Microsurgical Resection of Craniocervical Junction Meningioma via A Far-Lateral (Paracondylar) Approach.

Craniocervical junction meningiomas compress the ventral medulla and may encase the vertebral artery, making resection challenging. This operative video demonstrates a far-lateral paracondylar approach with C1 laminectomy and dentate-ligament division for broad exposure and safe resection. A far-lateral paracondylar approach (<25% condyle drilling) was performed with C1 laminectomy and extradural mobilization of the V3 segment of the vertebral artery; intradural C1-C2 dentate-ligament division enabled controlled medialization of the spinal cord for circumferential contralateral dissection. Maternal metabolomic signatures for risk of preterm and early term birth in African American women.

BACKGROUND AND PURPOSE: Compared to healthy controls, adult patients with Sickle Cell Disease (SCD) are anemic, and therefore have higher cardiac output and Cerebral Blood Flow (CBF) to maintain brain oxygenation. They also demonstrate comparatively more cognitive deficits due to either overt strokes or silent cerebral ischemia. However, there are few correlative studies between CBF and cognitive deficits, specifically processing speed in SCD. Such studies are important to develop biomarkers of central brain processing and ischemia for diagnosis, prognosis, and evaluating the effectiveness of

A confounding pediatric spinal cord injury: Anterior, central, or both?

Pediatric spinal cord injury (SCI) most commonly affects the cervical region. Central cord syndrome most often occurs in the lower cervical injury due to hyperextension injury, while anterior cord syndrome is primarily due to vascular infarction after hyperextension injury. An unusual case of a pediatric patient who physically presented with central cord syndrome but radiologically had evidence of anterior spinal artery syndrome is described.A two-year-old male presented after a fall from three feet with flaccid upper extremities and dysesthesias but maintained functional strength in bilateral

Posterior C1-C2 fixation for atlantoaxial instability due to a dysplastic C1 arch, complicated by an accidental vertebral artery injury managed with intravascular intervention and salvage contralateral unilateral fusion.

Posterior fixation with fusion is recommended in symptomatic congenital anomalies of the upper cervical spine. Altered vertebral anatomy in such cases increases the likelihood of intraoperative complications, where injury to the vertebral artery (VA) may result in cerebellar and dorsolateral medulla oblongata ischemia. Prompt and appropriate intervention is paramount if complications arise. We present a patient with a dysplastic C1 arch, leading to atlantodental arthrosis and anterior C1 subluxation with resultant compressive myelopathy of the spinal cord. During elective surgery, a VA injury

Cervical chordoma en bloc resection and staged suprascapular nerve transfer reanimation: illustrative case.

BACKGROUND: Cervical chordomas offer unique challenges to en bloc resection given their proximity to complex anatomy. Achieving tumor-free margins often requires extensive reconstruction that can limit radiotherapy dosing and postoperative visualization due to metal artifact, compounded by the unavailability of cervical carbon fiber implants. Multidisciplinary multistage approaches focused on oncological resections and maintenance of neurological function can improve quality and quantity of life. The authors present such an approach to the resection of a cervical chordoma with nerve transfer f

Diagnosis of C5 segmental vertebral artery using magnetic resonance angiography.

PURPOSE: The vertebral artery (VA) typically enters the subarachnoid space at the atlanto-occipital region. However, segmental variations can occur, with the VA entering the spinal canal at atypical levels. While a C3 segmental VA has been reported, no prior studies describe a C5 segmental VA. This case represents the first documented occurrence of this anomaly. METHODS: An 8-year-old girl underwent brain magnetic resonance imaging for headache screening, which incidentally revealed an abnormal VA course. RESULTS: Imaging revealed the absence of the left VA at the proximal V2 segment. Instea

Neurovascular considerations in patients with Down syndrome and moyamoya syndrome.

In this article, we describe a rare and complex case of moyamoya syndrome in a 7-year-old boy with Down syndrome and atlantoaxial subluxation. The patient presented with an ischemic stroke in the left hemisphere and cervical cord compression with increased cord edema. Diagnostic digital subtraction angiography revealed unique patterns of vascular involvement, with retrograde flow through the anterior spinal artery, ascending cervical artery, occipital artery, and multiple leptomeningeal arteries compensating for bilateral vertebral artery occlusion. This case underscores the underreported phen

Vertebral artery stenosis from osteophyte: A systematic review and case series.

BACKGROUND: Rotational vertebral artery syndrome, also referred to as Bow Hunter's syndrome (BHS), manifests when the vertebral artery (VA) is compressed following head rotation. This compression is often caused by an osteophyte and may lead to symptoms of a posterior stroke. This systematic review aims to shed light on the current management strategies for BHS resulting from osteophytes. Additionally, we present two illustrative cases where the VA compression by an osteophyte was effectively resolved by complete resection of the problematic bone spur. METHODS: A literature search was conduct

Analysis of Failed Atlantoaxial Reduction: Causes of Failure and Strategies for Revision.

