome. However, patients with Grade 4 radiological chang

HL-A antigens in patients with persistent palmoplantar pustulosis and pustular psoriasis.

HL-A typing was performed in a series of 22 patients with persistent palmoplantar pustulosis, 14 patients with palmoplantar pustulosis and psoriasis, 15 patients with generalized pustular psoriasis and 45 patients with uncomplicated psoriasis. None of the patients with persistent palmoplantar pustulosis alone had relatives with psoriasis, which was a common finding in the other groups. The HL-A antigens 13 and 17 were markedly increased in the group of uncomplicated psoriasis. In the group of patients with persistent palmoplantar pustulosis alone neither of these antigens was increased, as com

HL-A 27 and acute anterior uveitis.

51 (30 men and 21 women) of 90 consecutive patients with acute non-granulomatous anterior uveitis were HL-A 27 positive. This frequency of 55-7 per cent compares with 8-2 per cent in controls. Twenty-three patients (18 men and 5 women) had in addition evidence of systemic disease, including ankylosing spondylitis, sacroiliitis and Reiter's syndrome, sometimes associated with psoriasis. Twenty-eight of 63 patients without evidence of systemic disease were HL-A 27 positive, suggesting that the uveitis in many of these cases has a similar aetiology to those with rheumatic disease. The uveitis ass

Pyogenic infection of the sacro-iliac joint. Report of thirteen cases.

Thirteen cases of infectious sacro-iliitis caused by pyogenic organisms are reported and compared with thirty-four cases in the literature. Staphylococcus was the commonest infecting organism (six cases). Two clinical patterns were noted, acute (nine cases) and subacute (four cases). Treatment was by antibiotics and bed rest in all patients. Surgery was indicated for diagnosis in three patients and for abscess in three patients. All of our patients had very good functional recovery.

HL-A27 and ankylosing spondylitis in B.C. Indians.

HL-A antigens were determined in Haida and Bella Coola native Indians, two communities known to have a high prevalence of ankylosing spondylitis. Tests were conducted on those with x-ray evidence of sacro-iliitis and on a sample of the community at large. Sacro-iliitis was found to prevail in approximately 10 per cent of adult Haida males and in over two per cent of Bella Coola adult males. Of 20 Haidas with sacro-iliitis, 17 were HL-A 27 positive. Fifty per cent of the Haida community at large were HL-A 27 positive. Three Bella Coolas known to have sacro-iliitis were all HL-A 27 positive, whi

Studies on the major histocompatibility system in patients with ankylosing spondylitis.

One hundred % of 66 patients with pelvospondylitis were found to have HL-A27, whereas the frequency of the antigen in 60 patients with sacroiliitis was 70 %. The difference in frequency of HL-A27 between the two patient groups is statistically highly significant. This implies that tissue typing could be of clinical importance for a prognostic evaluation of patients with sacroiliitis. Studies in families with several HL-A/MLC identical sibs indicated a relatively low penetrance of disease in those carrying the gene for increased susceptibility to development of AS. No association of AS to parti

The association between HL-A antigens ankylosing spondylitis and sacro-iliitis.

The second series antigen W27 has been found to have an increased frequency in ankylosing spondylitis (82 per cent of 17 patients) and juvenile rheumatoid arthritis (Still's disease) (29 per cent of 24 patients). Three family studies are presented, including one in which the antigen W27 occurs both in the absence and the presence of ankylosing spondylitis. It is suggested that this family illustrates that the relationship between W27 and sacro-iliitis is affected by non-HL-A genes. The development of clinical disease depends upon the coincidence of several factors in one individual, some of wh
