Cervical spine instability in axial spondyloarthritis: prevalence, patterns, and predictive factors in a high-risk cohort.

OBJECTIVE: Although axial spondyloarthritis (axSpA) is an inflammatory disease primarily affecting the spine, studies investigating cervical spine instability (CSI) are limited, and no research has comprehensively evaluated all forms of CSI. This study aimed to determine the frequency and associated factors of CSI in axSpA patients. METHOD: This retrospective cross-sectional study, conducted in a single tertiary rheumatology unit, included adult axSpA patients with inflammatory back pain duration of > 10 years and receiving biological or targeted synthetic therapy. Patients with any concomita

Women with spondyloarthritis occurring in the peripartum period develop a singular phenotype.

BACKGROUND: Spondyloarthritis (SpA) has specific features in women and may be triggered by parturition. The peripartum period is marked by musculoskeletal remodelling and IL-17 secreting cell activation to enable childbirth. We therefore hypothesized that peripartum SpA might predominantly involve the entheses and be less dependent on HLA-B27 and TNFα. METHODS: We conducted a retrospective, observational, case-control study using the Rouen University Hospital's clinical data warehouse. Delivery dates and symptom onset were available for 154 women with SpA. Characteristics were compared betwee

Bilateral Brucellar Sacroiliitis Mimicking Tuberculous or Inflammatory Spondyloarthropathy: A Case Report.

Brucellosis is a zoonotic infection, endemic in rural areas. It is caused by Brucella species, which are transmitted through the intake of unpasteurized dairy products and contact with infected animals. Sacroiliac joint involvement is common in brucellosis and typically presents as unilateral sacroiliitis. Contrarily, bilateral sacroiliitis is less typical and may mimic tuberculosis, pyogenic infection, or inflammatory spondyloarthropathy. We report a case of a 19-year-old man who presented with a one-month history of bilateral buttock pain radiating to the legs and a 2-week history of fever,

Efficacy of Platelet-Rich Plasma Injections Versus Corticosteroid Injections in the Management of Chronic Low Back Pain Secondary to Sacroiliitis.

OBJECTIVES: Low back pain secondary to sacroiliitis (SI) significantly affects patients' quality of life. Although corticosteroid injections are commonly used, their effects may be temporary and have adverse effects. Platelet-rich plasma (PRP) has emerged as a potential alternative; however, comparative evidence remains limited. Therefore, this study aimed to compare the efficacy of PRP versus corticosteroid injections in managing chronic low back pain secondary to SI to provide evidence to guide clinical decision-making and optimize treatment strategies across different clinical contexts. Patient and physician global assessments capture distinct clinical domains in axial spondyloarthritis.

BACKGROUND: Patient global (PtGA) and physician global (PhGA) assessments are key components of disease evaluation in axial spondyloarthritis (axSpA). Although commonly used in composite indices, these measures are often interpreted as interchangeable indicators of disease activity. It remains unclear whether PtGA and PhGA reflect overlapping or distinct clinical domains. METHODS: In this cross-sectional study, 155 patients fulfilling the modified New York criteria, with ASAS classification criteria additionally recorded, were included. All patients had radiographic and MRI-confirmed sacroili

Prevalence of and factors associated with radiographic sacroiliitis in Japanese patients with inflammatory bowel disease: a computed tomography-based study.

OBJECTIVE: To determine the prevalence of radiographic sacroiliitis-a classification feature of spondyloarthritis-and to identify associated factors in Japanese patients with inflammatory bowel disease. METHODS: This retrospective study included 880 patients with ulcerative colitis or Crohn's disease who underwent abdominopelvic computed tomography at an inflammatory bowel disease centre. Sacroiliac joints were evaluated using a validated scoring system. Radiographic sacroiliitis was defined as a total erosion score ≥3 or the presence of sacroiliac joint ankylosis. Multivariate logistic regre

Does adding a high-resolution 3D sequence improve the accuracy of erosion detection in pediatric sacroiliac joint MRI?

