Pneumocystis jirovecii pneumonia in patients with CNS tumors receiving dexamethasone: clinical outcomes and prophylaxis gaps.

PURPOSE: Pneumocystis jirovecii pneumonia (PCP) now predominantly affects non-HIV immunocompromised patients, with corticosteroid exposure a major risk factor. Patients receiving high-dose dexamethasone for central nervous system (CNS) tumor–related indications represent a vulnerable but understudied subgroup. We sought to characterize outcomes in this population and compare disease severity and mortality with other immunocompromised patients with PCP. METHODS: We performed a retrospective multicenter cohort study of adults with proven or probable Pneumocystis jirovecii pneumonia (PCP) accordi

[Endoscopic Approaches to the Thoracic Spine].

PURPOSE: In spinal surgery, endoscopic surgical techniques have been gaining increasing importance in recent years. Although the technique itself has existed for decades, it has only begun to spread more widely in recent years due to technological advancements and increasingly structured training concepts. Nowadays, endoscopic procedures can be performed on all regions of the spine, including the thoracic spine. MATERIALS/METHODS: The two standardised endoscopic techniques for the thoracic spine are the interlaminar and the transforaminal approaches. Additionally, there are individual case re

Glycoprotein G enables HSV-2 neuroinvasion and provides protection as a glycosylated vaccine antigen.

The role of glycoprotein G (gG-2) of herpes simplex virus type 2 (HSV-2) in viral pathogenesis remains poorly understood. gG-2 is cleaved into a secreted form (sgG-2) and a membrane-associated form (mgG-2), but the in vivo function of mgG-2 and the contribution of its glycosylation to immune responses have not been defined. Here, we provide a comprehensive characterization of the N- and O-linked glycosylation profile of mgG-2 and investigate its functional relevance for viral spread and vaccine-induced immunity. Using a mouse genital infection model, we show that an mgG-2-deficient HSV-2 mutan

Vertebral osteomyelitis caused by Chaetomium Globosum in a young adult: A case report and literature review.

RATIONALE: Chaetomium globosum (C. globosum) is a saprophytic fungus widely distributed in nature, characterized by low clinical pathogenicity but capable of causing severe infections, particularly in immunocompromised hosts or under conducive environmental conditions. Spinal infections attributable to C. globosum are exceedingly rare and often present diagnostic dilemmas due to atypical presentations. This case underscores the importance of considering fungal etiologies in spinal pathologies. Furthermore, the strain mentioned in this study has been appropriately preserved and is available for

Development and evaluation of a nomogram model for predicting prolonged hospitalization after spinal tuberculosis focus decompression, fusion, and internal fixation surgery.

BACKGROUND: Spinal tuberculosis remains a significant clinical challenge in high-prevalence regions. Despite advances in medical treatment, surgical interventions, such as debridement, decompression, and fusion, are often required, but their complexity increases perioperative morbidity and prolongs hospitalization. Identifying preoperative predictors of extended hospital stay may improve patient management and resource allocation. METHODS: In this retrospective study, 256 patients with confirmed spinal tuberculosis who were treated between January 2021 and December 2024 were included. Prolong

Spinal Tuberculosis Diagnosed by Metagenomics Capture (MetaCAP) in a Patient Undergoing Maintenance Hemodialysis: A Case Report.

BACKGROUND Spinal tuberculosis is difficult to diagnose in patients undergoing maintenance hemodialysis (MHD) because of immunosuppression, atypical clinical manifestations, and the limited sensitivity of conventional microbiological assays. Rapid and accurate pathogen identification is essential to distinguish spinal tuberculosis from other causes of vertebral destruction, including metastatic malignancy and bacterial spondylitis. This report aims to illustrate the diagnostic value of capture-based targeted sequencing for detecting Mycobacterium tuberculosis in extrapulmonary infection when r

Drug-Resistant Tuberculous Spondylitis Treated with Bedaquiline-Containing Regimens in South Korea: Two Case Reports.

Background: South Korea continues to report a considerable burden of drug-resistant tuberculosis (TB). Bedaquiline-containing regimens are recommended for multidrug-resistant pulmonary TB, but evidence regarding the optimal treatment for extrapulmonary manifestations such as spinal TB remains limited. Case presentation: Herein, we report two cases of drug-resistant tuberculous spondylitis that were successfully managed using bedaquiline-containing regimens. Case 1 involved a 67-year-old man who was receiving chemotherapy for lymphoma and had a history of spinal TB treated 20 years earlier. The

Infectious Spondylodiscitis of Bacterial Causes in Adults: Epidemiology, Pathophysiology, Diagnostic and Treatment Challenges.

Spinal infections in general, and infectious spondylodiscitis in particular, are increasingly diagnosed in the Western world, in recent decades. This rise in incidence is associated with an ageing population and with an increased availability of accurate diagnostic modalities. Even so, due to the non-specific nature of clinical manifestations, and of the implicated blood and serum markers, there is a risk of underdiagnosis or misdiagnosis of the disease in its initial stages. Ionizing radiation methods, such as plain radiography (X-ray) and computed tomography (CT), are also not reliable in th

Delayed diagnosis of spinal tuberculosis mimicking spondyloarthritis: a case report.

