BACKGROUND: Psoriatic arthritis (PsA) exhibits the greatest predisposition to the development of metabolic disorders compared to other inflammatory joint diseases, including those with a high risk of type 2 diabetes mellitus (T2DM).
AIM: To study the clinical and imaging characteristics of patients with PsA depending on the presence of DM.
MATERIALS AND METHODS: This retrospective study included 309 patients (219 men and 90 women) with PsA. Patients were divided into subgroups based on the presence or absence of T2DM. A comparative analysis of quantitative and qualitative indicators, as well

OBJECTIVE: To assess radiographic sacroiliitis progression in PsA and factors associated with it.
METHODS: We analysed a prospective PsA cohort (1978-2025). Pelvic radiographs were obtained biannually. Change (Δ) in the sacroiliitis sum score (SSS, 0-8; left + right modified New York-mNY-grades) was the primary outcome, while patients with SSS = 8 at baseline were excluded from the analyses. Linear mixed-effects models assessed factors associated with ΔSSS over successive clinic visits. As a secondary outcome, time to incident mNY sacroiliitis was evaluated with Cox regression.
RESULTS: Amon

BACKGROUND: Articular manifestations are among the most common extraintestinal manifestations of inflammatory bowel disease (IBD) and may precede, accompany, or follow intestinal symptoms. Their recognition is clinically important because delayed diagnosis can lead to functional impairment and structural damage.
METHODS: We conducted a retrospective descriptive study in the Department of Hepato-Gastroenterology at Mohammed VI University Hospital, Marrakech, Morocco. Patients with confirmed IBD and associated rheumatologic manifestations who were followed between December 2023 and December 202

OBJECTIVE: To describe clinical, radiological, biochemical, and genetic features in a multigenerational family with HPRT1-related gout and highlight intrafamilial phenotypic variation.
METHODS: Seven family members underwent clinical, laboratory, radiological, and genetic evaluation. Clinical exome sequencing was performed in the index case, followed by targeted Sanger sequencing in available relatives. Dual-energy computed tomography was used to detect monosodium urate deposition in clinically suspected joints.
RESULTS: Seven patients from the same family were evaluated; five had geneticall

Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease characterized by autoantibody production and immune complex deposition. Antiphospholipid syndrome (APS) is frequently associated with SLE and is characterized by arterial or venous thrombosis and pregnancy morbidity in the presence of persistent antiphospholipid antibodies. The coexistence of ankylosing spondylitis (AS) with SLE and APS is extremely rare. A 38-year-old woman was admitted to our clinic with gangrene of the right fifth toe and swelling of both knees. She had a previous diagnosis of AS based on inflammatory lo

Sarcoidosis is a chronic granulomatous, multisystem disease of uncertain etiology that affects various organ systems, although musculoskeletal involvement is rarely the dominant feature. We present the case of a woman in her 50s with a history of pulmonary sarcoidosis who developed new lower back and hip pain. Magnetic resonance imaging demonstrated an asymmetric soft-tissue mass in the ligamentous left sacroiliac joint, along with osseous involvement of the sacrum and ilium. These findings were suspicious for malignancy. An 18F-FDG PET/CT subsequently confirmed FDG-avid disease in this soft-t

INTRODUCTION: Ulcerative colitis (UC) has a complex multifocal etiology that includes immune dysregulation of the gut microbiome leading to pro-inflammatory cytokine release. UC may present initially through subclinical manifestations such as sacroiliitis due to underlying inflammation, known as the gut-joint axis. JAK inhibition provides an effective treatment method for overlapping intestinal and articular inflammation.
CASE PRESENTATION: We describe a 32-year-old man without gastrointestinal symptoms who presented with chronic fatigue and a 2-week history of severe lower back pain. MRI rev

BACKGROUND: Non-radiographic axial spondyloarthritis (nr-axSpA) may present with heterogeneous and misleading clinical features, particularly when inflammatory back pain is absent and HLA-B27 is negative. Such presentations can delay recognition and make classification frameworks difficult to apply in routine diagnosis.
CASE PRESENTATION: We report a 22-year-old Syrian HLA-B27-negative male who initially presented with mild anterior chest wall pain that was clinically interpreted as mechanical and resolved with nonsteroidal anti-inflammatory drugs. Eight months later, he developed a generaliz

