Outcome after anterior spinal fusion has mainly been studied radiologically and reported fusion rates vary greatly. The aim of this study was to investigate radiological and long-term clinical outcome. The study comprised 120 consecutive patients, operated on during the period 1979-1987, with single-or two-level anterior interbody spinal fusion due to disc degeneration or isthmic spondylolisthesis with lumbar instability. In 64 patients a supplemental facet joint fusion was performed. Clinical outcome was evaluated 5-13 years after surgery using the patient-administered Dallas Pain Questionnai

A wide variety of lumbar spinal conditions have been managed by spinal fusion. Results appear better when the diagnosis is specific and related to definable instability or deformity in patients with a stable psychologic state. Success rates are higher in isthmic spondylolisthesis, unstable spinal stenosis syndromes (degenerative spondylolisthesis, degenerative scoliosis), and in patients with objective segmental instability. Variable success rates are reported for disk-related low back pain conditions and in patients with failed previous surgery. Instrumentation appears to be beneficial in sit

This study, conducted by a group of neurosurgeons who devote a large portion of their professional time to the treatment of degenerative lumbar spine lesions, was prompted by the dramatic increase in the number of lumbar spinal fusion procedures performed over the last few years in a broad spectrum of disorders ranging from chronic incapacitating low back pain to lumbar spinal stenosis. In the authors' experience, lumbar spinal fusion is rarely warranted and often of dubious efficacy. To investigate this contradiction, the authors reviewed the medical literature on lumbar spinal fusion for the

PURPOSE OF THE STUDY: Posterior lumbar interbody fusion (PLIF) as described by Cloward, with laminectomy of the entire separate neural arch in spondylolisthesis, is a difficult operation. The purpose of this study is to evaluate the results of PLIF with autologous bony graft, without reduction or instrumentation in grade I and II spondylolisthesis.
MATERIAL: The authors report the results of a series of 36 cases of spondylolisthesis (out of a series of 160 cases) operated on with PLIF, after a minimum of 10 years of follow-up (mean: 11.8 years, 10-16 years). 9 patients were lost for follow-up

Reported is the outcome for 25 patients in whom spondylolisthesis with radicular pain was treated by posterolateral fusion alone (Group A). These outcomes are compared with those obtained in 23 other patients with the same symptomatology and spondylolisthesis treated by root release and posterolateral fusion (Group B). Most patients had Grade I or II isthmic spondylolisthesis. Results were assessed functionally and radiographically with an average followup of 32 months. Posterolateral fusion in situ provided excellent or good results in 88% of patients according to the modified classification

The manifestation of a pyogenic spondylitis only in the dorsal part of a spinal segment is very rare and causes even more diagnostic problems than the typically anterior located spondylitis. We are reporting on a patient with known isthmic spondylolisthesis who developed the inflammatory process primarily in the intervertebral joint and in the interarticular portion. For a few months the cause of his complaints was thought to be due to spondylolisthesis till radiological adn laboratory methods lead to the correct diagnosis and adequate therapy.

The treatment of spondylolisthesis-in particular the indication of operation-is based on clinical symptoms as well as radiological criteria. For radiological techniques, the lateral profile, especially in function tests, is of importance. As a basic requirement the grade of Meyerding should be evaluated on x-ray in the exact lateral profile. We have observed, that even a slight rotation of the spine leads to misjudging of the real degree of vertebral dislocation. In 30 patients with spondylolisthesis of the lumbar spine the x-rays, made in exact lateral position, were compared with x-rays in 1

Treatment of spondylolisthesis is discussed differently even nowadays. In addition to that long term results of surgical reduction and stabilization by internal fixation are rare. Therefore the evaluation of the data of 61 patients treated operatively with the sacral plate by Schöllner with a mean follow-up of more than 10 years seemed to be justified. Additionally 48 out of 61 patients could be reexamined at follow-up and answered a questionnaire to measure their subjective status. Mean age at the time of surgery averaged almost 18 years. 14 male and 47 female patients were treated. Sagittal

STUDY DESIGN: Lumbosacral spondylolisthesis was simulated using four embalmed human spines, and the path of the L5 nerve was studied.
OBJECTIVES: To quantify the change in length of the L5 nerve root associated with reduction of spondylolisthesis, correction of slip angle, and changing disc height.
SUMMARY OF BACKGROUND DATA: Stretch injury to the lumbar nerves remains a complication of spondylolisthesis reduction. To date, no anatomic studies have been performed to quantify this effect of reduction on the lumbar nerves.
METHODS: The L5 vertebral body and the sacrum of four embalmed human s

The correlation between facet joint asymmetry and disorders of the lumbar spine was analyzed in a study of plain radiographs, computed tomograms and magnetic resonance images from 76 normal subjects and 173 patients who underwent lumbar surgery. Of 76 normal controls who were studied by means of plain radiographs, only 21 (27.6%) had facet asymmetry at the level of L4-5 or L5-S1, while 84 (48.6%) of 173 patients who were studied with plain radiographs, and partially with CT or MRI, were noted to have facet asymmetry. The difference was highly significant (P < 0.01). This study supports the eti

Combined anterior and posterior fusion with posterior instrumentation may be indicated in the treatment of select cases of L5-S1 spondylolisthesis. The instrumentation, however, is expensive and usually bulky, occasionally requiring removal. In an effort to avoid these problems, an L5-S1 paralaminar screw technique was developed for posterior stabilization after an L5-S1 anterior interbody fusion. The technique involves the placement of cortical screws from the base of the articular process of S1 to the pedicle of L5. This study evaluates the anatomic applications and clinical results of this

Five cases of degenerative subaxial spondylolisthesis of osteoarthritic etiology associated with significant cervical spondylotic myelopathy are described with treatment recommendations. Diagnostic studies included cervical plain films with flexion/extension, magnetic resonance imaging, and computed tomography/myelography. All patients had at least 2 mm of spondylolisthesis at one or more levels, and two patients had movement evident on flexion/extension films. Posterior laminectomy with AME (American Medical Electronics, Richardson, TX, U.S.A.) lateral mass plate and bone fusion was successfu

Congenital segmental spinal dysgenesis is a rare lesion that usually is associated with neurological deficits. Affected infants who are neurologically intact may suffer progressive kyphosis and subsequent neurological deterioration. Results of external casting, laminectomy with spinal fixation, or bone grafting of the defect have been disappointing. The authors present a successful staged surgical management scheme consisting of complete resection of the dysraphic vertebral bodies, maintenance of normal vertebral distance with rib strut grafting, and fusion with autogenous bone.
