[Occupationally-induced degenerative discopathies in the area of the lumbar spine].

A case-control study comprising 188 male and 160 female cases from an orthopedic's cabin explained occupational risk factors for osteochondrosis, spondylosis, and spondylarthrosis of the lumbar vertebra. The controls (109 men, 130 women) steaming mainly from other physician's offices were free from musculoskeletal symptoms. Age-adjusted odds ratio (OR) were calculated for both sexes. In women elevated ORs were found for working in standing position (OR 2.2 CI 1.31-3.85), for repetitive work (OR 1.9 CI 1.17-3.36), for lifting heavy things up to 20 kg (OR 2.1 CI 1.24-3.67), for working as a secr

A controlled study of diffuse idiopathic skeletal hyperostosis. Clinical features and functional status.

Diffuse idiopathic skeletal hyperostosis (DISH) is a common but little-studied disorder in the elderly that is infrequently recognized by physicians. Its prevalence in adults over 40 years of age is estimated at 3.8% for men and 2.6% for women. The present case-control study evaluated the history of pain and stiffness, radicular pain and enthesitis, physical findings on the musculoskeletal examination, and level of physical and psychologic disability in 130 persons: 56 patients with DISH, 43 control patients with spondylosis of the lumbar spine, and 31 healthy control patients. DISH patients w

[The validation of a procedure for the surgical treatment of intervertebral disk hernias of the cervical spine].

Thirty-four consecutive patients with cervical disc herniation underwent surgical treatment at Burdenko Neurosurgical Institute. The clinical signs and diagnostic findings as well as surgical results were analyzed. Predominance of direct spinal cord involvement, characteristic changes in the anterior spinal artery and afferent vessels confirm the fact that cervical disk herniation causes vascular changes to take place, which in turn affect neural structures. Microsurgical techniques with maximum preservation of bone structures and vertebral endplates is mandatory. If multiple disk herniations

Completely dislocated hangman's fracture with a locked C2-3 facet. Case report.

The authors report a rare case of a hangman's fracture involving complete dislocation of C-2 onto C-3, accompanied by a C2-3 locked facet and asymptomatic bilateral vertebral artery injuries. The patient, a 25-year-old man who sustained a neck injury in an industrial accident, presented with a mild central spinal cord syndrome. His initial lateral cervical radiograph showed complete anterior dislocation of the C-2 body onto C-3, bilateral neural arch fractures, and a unilateral locked facet. The mechanism was likely flexion and compression. The grossly unstable spine and the locked facet were

Vulnerability of vertebral artery in anterolateral decompression for cervical spondylosis.

This study aimed to provide anatomic data for the location of the vertebral artery and offer an optimal approach for lateral cervical decompression that minimizes the risk of injury to the vertebral artery. Anatomically, there has been little study documenting the safe zone to prevent vertebral artery injury during the resection of the uncinate process or uncovertebral joint during the lateral decompression of the nerve root. The transverse foramen and its related parameters were measured on dry cervical spines from C3 to C7. The cadaveric cervical spines were dissected to determine a method f

Increased cerebrospinal fluid nitrite and nitrate levels in patients with lumbar spondylosis.

Nitric oxide is thought to mediate the cytotoxic effects of certain neurologic diseases. To investigate whether overproduction of nitric oxide may play a role in lumber spondylosis, we compared the levels of nitrite and nitrate in the cerebrospinal fluid (CSF) from patients with lumbar spondylosis with those from a control group by automated flow injection analysis. CSF levels of nitric oxide metabolites were significantly higher in patients with lumbar spondylosis compared with the control (p < 0.01). Nitric oxide may play a role in lumbar pain or nerve damage in sciatic and the CSF nitrite/n

Cervical spondylotic radiculopathy involving two adjacent nerve roots. Anterior decompression through a single level intervertebral approach.

We describe a single level intervertebral approach to decompress two adjacent involved nerve roots in cases of cervical spondylosis. The operation was undertaken in 4 patients. We carried out discectomy, partial excision of the vertebral body with removal of the anteromedial part of the pedicles, removal of osteophytes and excision of the posterior longitudinal ligament, followed by an anterior interbody fusion. Fusion was achieved with the spine in normal lordosis and without complications. Pain and motor weakness was relieved in every case. This procedure can maintain movement at one additio

Anatomical observations of the developmental and spondylotic cervical spinal canal in South African blacks and whites.

Cervical vertebral columns (214) of South African black and white cadaver populations were examined and measured to determine the mean midsagittal and transverse diameters of cervical vertebral foramina. Findings reveal that blacks have significantly narrower midsagittal and transverse diameters of their cervical vertebral foramina than do whites. The significance of the findings with particular regard to spondylosis is discussed in some detail. In a discussion on the causes of differences in canal size in the two ethnic groups, the role of heredity and environment was addressed and attention

Monitoring of median nerve somatosensory evoked potentials during cervical spinal cord decompression.

