Posterior percutaneous fixation without anterior or posterior debridement for infectious spondylitis in high-morbidity patients: A retrospective comparative study.

Standard surgical treatment for infectious spondylitis involves anterior debridement and interbody fusion. However, for patients with high morbidity, this may result in many complications. We propose an alternative technique - posterior-only percutaneous pedicle screw fixation without anterior or posterior debridement ("internal orthosis") - to reduce surgical invasiveness while maintaining clinical efficacy. We retrospectively compare the outcomes between conventional and internal orthosis procedures. This retrospective comparative study included patients who underwent surgery for thoracolumb

Technical considerations and contemporary literature review of the Gore Thoracic Branch Endoprosthesis®.

Thoracic endovascular aortic repair (TEVAR) has become the preferred treatment for various thoracic aortic pathologies, including blunt thoracic aortic injury, aneurysms, and dissections. Involvement of Ishimaru Zone 2 occurs in up to 60% of TEVAR cases requiring extension across the distal aortic arch and left subclavian artery (LSA). Coverage of the LSA without revascularization increases the risk of complications such as upper extremity ischemia, stroke, and spinal cord injury. Therefore, the preservation of LSA perfusion during TEVAR is essential. Cervical debranching remains an effective

An unintended consequence: a review of iatrogenic central nervous system mold infections and outbreaks.

SUMMARYIatrogenic central nervous system mold infections (ICNSMIs) are rare and occur as sporadic complications of neurosurgical procedures or device insertions. However, recent outbreaks have been reported in outpatient settings, following epidural injections with contaminated medicines. We conducted a comprehensive literature review of studies published without date restrictions through July 2025 on ICNSMIs. We identified 905 cases of such infections, with 4% occurring in non-outbreak settings and 96% associated with five documented outbreaks. In both non-outbreak and outbreak settings, infe

Can we move thoracic disc surgery to the ambulatory setting? The role of endoscopy.

OBJECTIVE: While surgery for thoracic disc herniation causing spinal cord compression has traditionally been an inpatient procedure, fraught with risk and often requiring multilevel fusion, recent advances in endoscopic surgery challenge this paradigm. This study presents a single-surgeon case series of endoscopic thoracic discectomy performed in an ambulatory setting, outlining 1) the surgical technique, 2) patient selection and characteristics, and 3) postoperative outcomes. The goal of this study was to assess the safety and feasibility of performing endoscopic thoracic discectomy in an amb

Congenital Cervical Vertebral Malformations and Recurrent Bacterial Infections in Chickens Associated with In Ovo Vaccination Equipment Malfunction.

In ovo vaccination is an effective method of immunization that allows protection early in the life of the chick but requires precision during the administration. This present report describes an occurrence of cervical vertebral malformations in broiler chicks that coincide with mechanical failure of an in ovo vaccination system, in the absence of other detected infectious, nutritional, or toxic etiologies. Cervical scoliosis was observed in 2%-8% of chicks at hatch or within days post hatch in the affected flock, which all came from the same hatchery. Gross and histologic examination of eleven

Hidden in the Spine: Two Cases Illustrating Spondylodiscitis as an Early Red Flag for Mitral Valve Infective Endocarditis.

Infective endocarditis (IE) remains a severe condition with high morbidity and mortality, often complicated by metastatic infections. Spinal involvement, particularly spondylodiscitis (SD) and epidural or paraspinal abscesses, is increasingly recognized but frequently underdiagnosed. We report two cases of mitral valve IE complicated by SD. A 77-year-old woman with culture-negative IE underwent urgent mitral valve replacement; subsequent magnetic resonance imaging revealed L3-L4 SD and a large right psoas abscess managed conservatively with prolonged antibiotics. A 73-year-old woman presented

Occurrence of neuronal dysfunction in mice with virus- or toxic-induced lesions in the central nervous system.

Diseases of the central nervous system (CNS) often include neuronal damage. This study examined proteins crucial to apoptosis and neuronal signalling for identifying causes of neuronal dysfunction in two animal models: Theiler's murine encephalomyelitis virus-induced demyelinating disease (TMEV-IDD) and ethidium bromide (EtBr)-induced focal spinal cord injury. Transcriptomic analysis revealed increased levels of pro-apoptotic molecules including Caspase 8 (Casp8) and Cathepsin B (CTSB), but no significant changes in synaptic proteins including synaptophysin, synaptosomal-associated protein of

Whole-Spine MRI Reveals High Prevalence of Multifocal Spondylodiscitis and Identifies a High-Risk Subgroup: A Retrospective Cohort Study of 274 Patients.

Background and Objectives: Spondylodiscitis is a severe spinal infection associated with substantial mortality. Standard diagnostic imaging is often limited to the symptomatic spinal segment, which may fail to detect infection foci in other spinal regions. The prevalence and prognostic significance of multifocal spondylodiscitis remain insufficiently characterized. Materials and Methods: A retrospective single-center cohort study was conducted at the University Medical Center Mannheim, Germany. All patients with a first diagnosis of imaging-confirmed infectious spondylodiscitis treated between