OBJECTIVE: To evaluate whether adding an erosion-sensitive high-resolution 3D sequence to sacroiliac joint (SIJ) MRI protocols in children improves erosion detection and diagnosis of Juvenile Spondyloarthritis (JSpA). MATERIALS AND METHODS: SIJ-MRI were prospectively obtained in 84 children (50 girls, mean age 13.3 y) clinically suspected of JSpA, conforming to 2024 ASAS/SPARTAN recommendations including a volumetric interpolated breath-hold examination (VIBE) sequence. Three experienced radiologists recorded the presence of erosions, non-erosion bony defects, and the presence of JSpA, with c

Clinical Efficacy and Safety of Minimally Invasive Sacroiliac Joint Fusion: A Retrospective Cohort Study.

Background Sacroiliitis, inflammation of one or both sacroiliac (SI) joints, is a frequent yet often underrecognized source of chronic low back pain. Conservative treatments - such as physical therapy, medications, and corticosteroid injections - are the first-line treatment options for these patients. However, when they fail to provide lasting relief, surgery may become necessary. The purpose of this study is to evaluate the clinical outcomes of minimally invasive SI joint fusion (MI SIJF), specifically focusing on pain improvement and postoperative complication rates at the six-month follow-

Incidental detection of spondyloarthritis on FDG-PET/CT performed for unintentional weight loss.

18F-fluorodeoxyglucose (FDG)-positron emission tomography/computed tomography (PET/CT) is frequently used to evaluate malignancy. However, FDG uptake is not cancer-specific and may also be seen in inflammatory rheumatic diseases. Recognition of musculoskeletal uptake patterns can prevent misdiagnosis and prompt targeted evaluation. A 60-year-old woman developed unintentional weight loss. Malignant lymphoma was suspected based on elevated serum C-reactive protein, axillary lymphadenopathy, and elevated soluble interleukin-2 receptor. FDG-PET/CT demonstrated prominent uptake in the sternoclavicu

Role of MRI-based synthetic CT and low-dose CT for the assessment of structural damage in axial spondyloarthritis - a review.

Structural lesions, including erosions, sclerosis, and pathological new bone formation, are key features of disease progression in axial spondyloarthritis (axSpA) and are associated with irreversible functional impairment. Reliable imaging methods are therefore essential for accurate assessment and monitoring. Radiography remains the recommended method for evaluating structural damage; however, it is limited by poor visualization of the thoracic spine and early sacroiliitis, as well as having low sensitivity to change. Magnetic resonance imaging (MRI) enables early detection of inflammatory le

Relationship between sacroiliac joint involvement and intestinal disease activity in newly diagnosed Crohn's disease.

BACKGROUND: Axial musculoskeletal involvement represents an important component among the extraintestinal manifestations of Crohn's disease (CD). Sacroiliac joint (SIJ) involvement can be detected at an early stage using imaging modalities. The aim of this study was to evaluate the relationship between SIJ involvement and laboratory parameters, intestinal disease activity, and SIJ anatomical variations in patients with newly diagnosed CD. METHODS: In this single-center retrospective imaging-pathology correlation study, 68 adult patients with a multidisciplinary diagnosis of Crohn's disease es

Classifications of rheumatological extraintestinal manifestations in inflammatory bowel disease: Unmet needs in clinical practice and trials.

BACKGROUND: Rheumatological extraintestinal manifestations (EIMs) represent the most frequent complications of inflammatory bowel disease (IBD). Despite their clinical relevance, they remain heterogeneously defined and inconsistently assessed in both clinical practice and clinical trials. This narrative review aimed to summarize current evidence on the diagnostic assessment of IBD-associated rheumatological EIMs and to identify unmet needs for clinical evaluation and trial design. METHODS: A literature search was performed to identify studies evaluating the diagnosis and assessment of IBD-ass

Cracks in the Diagnosis: A Case of Hypermobility Spectrum Disorder as a Mechanical Mimic of Spondyloarthritis With a Brief Review.

Adolescents commonly present with chronic back and knee pain, and distinguishing mechanical from inflammatory causes is a frequent challenge in general and pediatric practice. Juvenile spondyloarthritis (jSpA) is a chronic inflammatory disease affecting joints, entheses, and potentially the axial skeleton that may require prolonged immunosuppression to prevent structural joint damage. Generalized joint hypermobility (GJH) and hypermobility spectrum disorder (HSD) can present with overlapping musculoskeletal symptoms, including chronic joint pain and back stiffness, and may therefore be mistake
