The spondylolytic vertebra and its adjacent segment. Mobility measured before and after posterolateral fusion.

STUDY DESIGN: By using roentgen stereophotogrammetric analysis in six patients having tantalum indicators implanted at a preoperative external fixation test, the mobility in the spondylolytic lumbosacral level and its adjacent segment could be studied before fusion and during the course of postoperative fusion consolidation. OBJECTIVE: To study the mobility effects on the segment adjacent to a lumbar fusion over time from the preoperative situation until fusion healing as defined by roentgen stereophotogrammetric analysis. SUMMARY OF BACKGROUND DATA: In vitro studies indicate that the altere

The retroisthmic cleft. Scintigraphic appearance and clinical relevance in patients with low back pain.

This article describes the scintigraphic appearance of both a symptomatic and asymptomatic retroisthmic cleft in two athletes with low back pain. This lesion, which involves the lamina, is the least common of the neural arch defects of which spondylolysis is the most common. The anatomy of the lesion is discussed and illustrated. The literature about the possible cause of these lesions is reviewed. The report emphasizes the valuable role of radionuclide bone imaging in patients who have pain of potentially osseous origin, and who have a lesion of uncertain physiologic significance seen on an a

Lumbar spondylolysis. A study of natural progression in athletes.

We examined the natural course of athletically active young people with back pain and a diagnosis of spondylolysis (stress fracture of the pars interarticularis of the facet joint of the lumbar spine). We limited the study to those with "subtle" fractures (normal radiographs and positive bone scans) and used computed tomography scanning to further characterize this lesion and to determine whether we could demonstrate healing in this population. The study group included 40 patients with low back pain and a diagnosis of spondylolysis by nuclear medicine study. Computed tomography scans were perf

Pathogenesis of cervical spondylotic myelopathy.

OBJECTIVE: To determine whether either of two mechanical theories predicts the topographic pattern of neuropathology in cervical spondylotic myelopathy (CSM). The compression theory states that the spinal cord is compressed between a spondylotic bar anteriorly and the ligamenta flava posteriorly. The dentate tension theory states that the spinal cord is pulled laterally by the dentate ligaments, which are tensed by an anterior spondylotic bar. METHODS: The spinal cord cross section, at the level of a spondylotic bar, is modelled as a circular disc subject to forces applied at its circumferenc

Treatment of spondylolysis and spondylolisthesis in children and adolescents.

The treatment of spondylolysis and spondylolisthesis in children depends on the severity of clinical symptoms, pathologic anatomy, and prognosis. Simple spondylolysis can be cured by immobilization alone in selected cases, or by surgery when it remains symptomatic and resistant to nonoperative treatment. The majority of cases are asymptomatic and require no treatment. Spondylolisthesis is classified into 2 types based on the magnitude of the lumbosacral angle: spondylolisthesis with a horizontal sacrum (lumbosacral angle > or = 100 degrees), which seldom requires surgical treatment, usually re

Screw placement in the lumbar vertebral isthmus.

Anatomic parameters of the isthmus from L1 to L5 were measured in 30 dried lumbar spines. All measured parameters were fairly constant from L1 to L5. The mean values of the core length, thickness of the superior and inferior borders of the isthmus, superoinferior diameter and posterior and medial inclinations of the lumbar isthmus at L5 were 39.9 +/- 2.3 mm, 2.0 +/- 0.9 mm, 8.9 +/- 1.0 mm, 13.2 +/- 2.5 mm, 35.9 degrees +/- 5.7 degrees and 31.8 degrees +/- 6.3 degrees, respectively. This study shows that a 40 mm long, 4 to 5 mm diameter screw should be inserted in the lumbar vertebral isthmus a

[Sacralization of the 5th lumbar vertebra and backache: what's the possible relationship?].

Transitional vertebrae is a controversial problem in occupational health and in forensic medicine, in view of its prognostic value and especially because of the implications of law 626/94 which requires a worker assessment for manual handling tasks. The purpose of this review was to assess the relationship between transitional vertebrae and low back pain. Data sources comprised computer-aided search of published studies on Medline and Embase. There were seven studies evaluating the prevalence of the abnormality, the relationship with low back pain and the different aspects of spinal degenerati

Repair of the defect in spondylolysis. Durable fixation with pedicle screws and laminar hooks.

Direct repair of a defect in the pars interarticularis was performed with use of bone-grafting and internal fixation with a pedicle screw, rod, and laminar hook in order to achieve a higher prevalence of osseous union than that achieved with commonly used procedures. The configuration of the head of the screw, which is designed to allow it to connect with the rod at the necessary angle, simplified the placement of the rod. The procedure was performed in sixteen patients who had a bilateral defect of the pars interarticularis with or without grade-I or II spondylolisthesis, had had failure of n

[Lumbar fusion in adults--dorsal or combined ventral/dorsal approach?].

A retrospective study was conducted to compare the results of posterior with combined anterior/posterior lumbar fusion in adults. Seventy-six consecutive posterior cases fused with pedicle screws and 46 combined cases were included and followed for at least 2 years. Subjective assessment was based on the Visual Analogue Scales, Waddel Disability and Impairment Score and the GBB for objective quantification of complaints. Furthermore, a thorough clinical examination was done and X-rays including flexion/ extension radiographs, were taken. Questions were asked about the occupational status as we

Pedicular stress fracture in the lumbar spine.

Spondylolisthesis with or without spondylolysis is common in the lumbar spine. Associated fracture in the pedicle ('pediculolysis') is unusual. The margins of pedicular stress fractures, like spondylolysis, usually appear sclerotic. A patient with a pedicular stress fracture with minimal marginal sclerosis suggesting an injury of recent onset is presented here. There was associated bilateral spondylolysis. The findings in this patient suggest that established pediculolysis probably represents a stress fracture that has failed to heal.

The value of lumbar spine MRI in the assessment of the pars interarticularis.

The aim of this study is to assess the ability of conventional lumbar spine magnetic resonance imaging (MRI) to demonstrate the pars interarticularis. The right and left L4 and L5 pars in 50 patients undergoing lumbar spine MRI for low back pain, with or without radiculopathy, were assessed and classified into four types. Based on their appearance, 59 (29.5%) were classified as normal (Type 1), 59 (29.5%) as hypointense (Type 2), 53 (26.5%) could not be demonstrated (Type 3) and 29 (14.5%) were thought to have defects (Type 4). The gold standard was set by the consensus opinion of two consulta

Magnetic resonance imaging of the musculoskeletal system. Part 8. The spine, section 2.

Magnetic resonance imaging has revolutionized the noninvasive evaluation of degenerative disc disease and its complications. Compared with computed tomography and computed tomographic myelography, magnetic resonance allows specific determination of the nature of disc protrusions and other degenerative related soft tissues about the spine. Magnetic resonance offers the most complete evaluation of specific degenerative disorders including degenerative facet disease, spondylolysis, spondylolisthesis, spontaneous lumbar epidural hematomas, and juvenile discogenic disease.

Spondylolysis in children who have osteopetrosis.

Five of seven children who were managed for osteopetrosis had either cervical spondylolysis or lumbar spondylolysis, or both. Three of the children had lumbar spondylolysis only, one had cervical spondylolysis only, and one had spondylolysis in both areas. The five patients were followed for an average of forty-one months (range, sixteen to seventy-two months) after the diagnosis of spondylolysis was made. Three of the patients were managed non-operatively with a lumbosacral corset or a thoracolumbosacral orthosis. One of these three patients, who also had cervical spondylolysis and neck pain,
