Diagnostic Algorithm for Axial Involvement in Psoriatic Arthritis.

To develop a unified diagnostic algorithm for axial psoriatic arthritis (axPsA). MATERIALS AND METHODS. : A total of 122 patients with psoriatic arthritis (PsA), duration less than 10 years, were included in the study according to CASPAR criteria, provided that they also had axial involvement. Axial involvement was detected in case of radiographic sacroiliitis [(rSI); bilateral grade ≥2 or unilateral grade ≥ 3] or active MRI SI (MRI-SI), or ≥ 1 syndesmophyte(s) of the cervical and/or lumbar spine (CS/LS), or facet joints ankyloses of the CS. Patients were evaluated for the presence of inflamma

Pediatric Sacroiliac Joint: Normal Development and Pathologic Disorders.

The sacroiliac joint (SIJ) is a critical anatomic structure that connects the axial skeleton with the lower limbs. Compared with its adult counterpart, the pediatric SIJ exhibits unique anatomic features, and imaging evaluation may then present significant challenges due to overlapping features between normal developmental manifestations, anatomic variants, and true pathologic conditions. Two frequently observed developmental findings can resemble pathologic conditions: subchondral irregularities and subchondral flaring. These are commonly seen on MR images of the developing pelvis. A diverse

Phenotypic differences and treatment persistence in a large multicentre cohort of axial psoriatic arthritis.

OBJECTIVE: To compare clinical features across three axial psoriatic arthritis (PsA) phenotypes: (i) isolated axial PsA, (ii) axial PsA with oligoarticular involvement (axial PsA+oligo), and (iii) axial PsA with polyarticular disease (axial PsA+poly), and assess persistence of first-line biologic and targeted synthetic Disease-Modifying Anti-Rheumatic Drugs (b/tsDMARDs). METHODS: Baseline demographic, clinical, therapeutic, laboratory, and imaging data were retrospectively collected. Kaplan-Meier analysis evaluated treatment persistence, and LASSO Cox regression identified baseline factors as

HLA-B*51 Beyond Behcet's Disease: Topography of Symptoms, Associated Diagnoses, and Characterization of Chronic Inflammatory Arthritis Phenotypes.

In the clinical context of the new medical concept of diseases related to the Major Histocompatibility Complex class I (MHC-I-opathy), contrasting data are available on the possible association among HLA-B*51, Behcet's disease (BD), and spondyloarthritis (SpA). The aim of this retrospective study on a cohort of HLA-B*51-positive patients who were clinically observed for almost 5 years is primarily to evaluate which classification criteria they satisfy among BD, axial (ax) or peripheral (p) SpA, and psoriatic arthritis (PsA). Furthermore, we characterized the possible impact of different arthri

Comparative radiologic characteristics of isotretinoin-induced sacroiliitis and axial spondyloarthritis.

BACKGROUND: Sacroiliitis is a major cause of inflammatory low back pain in young adults and a key feature of spondyloarthritis (SpA), but it may also occur with infection, pregnancy, or drug exposure. Isotretinoin, widely used for severe acne, can rarely induce sacroiliitis. AIM: This study aimed to compare the MRI characteristics of isotretinoin-induced sacroiliitis with those of axial spondyloarthritis (axSpA). METHODS: The study population consisted of 68 patients, comprising 28 with isotretinoin-induced sacroiliitis (median age 24 years; 5 males) and 40 with axSpA (median age 30 years; 2

SpA in animal models: Unraveling the duality of inflammation and bone formation.

Spondyloarthritis (SpA) comprises a heterogeneous group of chronic inflammatory rheumatic diseases affecting the axial skeleton, peripheral joints, and entheses. It is uniquely characterized by the coexistence of inflammation and pathological new bone formation, ultimately leading to ankylosis. Although the precise pathogenic sequence remains incompletely defined, experimental animal models have provided essential mechanistic insights by reproducing key immunological and structural features of the disease. This narrative review synthesizes current knowledge on SpA pathophysiology derived from

Spondyloarthritis Research and Treatment Network Recommendations for the Referral of Adults With Chronic Back Pain and Suspected Axial Spondyloarthritis to a Rheumatologist.

