Contribution of rare variation to degenerative orthopedic diseases.

OBJECTIVE: Degenerative orthopedic diseases (DODs) such as osteoporosis, osteoarthritis, spondylosis, spinal stenosis, and disc herniation are common disorders. Both common and rare genetic risk factors may contribute to DODs, but few large-scale whole-exome sequencing studies elucidating the contribution of rare variations to DODs have been published. The updated version of the Astra Zeneca portal (https://azphewas.com) was used to access gene collapsing analysis of rare variations for DODs. METHOD: One published UK Biobank portal: the updated Astra Zeneca portal based on whole genome sequen

Axial pain after anterior cervical discectomy and fusion: risk factor analysis and imaging evaluation.

BACKGROUND: Axial pain can occur as a complication following anterior cervical discectomy and fusion (ACDF), which is frequently regarded as a complication of posterior cervical surgery and is often overlooked after ACDF. However, the risk factors of postoperative axial pain (PAP) remain incompletely elucidated. This study aimed to analyze the risk factors and radiological assessment of PAP in patients undergoing ACDF. METHODS: Individuals undergoing ACDF between January 2020 to March 2023 were included. Participants were categorized into two groups by PAP presence. Clinical parameters were c

Conservative Management of a Symptomatic Anterior Limbus Vertebra in a Non-athletic Patient: A Case Report.

Limbus vertebra (LV) is a frequently underdiagnosed spinal anomaly resulting from intraosseous herniation of nucleus pulposus material during skeletal immaturity, often discovered incidentally but capable of causing chronic low back pain (LBP) and neurological symptoms. We present the case of a sedentary adult with persistent LBP and paresthesia, whose imaging revealed an anterior LV at L5/S1 with concomitant intervertebral disc degeneration. Unlike most reported cases involving athletes, this case highlights LV as a cause of symptoms in non-athletic populations. The patient achieved significa

Galectin-4: A Novel Mediator of Human Intervertebral Disc Degeneration.

BACKGROUND: Intervertebral disc (IVD) degeneration is associated with severe clinical symptoms including chronic back pain. Galectins are a family of carbohydrate-binding proteins, some of which can induce functional disease markers in IVD cells and other musculoskeletal tissues. Galectin-4 and -8 were shown to trigger disease-promoting activity in chondrocytes, but their effects on IVD cells have not been investigated yet. METHODS: IVD specimens from 36 patients with spondylosis, spondylolisthesis, and scoliosis were assessed immunohistochemically for the presence of galectin-4 and -8. The d

Diagnostic value of upright-supine imaging and its association with postoperative outcomes in degenerative lumbar spondylolisthesis-a 2-year patient-reported outcome measure-based comparison with flexion-extension radiographs.

BACKGROUND CONTEXT: The optimal surgical treatment of degenerative lumbar spondylolisthesis (DLS) is a subject of ongoing debate, particularly regarding the choice between decompression-only and fusion surgery. Flexion-extension radiographs (FER) are commonly used to assess segmental instability and guide this decision, but in symptomatic patients, their reliance on active patient motion can lead to underestimation of instability that may be associated with postoperative outcomes. Upright-supine imaging (USI), which compares upright radiographs with supine magnetic resonance imaging, assesses

Radial shockwave therapy versus short-wave diathermy for pain, neuromechanical, and functional outcomes in older adults with lumbar spondylosis: a randomized controlled trial.

BACKGROUND: Lumbar spondylosis is a common degenerative condition in older adults, leading to chronic low back pain, spinal stiffness, and functional limitation. This study compared radial shockwave therapy (RSWT) and short-wave diathermy (SWD) for pain and lumbar function in older adults with lumbar spondylosis. METHODS: Thirty participants aged ≥60 years were randomized to RSWT (n = 15) or SWD (n = 15). Both groups received eight sessions over four weeks, twice weekly, with identical standardized adjunctive co-interventions consisting of superficial heat and home-based lumbar and hamstring

Functional brain alterations associated with acupuncture for chronic pain: a scoping review of fMRI studies.

BACKGROUND: Chronic pain (CP) is a public health challenge recognized as involving large-scale functional brain dysregulation. Acupuncture is widely used as a non-pharmacological intervention for CP, yet its central mechanisms remain incompletely understood. fMRI provides an approach for investigating acupuncture-related brain alterations in CP. METHODS: Eight databases were searched from inception to March 27, 2025 for fMRI studies investigating acupuncture's central effects in CP. Eligible studies included randomized controlled trials and observational studies involving migraine, knee osteo

Large language models for lumbar spondylolisthesis detection: a multi-center pilot comparative radiographic accuracy study.

INTRODUCTION: Lumbar spondylolisthesis remains a radiographic diagnosis. With the improvement of large language models (LLMs), such as ChatGPT, in interpreting multimodal images, many patients have turned to LLMs for diagnostic insight. Yet the diagnostic reliability of emerging LLMs for spinal pathologies remains understudied. This study sought to evaluate the performance of both ChatGPT-4o and ChatGPT-5 in detecting lumbar spondylolisthesis on standing lateral lumbar radiographs from a public imaging dataset evaluated by expert spine surgeon consensus. METHODS: From the VinDr-SpineXR datase

The prevalence of isthmic and degenerative lumbar spondylolisthesis: an analysis of 1376 patients.

INTRODUCTION: Typically, spondylolisthesis is an asymptomatic spinal condition that is often captured accidently in radiographic studies. The limited studies reviewing incidence primarily used lateral radiographs, which lack the granularity of advanced imaging. In response, computed tomography (CT) has been recommended to enhance the accuracy of spondylolisthesis diagnosis (degenerative versus isthmic). In the present study, we sought to determine the prevalence of isthmic and degenerative spondylolisthesis using CT imaging. METHODS: We conducted a retrospective study of 1,680 patients who un