We evaluated the intraoperative utility of monitoring median nerve somatosensory evoked potentials (SEPs) in 31 consecutively hospitalized neurosurgical patients (mean age 55.3 +/- 2.1 years) who underwent spinal cord decompression for cervical herniated disc, spondylosis, or tumor. Pre- and postoperative standard neurological examinations included evaluation of motor function, sensory responses, gait, tone, and reflexes. Evoked potentials were recorded from clavicular Erb's (N9) and contralateral cortical (N20) points. Intraoperatively, SEP measurements were obtained after the onset of anesth

Two-dimensional MRI at 1.5 and 0.5 T versus CT myelography in the diagnosis of cervical radiculopathy.

A prospective comparison was made of standard two-dimensional MRI sequences, at both high and midfield strength, with CT myelography in 23 patients with cervical spondylosis. MRI is adequate for assessment of cord compression, where high field strength is superior to midfield strength. MRI using 4-mm sections is inadequate for presurgical assessment of root compression. It remains to be proven whether thin-section white-CSF volume sequences or gadolinium-enhanced volume studies can replace CT myelography.

[Late-onset adrenomyeloneuropathy perplexed with spondylosis. A case report].

We reported a 60-year-old man with late-onset adrenomyeloneuropathy (AMN). He had been well until 10 years before entry, when he noticed numbness in the legs with gait difficulty; symptoms worsened gradually with additional urinary disturbance. Transient improvement occurred after cervical and lumbar spinal operation under the diagnosis of spinal spondylosis, while his spastic gait got worse. Neurological examination on admission disclosed bilateral horizontal nystagmus, ataxic and spastic gait, increased patellar tendon reflexes, Chaddock sign, and impaired deep sense in the lower limbs with

[Acute paraparesis associated with dorsal diastematomyelia in an adult].

Diastematomelia presenting in adult life has long been considered a rare occurrence. With the development of modern imagining techniques, cases of adult are being discovered with increasing frequency, but their exact number remains unclear. It occurs most often in the lumbar region, is rarely located above the level of T3 and it present clinical features such as deformity of the food, peculiar posture, or gait disturbance. The symptoms and signs of diastematomelia usually are recognized in children. We are reporting the case of an adult patient who had a forme frustre of Sjögren's syndrome and

The comparison of a rapid screening MR protocol with a conventional MR protocol for lumbar spondylosis.

PURPOSE: To assess the degree of agreement between a rapid T2-weighted fast spin-echo protocol and a detailed protocol for the detection of lumbar spondylosis and to determine the sensitivity and specificity of the rapid protocol with respect to the detailed protocol. MATERIALS AND METHODS: Forty-eight patients with low back pain were studied with both a detailed four-sequence protocol and a two-sequence rapid screening protocol (acquisition time of 28 and 2.5 min for the detailed and screening protocols, respectively). The two protocol groups were numerically graded for the findings of lumba

Cervical spondylosis: three-dimensional gradient-echo MR with magnetization transfer.

PURPOSE: To compare a three-dimensional Fourier transform (3DFT) gradient-echo pulse sequence with magnetization transfer at a short echo time against standard 3DFT gradient-echo technique in the evaluation of cervical spondylosis, specifically addressing the effects of motion and susceptibility artifacts on the dimensions of the neural foramina and contrast at the cerebrospinal fluid (CSF)-spinal cord interface. METHODS: Ten patients with clinically suspected cervical spondylosis were examined with axial MR imaging using both our standard 3DFT gradient-echo sequence and a 3DFT gradient-echo

Thoracic spondylosis presenting with spastic paraparesis.

Spondylotic change of the spine is common in the cervical and lumbar regions and may present with compression of the spinal cord and nerve roots. Myelopathy due to degenerative disease in the thoracic spine is exceptional. Only a few cases have been reported in the literature and these reports have described disease in the lower four thoracic segments. We report a case of paraparesis caused by degenerative disease in the upper thoracic spine which has not previously been described.

[Bone density measurements of the proximal femur in relation to torsion of the femoral neck].

The DEXA-method allows for bone mineral density measurements of different regions of the skeletal system. It is known that results obtained from the spine with this two-dimensional method may be biased in cases of spondylosis, spondylarthritis, and compression fractures; similarly biased results can be expected at the proximal femur due to morphometric peculiarities. The influence of anteversion and rotation of the femoral neck has been investigated in the present study. The results indicate that errors due to rotation as occurring in incorrect positioning or in arthrogenic variabilities mainl

An unusual cause of dysphagia.

A case of dysphagia caused by anterior cervical osteophytes is presented. Although dysphagia and cervical spondylosis are common presenting problems, they are often unrelated to each other. The association between the two should, therefore, be accepted with caution; adequate investigation is necessary to avoid misdiagnosis. Review of the literature showed that this condition is relatively rare. The most likely mechanism of dysphagia is interference with swallowing at the pharyngo-oesophageal junction, although osteophytes in the lower cervical spine may also interfere with oesophageal peristal