OBJECTIVE: In the United States, axial spondyloarthritis (axSpA) diagnosis is delayed up to 14 years preventing effective management. We convened a multidisciplinary clinician and patient group to develop recommendations for rheumatology referral of adults with chronic back pain. METHODS: Systematic review (SR) and meta-analysis determined predictive values of clinical, laboratory, and imaging axSpA features for diagnosis. In a Delphi exercise, experts reviewed these test characteristics and voted to gain consensus for inclusion or exclusion in the final recommendations. Features that did not

Clinical and imaging characteristics of patients with axial psoriatic arthritis: Baseline findings from the GESPIC-axial-PsA cohort.

BACKGROUND: Axial disease is a domain of psoriatic arthritis (PsA), referred to as axial-PsA. Despite recent advances and increasing recognition, axial-PsA remains poorly defined; validated classification criteria and longitudinal cohorts are lacking. OBJECTIVES: We aimed to characterise axial-PsA and delineate phenotypes in an imaging-confirmed cohort. DESIGN: GESPIC-axPsA, a dedicated arm of the German Spondyloarthritis Inception Cohort (GESPIC), is a prospective, monocentric, study including patients with psoriasis and clinically diagnosed axial-PsA confirmed by imaging. International Imaging Practices for Sacroiliac Joint MRI Acquisition Protocols in Patients with Juvenile Spondylarthritis.

Background/Objective: International recommendations exist for MRI evaluation of sacroiliac joints (SIJs) in adult spondylarthritis (SpA) and other joints in juvenile (J)SpA. However, consensus recommendations for MRI acquisition protocols for SIJs in JSpA are lacking. Our survey gathered information on current imaging practices on SIJ MRI and expert opinions about imaging protocols. Methods: A survey was sent to an international group of pediatric radiologists and rheumatologists. Participants were asked about practices regarding sequences/planes, use of contrast, and preferred sequences/plane

Nontraumatic unilateral ureteral entrapment at the sacroiliac joint: A rare cause of obstructive uropathy.

Ureteral entrapment within the sacroiliac joint is an exceedingly rare anatomical phenomenon. To date, 6 cases have been documented in the literature: 1 cadaveric and 5 clinical reports. Among the clinical cases, 3 followed high-energy pelvic trauma, while 2 were nontraumatic. We report the third nontraumatic clinical case (and seventh case overall) in a 77-year-old woman presenting with chronic lower back pain. Computed tomography intravenous pyelography revealed focal entrapment of the right ureter at the level of the sacroiliac joint due to advanced degenerative sacroiliitis. Despite a 10-1

Preliminary Validation of the Early Sacroiliitis Screening Tool (ESHST) for Identifying Hidradenitis Suppurativa Patients at High Risk of Spondyloarthritis.

BACKGROUND: Hidradenitis suppurativa (HS) is increasingly recognized as a systemic inflammatory disease associated with spondyloarthritis (SpA). However, validated screening tools for SpA in dermatology practice are lacking. We therefore developed the Early Sacroiliitis Screening Tool (ESHST), an eight-item symptom-based questionnaire for early SpA identification in HS. METHODS: In this prospective, cross-sectional, preliminary validation study, 100 consecutive HS patients (median age 36.5 years, 72% male) were enrolled in a tertiary-care setting. All participants underwent blinded rheumatolo

Prevalence and determinants of sacroiliac joint bone marrow oedema in the general population in Germany: a population-based cross-sectional study.

BACKGROUND: MRI-detected bone marrow oedema in sacroiliac joints is central to diagnosing axial spondyloarthritis, influencing treatment decisions including anti-inflammatory therapy initiation. However, the prevalence of bone marrow oedema in the general population remains unknown, restricting interpretation of MRI findings and potentially leading to overdiagnosis when imaging findings are considered without clinical context. We aimed to establish the prevalence and determinants of sacroiliac bone marrow oedema in the general adult population. METHODS: In this national, population-based, cro

Development of structural damage in the spine in patients with PsA: findings from a longitudinal cohort study.

