[Needle electromyography in the thoracic paraspinal muscles of motor neuron disease].

Usefulness of needle electromyography (EMG) in the thoracic paraspinal muscles was investigated in 22 patients with amyotrophic lateral sclerosis (ALS). All patients revealed denervation changes in the thoracic paraspinal muscles, though the EMG findings were insufficient to fulfill the WFN criteria of lower motor neuron sign. We could not diagnose three patients as having ALS at their first visits by conventional EMG. One patient had restricted neurogenic change within one limb and two patients had cervical spondylosis causing difficulty to diagnose anterior horn cell involvement at the cervi

Bilateral multilevel laminectomy with or without posterolateral fusion for cervical spondylotic myelopathy: relationship to type of onset and time until operation.

The authors retrospectively evaluated the relationship of several preoperative factors in 69 patients who had myelopathy due to multilevel cervical spondylosis without ossification of the posterior longitudinal ligament treated with Kirita's bilateral wide laminectomy. In 34 patients with focal instability or malalignments, posterolateral fusion was also combined. The clinical results at an average follow-up period of 3.5 years (range 1-10 years) after operation in the groups that had and had not undergone fusion were equally satisfactory, and preoperative focal instability was believed to be

Degenerative changes following anterior cervical discectomy and fusion evaluated by fast spin-echo MR imaging.

PURPOSE: To review pre- and postoperative fast spin-echo (FSE) MR images of disc herniation and spondylosis in patients after spinal cervical surgery. MATERIAL AND METHODS: Data were reviewed of 68 patients after anterior discectomy and fusion (ADF) operations using the Cloward technique with solid single level (C5-C6 or C6-C7) or 2-level fusions (C5-C7). The average interval from surgery to review was 37 months. Age- and sex-matched controls without neck problems were examined. RESULTS: Preoperatively, the fusion groups had a higher incidence of protruded disc, and anterior and posterior os

[Postoperative radiotherapy in the prevention of heterotopic ossification after endoprosthetic hip joint replacement].

BACKGROUND: Following total hip arthroplasty high-grade heterotopic ossification occurs with a range of 0.5 to 27%. Local postoperative hip irradiation using different dosage and fractionation schedules reduces the incidence in high-risk cases on an average of 8%. The present retrospective study compares results after conventionally fractionated and high-dose radiotherapy. PATIENTS AND METHOD: A total of 50 hips (46 patients) from 2 different risk groups had complete follow-up. In group I (40 hips [80.0%]) radiotherapy was indicated after surgical excision of pre-existing high-grade heterotop

[Comparative assessment of conventional spin echo sequences and turbo spin echo in the study with magnetic resonance of the lesions of the spinal cord].

In the last few years, new magnetic resonance (MR) pulse sequences called Fast or Turbo Spin Echo (TSE) sequences have become available. This kind of T2-weighted images is particularly useful for the study of spondylosis and degenerative spinal conditions, because it both reduces involuntary motion artifacts and its acquisition time is shorter than that of conventional SE T2-weighted images. Our study was aimed at assessing the diagnostic gain of this new type of pulse sequences in intrinsic spinal cord conditions. Therefore, we acquired TSE and conventional SE sequences, consequently, in 36 p

Paravertebral muscles in disease of the cervical spine.

OBJECTIVES: Cervical spine disorders are common in the older population. The paravertebral muscles are essential to the support and stabilisation of the cervical spine but have been little studied. The aim was to determine whether pathological changes develop in these muscles in patients with severe cervical spine disease, which, if present, might contribute to the pathogenesis and symptomatology of their disorder. METHODS: Open biopsies of superficial and deep paravertebral muscles were obtained during the course of surgical procedures to alleviate cervical myelopathy. Most of these patients

[Symptoms and pathological anatomy of the degenerative cervical spine].

Clinical symptoms and patho-anatomic changes in cervical myelopathy due to spondylotic changes are described. The leading symptoms are numbness and clumsiness of upper and lower extremity, mostly combined with gait disturbances. Muscle wasting primarily on the upper extremity leads to the myelopathic hand. Cervical myelopathy can be classified into five main groups: 1. Spastic tetraparesis with numbness and hyperreflexivity of upper and lower extremity. The majority of patients present with the myelopathic hand. 2. Spastic paraparesis with lesion below C6. 3. Spastic tetraparesis, mild or mode

[Surgical treatment of cervical spondylotic myelopathy in the Czech and Slovak Republic in 1994].

The authors investigated the state of surgery on account of cervical spondylosis and in particular spondylogenic cervical myelopathy in the Czech and Slovak Republic in 1994. They assessed the number of operated patients, surgical methods, indication criteria and the diagnostic strategy of different departments. The total number of patients operated on account of cervical spondylosis in 1994 was 347, incl. 113 on account of cervical spondylogenic myelopathy. As to surgical techniques in the majority the anterior approach was selected (Smith-Robinson's method). Laminectomy was performed in 14 a

[The operative treatment of the cervical spondylotic myelopathy. Indication and technique].