OBJECTIVE: The craniovertebral junction (CVJ) presents intricate anatomical challenges. In severe or irreducible malformations, complications such as reduction loss and fixation failure may occur, necessitating revision surgery. The posterior facet joint distraction and fusion (PFDF) technique, offers a solely posterior approach for revisions. Hence, we delineate varied revision scenarios, proposing surgical strategies and technical details to enhance outcomes and mitigate risks, thereby enriching the neurosurgical community's repertoire. METHODS: This was a retrospective cohort study, analyz

Cervical anterior spinal artery infarction associated with anomalous vertebral artery: a case report.

We report a unique case of cervical anterior spinal artery (ASA) infarction in a 49-year-old male with hypercholesterolemia and sleep apnea. The patient experienced sudden cervical pain, quadriparesis, areflexia, and urinary incontinence after swallowing a large food bolus. Imaging revealed an infarction at the C3-C5 levels and an anomalous right vertebral artery (VA) originating from the thoracic aorta, tightly enclosed between the aorta and a vertebral column with an anterior osteophyte. This aberrant VA was the primary vascular supply to the ASA, with no contribution from the left VA or sup

Delayed onset of anterior spinal artery syndrome caused by retropulsed bone fragment after kyphoplasty: Case report and literature review.

BACKGROUND: Percutaneous balloon kyphoplasty is a minimally invasive technique to treat refractory symptomatic osteoporotic vertebral compression fractures. A rare complication called anterior spinal artery syndrome has been documented several times in the literature after the procedure from cement embolism; however, the authors report an unusual case of anterior spinal artery syndrome following kyphoplasty through retropulsion of bone fragmentation. CASE PRESENTATION: An 83-year-old male was admitted to an acute care hospital for severe low thoracic back pain without neurological symptoms. C

Multiple Myeloma Treatment Challenges: A Case Report of Vertebral Artery Pseudoaneurysm Complicating Occipitocervical Arthrodesis and a Review of the Literature.

Multiple myeloma is a hematological neoplasm that frequently affects the spinal column. Less than a fifth of this vertebral involvement corresponds to the cervical spine and cranio-cervical junction. When there is instability or neurological involvement due to compression or deformity, approaches for anterior decompression and occipitocervical stabilization are required. The correct managment of vertebral artery aneurysm associated with occipitocervical arthrodesis requires extensive knowledge of anatomy and pathology. We present a case of a vertebral pseudoaneurysm that occurred late after th

Intracranial aneurysm as a possible complication of osteogenesis imperfecta: a case series and literature review.

Osteogenesis imperfecta (OI) is an inherited disease characterized by bone fragility due to impaired type I collagen. Although orthopedic management is improving, other complications are poorly understood. We describe three patients with OI with unruptured intracranial aneurysm (IA) detected by magnetic resonance angiography (MRA) screening of 14 patients. Case 1 was a 73-year-old woman with type 1 OI with blue sclera, vertebral compression fractures, and impaired hearing. Lumbar spine bone mineral density (BMD) was preserved (young adult mean (YAM): 86%). MRA revealed an IA in the right inter

Long Segment Anterior Cervical Discectomy and Fusion, Including C2: Surgical Pearls and Review of Our Experience.

STUDY DESIGN: A retrospective study of thirteen patients undergoing 5-level anterior spinal surgery for cervical myelopathy. OBJECTIVE: While limited literature exists in reviewing the treatment of high cervical pathology extending caudally, we believe long segment surgery beginning at C2-3 can be accomplished with good success and is an option more patients may benefit from. We aim to describe the technique in accessing the C2-C3 disk space and efficacy of treating multilevel disease beginning at the C2 vertebral body. This includes an extensive technical report and surgical pearls. SUMMARY

Vertebral arteriovenous fistula due to blunt neck trauma: A case report.

The arteriovenous fistulas (AVFs) of the vertebral artery are usually caused by iatrogenic and penetrating traumas. Vertebral AVF is rarely seen after blunt cervical trauma. A 65-year-old male patient applied to the emergency clinic due to falling from a height of about 3 m. In his neurological examination, he had weakness in the left upper limb and tinnitus in the left ear. The cervical computed tomog-raphy examination showed a Hangman's fracture, a C3 vertebra corpus compression fracture, and a C1 anterior arch fracture. There was a separation on the C1-2 transverse foramen due to the fractu

Acute Basilar Artery Occlusion and Death Secondary to Bilateral Vertebral Artery Dissection.

Vertebral artery dissection as a cause of basilar artery thrombosis is an exceedingly rare event that is associated with significant morbidity and poor outcomes. We present an unusual case of bilateral vertebral artery dissection and spinal cord compression in a 21-year-old male involved in a diving accident. The patient received limited antithrombotic therapy in pursuit of surgical spinal decompression, ultimately contributing to thrombosis of the basilar artery in the post-operative period and death following anterior cervical discectomy and fusion. Our goal is to highlight the severity of v