BACKGROUND: Spinal tuberculosis (TB) remains a significant health burden in endemic regions. Its diagnosis is frequently delayed due to non-specific early symptoms and radiological findings that can mimic other inflammatory spinal pathologies, leading to severe complications such as kyphosis and neurological deficit. CASE PRESENTATION: A 71-year-old woman from Southeast China presented with a 1-year history of progressive low back pain, initially misdiagnosed as axial spondyloarthritis. She experienced temporary relief with symptomatic treatment, including secukinumab. One year later, her sym

Burden and health-system challenges of diabetes mellitus in urban slum populations of Iran: a scoping review.

Multiple myeloma (MM) is a clonal plasma cell disorder characterized by aberrant immunoglobulin deposition causing end-organ damage (Jones et al. in Hum Pathol 30:1441-1448, 1999). Rarely, paraproteins crystallize within histiocytes, resulting in crystal-storing histiocytosis (CSH) (Jones et al. in Hum Pathol 30:1441-1448, 1999). The coexistence of MM, CSH, and amyloidosis is exceedingly uncommon (Takahashi et al. in Pathol Int 37:141-154, 1987). A 56-year-old male presented with chronic low back pain and elevated creatinine (~ 2.05 mg/dL). Myeloma workup revealed elevated beta-2 microglobulin

Paralytic rabies outbreak mimicking guillain-Barré syndrome in French Amazonia.

BACKGROUND: In the Amazonian region, vampire bats are the primary reservoir of rabies virus, causing sporadic and lethal human rabies cases that often remain unnoticed. Managing human cases in this region is challenging and further complicated by atypical clinical forms and the potential exposure to various toxic compounds, particularly among gold miners. METHODS: We carried out clinical, electrical, biological and histological analysis of concurrent cases of progressive motor neuronopathy and fatal encephalitis in a context of regular exposure to bat bites of gold miners living in a small an

Capnocytophaga canimorsus spondylodiscitis: an unusual presentation of a zoonotic infection.

Capnocytophaga canimorsus is a slow-growing, Gram-negative bacterium commonly found as a part of the normal oral flora of dogs and cats, and it is associated with severe systemic infections following dog bites, especially in immunocompromised patients. Although spondylodiscitis due to Capnocytophaga species has been reported in the literature, cases involving previously operated spinal segments are extremely rare. A 52-year-old woman working as a dog trainer presented with fever, headache, photophobia, and lumbar back pain. Her medical history included an L5-S1 hemilaminectomy performed 9-year

[Immobilizing back pain in an elderly patient: Differential diagnostic considerations and diagnosis].

A 77-year-old patient presents with acute back pain. Due to elevated CRP and other lab abnormalities, bacterial spondylodiscitis is initially suspected, and blood cultures are obtained early. However, a biopsy reveals uric acid crystals and cultures remain negative, leading to the diagnosis of gout spondylodiscitis. Treatment includes renal-adjusted anti-inflammatory therapy, urate-lowering therapy and flare prophylaxis.

Claw lesions in dairy cattle pose a significant risk to dairy farmers worldwide in terms of animal welfare concerns and economic profitability. However, the use of different data sources, classification systems, and definitions of reference groups limits the comparison across herds and decreases the usability of the recordings for phenotypic and genetic analyses. In South Africa, information on claw lesions is not routinely collected by dairy farmers and data are limited to hoof trimmers recording lesions during preventative trimming or as needed by producers. Records of the most common claw l

Combined phage therapy and faecal microbiota transplantation to treat recurrent urinary tract infection: a case series.

Recurrent urinary tract infections are recalcitrant and difficult-to-treat bacterial infections that primarily affect women. Here we combine phage therapy with faecal microbiota transplantation to decolonize the urinary and intestinal reservoirs of patients with recurrent urinary tract infections. Three women received oral and intravesical phage therapy for 8 days, and two underwent subsequent faecal microbiota transplantation. Treatments were well tolerated, and although Escherichia coli was detected in follow-up samples, patients experienced none or fewer and less severe episodes of urinary

CHCHD2 as a prognostic biomarker and therapeutic target in lung adenocarcinoma: Insights from pan‑cancer multi‑omics and functional studies.

Helminths include both parasitic nematodes (roundworms) and platyhelminths (trematode and cestode flatworms) that are abundant, and are of clinical importance. The genetic characterization of parasitic flatworms using advanced molecular tools is central to the diagnosis and control of infections. Although the nuclear genome houses suitable genetic markers (e.g., in ribosomal (r) DNA) for species identification and molecular characterization, the mitochondrial (mt) genome consistently provides a rich source of novel markers for informative systematics and epidemiological studies.
