Cervical epidural plasmacytoma causing recurrent radiculopathy and revealing plasma cell leukemia.

BACKGROUND: Cervical epidural plasmacytomas are an uncommon manifestation of plasma cell dyscrasias and may mimic degenerative cervical spondylosis. Here, we report an epidural plasmacytoma discovered during reoperation for recurrent radiculopathy that led to the diagnosis of plasma cell leukemia (PCL). CASE DESCRIPTION: A 76-year-old female underwent a C5-C7 anterior cervical discectomy and fusion for cervical spondylotic radiculopathy. After initial improvement, severe radiculopathy recurred 6 weeks later. During posterior C6-T1 decompression, a gray, friable epidural mass encasing the left

Semi-quantitative assessment of paraspinal muscle fat infiltration in single-level C5-6 cervical spondylosis using gray-level thresholding on conventional MRI: associations with disc degeneration and posture-related segmental angular change.

BACKGROUND: Cervical spondylosis at C5-6 is common, but conventional imaging may not fully capture paraspinal muscle composition or posture-related segmental angular behavior. This study assessed layer-specific paraspinal muscle fat infiltration (FI) and cross-sectional area (CSA) on conventional MRI and examined their associations with C5-6 disc degeneration and posture-related angular change (PAC). METHODS: In this retrospective cross-sectional study, 94 patients with single-level C5-6 cervical spondylosis who underwent cervical MRI and standing lateral radiography between January 2023 and

Incidence and Time Course of Adjacent Segment Disease After Anterior Cervical Fusion: A Multimodal Assessment in 400 Patients.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To evaluate the incidence and time course of adjacent segment disease (ASD) after anterior cervical fusion (ACF) using radiographic, MRI, clinical, and reoperation endpoints. SUMMARY OF BACKGROUND DATA: ASD is a recognized concern after ACF, but reported rates vary by definition. Radiographic degeneration, MRI-confirmed neural compression, symptoms, and reoperation may represent different stages, yet these endpoints are rarely assessed together in one cohort. METHODS: We reviewed 400 patients who underwent ACF for degenerative cervical sp

GFPT1 as a cross-ancestry validated target for degenerative spinal disease: genetic association in a Chinese cohort and functional characterization in zebrafish.

Degenerative spinal disease (DSD), including spinal stenosis and spondylosis, lacks effective pharmacological treatment. To identify druggable targets and assess cross-ancestry applicability, we integrate multi-omics analyses using Summary-data-based Mendelian Randomization (SMR), colocalization, and two-sample Mendelian randomization with European whole-blood, peripheral-blood, and CSF eQTL/pQTL datasets, followed by whole-genome sequencing (WGS) validation in a Chinese cohort. We identify 7 genes/proteins associated with spinal stenosis and 5 with spondylosis, with GFPT1, GPX1, and SERPINA1

Spinal Cord Stimulation Avoiding Deformity Fusion in an Osteoporotic Patient with Sagittal Imbalance: A Case Report.

CASE: A 78-year-old woman had gluteal pain radiating to both lateral thighs and calves and progressive neurogenic claudication. Imaging revealed osteoporotic thoracolumbar compression fractures, L4 spondylolisthesis with severe L4-5 stenosis, and sagittal kyphosis. Spinopelvic parameters were not measured, owing to extreme forward-flexed posture. Given the high risk of instrumentation failure and complications, spinal cord stimulation (SCS) was selected after initiating osteoporosis therapy. The pain improved, with pain relief and ambulatory function maintained at 18 months without surgery. C

Acupotomy for Cervical Spondylosis-Related Neck Pain: A Study Protocol for a Randomized Controlled Trial.

BACKGROUND: Cervical spondylosis (CS) is a common source of neck pain and imposes a substantial global health burden. Conservative treatments, including oral medication, physical therapy, and rehabilitation exercises, are widely used in clinical practice, but their effects are often limited or slow to become apparent. Acupotomy, a minimally invasive intervention that combines acupuncture principles with soft-tissue release techniques, may alleviate CS-related neck pain by releasing local adhesions and reducing muscle tension. However, methodologically rigorous evidence from randomized controll

Radiological Outcomes of PEEK Versus Rigid Rod Stabilization in Lumbar Spinal Stenosis Surgery: The Role of Preoperative and Postoperative Findings in Adjacent Segment Disease.