BACKGROUND: Chronic pain in patients with multiple overlapping pain syndromes is often refractory to conventional treatments. Peripheral nerve stimulation (PNS) has emerged as a promising neuromodulation therapy, especially when spinal cord stimulation (SCS) is contraindicated or ineffective.
CASE REPORT: We present the case of a 68-year-old man with severe chronic pain in the left hand and wrist following carpal tunnel release surgery, superimposed on a background of cervical and lumbar radiculopathies, degenerative cervical spondylosis, sacroiliitis, and failed back surgery syndrome. PsAPASH syndrome is a rare autoinflammatory disorder comprising psoriatic arthritis, pyoderma gangrenosum, acne, and hidradenitis suppurativa, characterized by severe cutaneous and musculoskeletal manifestations that are often refractory to systemic therapies. Sacroiliitis is a recognized feature of PsAPASH, contributing significantly to functional disability. While cooled radiofrequency ablation has emerged as an effective treatment for chronic sacroiliac joint pain, performing interventional procedures in patients with extensive active hidradenitis suppurativa poses unique challenges due to

A female patient in her late 20s, with a history of ulcerative colitis since adolescence, underwent ileoanal canal anastomosis for ulcerative colitis. She developed diarrhea, rectal pain, and pyoderma gangrenosum and underwent resection of the remaining rectum and ileal pouch-anal anastomosis. One year postoperatively, she developed ileocolitis, left sacroiliitis, and right dorsal foot pain, which were diagnosed as extraintestinal manifestations, and adalimumab was effective. The patient should be introduced to biologic agents at an earlier stage, considering the possibility of flare-ups or th

Tuberculous sacroiliitis is a rare form of extrapulmonary tuberculosis. Its nonspecific clinical presentation often leads to delayed diagnosis and treatment, increasing the risk of severe complications such as deformities and functional limitations. We report the case of a 67-year-old male patient who presented with a progressively enlarging gluteal mass and weight loss. Clinical examination revealed a 20 × 20 cm soft, non-tender mass in the right gluteal region, associated with limited joint mobility. Computed tomography (CT) identified osteolytic lesions in the right sacroiliac joint and a c

UNLABELLED: Universal trauma-informed screening is increasingly recommended across pediatric settings, yet it remains unclear which forms of adversity most accurately and equitably identify youth at risk for psychopathology. In response, the current study tested whether defining maltreatment by cumulative quality (i.e., number of subtypes) versus cumulative frequency (i.e., number of experiences regardless of subtypes), as well as potential differences within each subtype, yielded more accurate and fair predictions of trauma-related psychological distress. Participants included 839 adolescents

Background. Unlike full reading, 'skim-reading' involves the process of looking quickly over information in an attempt to cover more material whilst still being able to retain a superficial view of the underlying content. Within this work, we specifically emulate this natural human activity by providing a dynamic graph-based view of entities automatically extracted from text. For the extraction, we use shallow parsing, co-occurrence analysis and semantic similarity computation techniques. Our main motivation is to assist biomedical researchers and clinicians in coping with increasingly large a

Brucellosis is a zoonosis acquired direct contact with infected animals or by consumption of unpasteurized animal by-products. The most common complications involve the musculoskeletal system, such as sacroiliitis or vertebral osteomyelitis. However, skin and soft tissue involvement is uncommon, and cervical abscess are rarely reported. We report the case of a 19-year-old male patient, with no medical history, from the metropolitan region of Caracas, Venezuela, with no history of travel or direct contact with farm animals, but regular consumption of cheese. In July 2024, he presented with undu

Brucellosis is a widespread zoonotic infection that often follows a chronic course and may involve the osteoarticular system. Patients with brucellosis-related musculoskeletal manifestations are frequently evaluated by rheumatologists and may be misdiagnosed with rheumatic diseases, including ankylosing spondylitis. We present the case of a 45-year-old female patient who was diagnosed with osteoarticular brucellosis approximately 20 years ago and was successfully treated with etiotropic therapy. Eighteen years later, she developed back pain that worsened at night and did not improve with rest,

UNLABELLED: Exertional leg pain (ELP) is a common but frequently underrecognized symptom in children with familial Mediterranean fever (FMF). This study aimed to evaluate the clinical characteristics of ELP, its relationship with disease severity, and its potential impact on treatment modifications in a large pediatric cohort. This study evaluated pediatric FMF patients (2013-2024) fulfilling the 2019 Eurofever/PRINTO criteria. Demographic, clinical, genetic, and treatment data were analyzed. Disease severity was assessed using the International Severity Score for FMF (ISSF). Among 1290 patien
