OBJECTIVE: Acupotomy in the treatment of cervical spondylosis (CS) is effective and widely used in clinic, but the mechanism is still unclear. In this study, we started from treating CS with acupotomy "regulating muscles to treat bone disorders" and cervical myocyte apoptosis, the mechanism of acupotomy activates PI3K/Akt signaling pathway mediated by FGF7,10/KDR signaling axis to regulate apoptosis was explored, providing theoretical basis for clarifying the target and molecular mechanism of acupotomy for CS.
METHODS: Seventy-four SPF SD rats were divided into 9 groups, including acupotomy g

PURPOSE: Posterior endoscopic surgery has become a mainstream minimally invasive approach for the treatment of cervical spondylotic radiculopathy (CSR). Arthroscopic-assisted uni-portal spine surgery (AUSS), an emerging technique, has demonstrated favorable clinical outcomes in lumbar spine surgery; however, its feasibility and effectiveness in cervical spine surgery have not yet been reported. Accordingly, this technical note aimed to describe the surgical technique of AUSS combined with Kirschner wire anchoring for the treatment of CSR and to evaluate its preliminary clinical outcomes.

BACKGROUND: Cervical spondylosis is a degenerative disorder of the spine, frequently associated with chronic neck pain, reduced mobility, and functional impairment. Patients develop alterations in muscle tone, stiffness, and elasticity, which further contribute to disability. This study aimed to investigate the effects of a 14-day standardized rehabilitation program on the biomechanical and contractile properties of cervical and scapular muscles in patients with cervical spondylosis.
METHODS: This study used a single-group pre-post observational design on 23 patients (16 women, 7 men; mean ag

BACKGROUND: Chronic low back pain due to degenerative disc disease (DDD) and lumbar spondylosis is a leading cause of disability. Many patients exhaust conservative and interventional treatments yet remain poor surgical candidates. Spinal cord stimulation may be an effective therapy for many of these patients.
CASE PRESENTATION: A 65-year-old female with lumbar DDD and lumbar spondylosis presented with chronic low back pain refractory to conservative therapy. She previously failed acetaminophen, NSAIDs, gabapentinoids, muscle relaxants, and opioids, as well as multiple interventional procedur

OBJECTIVE: Postoperative loss of cervical lordosis remains a common and clinically relevant complication following laminoplasty, negatively affecting neck pain, neurological recovery, and long-term sagittal balance. However, reliable and easily applicable preoperative predictors for identifying patients at high risk of cervical lordosis deterioration remain limited. This study aims to investigate whether preoperative C2 slope (C2S) independently predicts cervical lordosis deterioration following laminoplasty.
METHODS: This retrospective cohort study included 179 patients who underwent cervica

BACKGROUND: Anterior disc space widening (ADSW), defined in this study as an autopsy-based category of cervical discoligamentous injury, is difficult to diagnose on postmortem computed tomography. The vacuum phenomenon (VP) has been debated as a trauma-related finding, and several studies have suggested that its presence may indicate ADSW. However, VP generation requires a negative-pressure environment and may not occur in structurally disrupted disc spaces.
METHODS: We retrospectively evaluated 395 cases that underwent autopsy and postmortem kinetic CT of the cervical spine, including neutra

INTRODUCTION: Neurogenic Thoracic Outlet Syndrome (nTOS) is characterized by compression of neural structures within the interscalene triangle of the thoracic outlet, with anatomical variations frequently implicated in its etiology. In patients with TOS, spinal nerves pass through a narrow anatomical space as they exit the foramen and traverse the thoracic inlet. The tension of the suprapleural bands extending between the C7 transverse process and the first rib, along with changes in neck position and certain anatomical variations, can lead to compression of the lower trunk. The condition of t

Currently, an association between Helicobacter pylori (Hp) infection and cervical spondylosis has been found in clinical practice. However, it is unclear whether there is a causal relationship between the two. To reveal the causal relationship between Hp infection and cervical spondylosis, we performed a two-sample two-way Mendelian randomization (MR) analysis. This study was based on a two-sample Mendelian randomized design with a genome-wide association study of Hp infection and cervical spondylosis, systematically screened for genetic instrumental variables (IV), applied inverse-variance we

BACKGROUND: When patients suspected of degenerative cervical myelopathy present with atypical neurological findings, the spine surgeons generally referred the patients to neurologists preoperatively to distinguish it from non-compressive neurological diseases. This study aimed to elucidate the diseases that should be differentiated from degenerative cervical myelopathy and the differentiation points. METHODS: We conducted a retrospective cohort study, and extracted consecutive patients who were suspected of degenerative cervical myelopathy with spinal cord compression and consulted a spine sur

Cervical spondylosis is one of the most common degenerative diseases, seriously affecting life quality. Unlike diseases with explicit lesions like cancer, hydroncus, or fracture, the degeneration of the cervical spine cannot be explicitly detected from the appearance of medical images, requiring extensive experience of doctors to interpret subtle clues. However, the extremely high incidence of cervical spondylosis coincides with a serious shortage of experienced doctors and uneven distribution of medical resources, hindering early diagnosis. We propose a cascade-ensemble deep learning framewor

Robot-assisted (RA) spinal fusion is an emerging technology for the treatment of lumbar spondylolisthesis. However, its purported advantages currently lack robust empirical evidence. This study aimed to evaluate whether RA spinal fusion offered superior outcomes compared to fluoroscopy-guided (FG) techniques in management of lumbar spondylolisthesis. This study was prospectively registered with PROSPERO (CRD420251083476). A systematic review was conducted by searching six databases for studies comparing RA and FG spinal fusion in patients with lumbar spondylolisthesis, with data collected up t