Background/Objectives: Lumbar spondylosis is a degenerative disorder that may require decompression and stabilization surgery. Rigid titanium rods provide strong fixation, whereas polyetheretherketone (PEEK) rods have been proposed to offer a more flexible load distribution profile. This study compared the radiological outcomes after PEEK versus rigid rod stabilization and evaluated whether the preoperative degenerative findings contributed independently to the postoperative adjacent segment radiological status. Methods: A retrospective cohort of 106 patients undergoing lumbar decompression an

The "one-and-a-half" minimally invasive transforaminal lumbar interbody fusion: a single-center retrospective case series.

OBJECTIVE: Symptomatic lumbar spondylolisthesis with multilevel spinal stenosis poses a complex surgical challenge, often managed through multilevel decompression and fusion. There is a hybrid surgical approach, termed "one-and-a-half," that combines minimally invasive transforaminal lumbar interbody fusion (mTLIF) at the unstable segment with unilateral laminotomy for bilateral decompression (ULBD) at an adjacent stenotic level without fusion. The aim of this study was to assess the reoperation rate, specifically fusion extension to adjacent decompressed segments, in patients who underwent th

A Multivariate Analysis of Radiographic Healing in Pediatric Athletes Diagnosed With Spondylolysis.

BACKGROUND: Spondylolysis is commonly seen in adolescent athletes with lumbar pain after acute or repetitive low back hyperextension. Nonoperative management and rest often lead to symptom resolution and return to sport in acute injuries. However, follow-up imaging confirming radiographic healing is rarely utilized. PURPOSE/HYPOTHESIS: The purpose of this study was to investigate the relationship between age, sex, and radiographic healing in pediatric athletic patients with spondylolysis after nonoperative treatment. It was hypothesized that younger athletes would have better healing rates as

Inflammatory protein mediators linking gut microbiota to degenerative lumbar spine disorders: cross-disease genetic evidence.

BACKGROUND: Degenerative lumbar spine disorders (DLSD), including intervertebral disc disorders (IDD), degenerative spondylolisthesis, and lumbar spinal stenosis (LSS), are major contributors to low back pain and disability. Associations among gut microbiota (GM), inflammatory proteins, and DLSD have been demonstrated in prior studies. Yet, two key questions persist: whether specific circulating inflammatory proteins (IPs) mediate this association, and whether such mediation is shared across different diseases. METHODS: We performed two-sample Mendelian randomization (MR) to evaluate causal a

Utilization of robotic pars repair for early return to activity in adolescents with symptomatic spondylolysis: a case series.

OBJECTIVE: Although the mainstay of treatment for lumbar spondylolysis is nonoperative management with activity modification, a sizeable portion of patients will go on to nonunion. The growing use of robotic surgical guidance can allow for placement of pars screws through percutaneous incisions. The objective of this case series was to report on the largest series of adolescent patients with lumbar spondylolysis treated with robot-assisted pars repair to date. METHODS: A retrospective review of a prospectively collected database was performed for all adolescent patients (aged 13-25 years) who

Posterior lumbar facet arthroplasty versus fusion for the treatment of spondylolisthesis: 3-year results from the Total Posterior Spine System investigational device exemption study.

OBJECTIVE: The optimal surgical treatment for lumbar spinal stenosis (LSS) with degenerative spondylolisthesis (DS) remains controversial. Posterior lumbar facet arthroplasty preserves motion after decompression and may address some of the limitations of fusion techniques. The Total Posterior Spine System (TOPS) investigational device exemption trial compared decompression with facet arthroplasty to decompression plus fusion (open with interbody). This study reports the 3-year outcomes from this trial. METHODS: This randomized, controlled, multicenter trial enrolled 321 patients with LSS and