The cervical spondylotic myelopathy is the most frequent spinal dysfunction. Its spontaneous course is very variable, from progress to stagnation. This makes prognosis and decision-making difficult for conservative or operative treatment. The authors attempt to provide some basis for decision-making by observing the individual course of neural dysfunction, consideration of morphometric data and the use of functional scores. To advise a patient, the outcome of conservative and operative treatment as described in the literature has to be considered, as well as the possible complications. The adv

"Aging bull'.

An old bull, it is said by those who know, can have his troubles. Included among these are vertebral osteosclerosis and ankylosing spondylosis; this stiffening up limits, rather than accentuates, the value and reproductive potential of a stud bull past his prime. Associated with these abnormalities, however-and not seen in age-matched cows of comparable breeds-are fascinating endocrine neoplasms suggestive of a pattern that could be productive as a model of human hereditary endocrine abnormalities. Adjacent to the thyroid gland in other vertebrates are ultimobranchial bodies that are incorpora

Motor recovery following spinal cord injury associated with cervical spondylosis: a collaborative study.

A prospective multicenter study was conducted within the National Model Spinal Cord Injury System program to examine neurological deficits and recovery patterns following spinal cord injury (SCI) in individuals with cervical spondylosis and without a spinal fracture. Nineteen patients were evaluated. Sixty-eight percent presented initially with motor incomplete lesions. Of those who presented with motor incomplete injuries at their initial examination, 69 percent had less deficit in the lower than in the upper extremities, indicative of a central cord syndrome. At follow-up, 12 subjects were u

Advanced cervical spondylosis with ossification into the posterior longitudinal ligament and resultant neurologic sequelae.

Advanced cervical spondylosis (ACS) with ossification of the posterior longitudinal ligament (OPLL) may be distinguished from classical OPLL on magnetic resonance- and computed tomography-based studies by the presence within the hypertrophied posterior longitudinal ligament (PLL) of segments of punctate calcification-ossification. Between 1989 and 1993, 50 patients with an early form of classical OPLL, variably associated with ACS, had cervical surgery. Averaging in their mid-40s in age, and exhibiting signs of radiculopathy or myeloradiculopathy, patients were uniformly managed with anterior

Load sharing principles: the role and use of anterior structural support in adult deformity.

The goals of load sharing are to restore balance between posterior tension and anterior compression, ideally to maintain the instantaneous axis of rotation just posterior to the anulus and to thereby minimize the forces on the reconstruction. To accomplish this, the surgeon should understand the anatomic column deficiencies of the patient's pathology and the appropriate options for anterior and posterior reconstruction.

A biomechanical evaluation of internal lumbar laminoplasty: the preservation of spinal stability during laminectomy for degenerative spinal stenosis.

The long-term success of surgical management of spinal stenosis by decompressive lumbar laminectomy is predicated partly by the maintenance of intervertebral stability. Excessive bone removal, especially of the pars interarticularis, may predispose the spine to iatrogenic segmental instability and spondylolisthesis. Removal of too little bone may be inadequate to relieve stenotic or radicular symptoms. Wide decompression with concomitant arthrodesis has been advocated, but arthrodesis compounds the surgical complexity and increases the risks of surgical treatment. Internal lumbar laminoplasty

Results of a trial of anterior or circumferential lumbar fusion in the treatment of severe isthmic spondylolisthesis in young patients.

The radiological and clinical results in 27 patients operated on for severe isthmic spondylolisthesis at age < 20 years with uninstrumented anterior interbody fusion alone in 11 or combined anterior and posterolateral (circumferential) fusion in 16 are reported. The mean follow-up time was 5.9 +/- 2.4 years. The two groups were comparable with regard to age at operation, sex, preoperative symptoms, mean preoperative slip and sagittal rotation angle. The follow-up assessment, made by an independent observer, included interview, Oswestry disability index, physical examination, functional tests,

Deep vein thrombosis in lumbar spinal fusion: a prospective study of antiembolic and pneumatic compression stockings.

We prospectively studied the incidence of deep vein thrombosis (DVT) of the thigh in 117 patients having posterior lumbar spinal fusion with instrumentation and bone grafting for degenerative disk disease or spondylolisthesis. Patients with neoplasm, infection, trauma, or history of DVT were excluded. Patients were randomized into two groups. In the operating room, group 1 patients were placed in thigh-high antiembolic compression stockings (TED hose), and group 2 patients were placed in antiembolic stockings and pneumatic compression stockings.