OBJECTIVES: This study aimed to examine associations between disease activity/severity markers across multiple PsA domains and structural damage development in the spine in a large longitudinal cohort. METHODS: We analysed patients from a prospective longitudinal PsA cohort and evaluated structural damage in the spine by assessing the development of new syndesmophytes (a binary outcome) on radiographs of the cervical and lumbar spine. We used Cox regression to examine the association between syndesmophyte development and disease activity and severity variables, including nail involvement, PAS

Risk factors of delayed kidney graft function following donation after circulatory death - a retrospective cohort study of Swiss kidney transplants.

AIMS OF THE STUDY: On 1 September 2011, donation after circulatory death (DCD) organ donation resumed in Switzerland after the DCD programme had been suspended for four years due to jurisdictional and medico-ethical discrepancies. DCD has gained relevance due to the worldwide scarcity of donor organs and now accounts for nearly half of all donations in the Swiss transplant setting. This study aimed to identify risk and protective factors for the occurrence of delayed graft function, which is a common early complication following kidney transplantation, especially in DCD transplants. METHODS:

Autoimmune Hepatitis-like Syndrome in a Patient with Ankylosing Spondylitis: A Case Report.

Background and clinical significance: Autoimmune hepatitis (AIH) and ankylosing spondylitis (AS) are distinct immune-mediated disorders that only rarely coexist. Diagnostic interpretation becomes especially challenging when the liver biochemistry is not classically hepatocellular and the histology is unavailable. Case presentation: We report a 51-year-old man with inflammatory back pain, polyarthralgia, weight loss, fatigue, night sweats and fever. Laboratory tests showed marked systemic inflammation, anemia and a cholestatic-predominant liver profile with associated aminotransferase elevation

Acute Motor Axonal Neuropathy Coexisting With Ankylosing Spondylitis: A Case-Based Review.

Guillain-Barré syndrome (GBS) is a rare autoimmune disorder characterized by peripheral nerve involvement. Among the many kinds of GBS, acute motor axonal neuropathy (AMAN) syndrome has been shown to have distinct clinical characteristics and consequences. Meanwhile, ankylosing spondylitis (AS) is a long-term inflammatory joint illness that mostly affects the sacroiliac joints and the spine; this disorder has been known to cause discomfort and stiffness, which may lead to decreased flexibility over time. Some studies have found a link between anti-TNF α medication and GBS. GBS was commonly rel

Case Report: Achilles enthesitis as the presenting clue to occult sacroiliitis in hidradenitis suppurativa.

Hidradenitis suppurativa is increasingly recognized as a systemic inflammatory disease with an established overlap with spondyloarthritis. Axial spondyloarthritis is usually suspected when patients report inflammatory back pain, which remains the entry feature of the ASAS axial spondyloarthritis classification criteria. We report a 31-year-old man with Hurley stage II hidradenitis suppurativa who presented with Achilles enthesitis supported by point-of-care ultrasound but denied any current or previous back pain lasting 3 months or longer, including inflammatory back pain and alternating butto

SAPHO Syndrome Presenting With Severe Inflammatory Back Pain (Sacroiliitis) and Rare Retinol Associated Myopathy in an Iraqi Adolescent Male: A Case Report and Literature Review.

SAPHO syndrome is a rare autoinflammatory disorder characterized by synovitis, acne, pustulosis, hyperostosis, and osteitis. Although musculoskeletal and dermatologic manifestations are well recognized, extra-articular involvement remains uncommon, particularly muscular inflammation. We report the case of a 17-year-old male who presented with severe inflammatory low back pain and extensive nodulocystic acne for which he took escalating doses of isotretinoin. Clinical examination demonstrated positive sacroiliac stress tests, proximal muscle myopathy, and dermatologic evaluation confirmed acne
