Intradiscal vacuum phenomenon on postmortem kinetic computed tomography of the cervical spine suggests intact discoligamentous structures.

BACKGROUND: Anterior disc space widening (ADSW), defined in this study as an autopsy-based category of cervical discoligamentous injury, is difficult to diagnose on postmortem computed tomography. The vacuum phenomenon (VP) has been debated as a trauma-related finding, and several studies have suggested that its presence may indicate ADSW. However, VP generation requires a negative-pressure environment and may not occur in structurally disrupted disc spaces. METHODS: We retrospectively evaluated 395 cases that underwent autopsy and postmortem kinetic CT of the cervical spine, including neutra

A different perspective on cervical X-ray parameters in nTOS: could it play a role in pathophysiology?

INTRODUCTION: Neurogenic Thoracic Outlet Syndrome (nTOS) is characterized by compression of neural structures within the interscalene triangle of the thoracic outlet, with anatomical variations frequently implicated in its etiology. In patients with TOS, spinal nerves pass through a narrow anatomical space as they exit the foramen and traverse the thoracic inlet. The tension of the suprapleural bands extending between the C7 transverse process and the first rib, along with changes in neck position and certain anatomical variations, can lead to compression of the lower trunk. The condition of t

Exploring the causal relationship between Helicobacter pylori infection and cervical spondylosis based on the Mendelian randomization method.

Currently, an association between Helicobacter pylori (Hp) infection and cervical spondylosis has been found in clinical practice. However, it is unclear whether there is a causal relationship between the two. To reveal the causal relationship between Hp infection and cervical spondylosis, we performed a two-sample two-way Mendelian randomization (MR) analysis. This study was based on a two-sample Mendelian randomized design with a genome-wide association study of Hp infection and cervical spondylosis, systematically screened for genetic instrumental variables (IV), applied inverse-variance we

Differential diagnosis of degenerative cervical myelopathy considered in patients spine surgeons referred to neurologists: a retrospective cohort study.

BACKGROUND: When patients suspected of degenerative cervical myelopathy present with atypical neurological findings, the spine surgeons generally referred the patients to neurologists preoperatively to distinguish it from non-compressive neurological diseases. This study aimed to elucidate the diseases that should be differentiated from degenerative cervical myelopathy and the differentiation points. METHODS: We conducted a retrospective cohort study, and extracted consecutive patients who were suspected of degenerative cervical myelopathy with spinal cord compression and consulted a spine sur

Automated diagnostic of cervical spondylosis on multimodal medical images with a multi-task deep learning model.

Cervical spondylosis is one of the most common degenerative diseases, seriously affecting life quality. Unlike diseases with explicit lesions like cancer, hydroncus, or fracture, the degeneration of the cervical spine cannot be explicitly detected from the appearance of medical images, requiring extensive experience of doctors to interpret subtle clues. However, the extremely high incidence of cervical spondylosis coincides with a serious shortage of experienced doctors and uneven distribution of medical resources, hindering early diagnosis. We propose a cascade-ensemble deep learning framewor

Robot-assisted versus fluoroscopy-guided spinal fusion for lumbar spondylolisthesis: a GRADE-assessed meta-analysis on surgical parameters, clinical outcomes, and complications.

Robot-assisted (RA) spinal fusion is an emerging technology for the treatment of lumbar spondylolisthesis. However, its purported advantages currently lack robust empirical evidence. This study aimed to evaluate whether RA spinal fusion offered superior outcomes compared to fluoroscopy-guided (FG) techniques in management of lumbar spondylolisthesis. This study was prospectively registered with PROSPERO (CRD420251083476). A systematic review was conducted by searching six databases for studies comparing RA and FG spinal fusion in patients with lumbar spondylolisthesis, with data collected up t

Using artificial intelligence and a walking aid to improve lumbar stability parameter evaluation methods.

PURPOSE: To refine a method of assessing lumbar stability by applying artificial intelligence and incorporating the use of a walking aid during imaging procedures. METHODS: A software application was developed to evaluate lumbar stability parameters using machine learning and neural network models (Swin-Resnet). The intra-class correlation coefficient (ICC) was used to assess the agreement between Swin-Resnet and the evaluations conducted by three spine surgeons, each with over ten years of experience. Subsequently, traditional flexion-extension radiographs (FET) and flexion-extension radiogra

Literature Review: Management of Primary Atlantoaxial Osteoarthritis and Associated Occipital Neuralgia.

Primary atlantoaxial osteoarthritis, or C1-C2 (cervical 1-cervical 2) lateral mass arthritis, is an increasingly recognized but often underdiagnosed source of upper cervical and occipital pain, particularly in older adults. Unlike subaxial cervical spondylosis, which typically causes diffuse neck or upper back pain, this condition usually produces localized pain in the upper neck, occiput, or behind the ear, without frequent neurological symptoms. Clinical presentation is often unilateral, with pain exacerbated by lateral flexion and rotation, and diagnosis relies on radiographic and magnetic

Beyond the pars defect: multifidus degeneration as an independent predictor of vertebral slippage severity in isthmic lumbar L5 spondylolisthesis.

INTRODUCTION: Isthmic lumbar spondylolisthesis (ILS) results from a defect of the pars interarticularis, leading to anterior vertebral displacement. We hypothesize that paraspinal muscle atrophy contributes to the degree of vertebral slippage in symptomatic ILS. METHODS: This retrospective cross-sectional study included 78 patients (46 females, 32 males). Fat infiltration (FI) and height-adjusted functional cross-sectional area (HI) of the multifidus (MF), erector spinae (ES), and psoas (PS) muscles were quantified applying the Otsu thresholding method. Multivariable linear regression was used

Impact of lateral plate and screw fixation on long-term radiographic subsidence in lateral lumbar interbody fusion.

OBJECTIVE: Lateral lumbar interbody fusion (LLIF) can be performed as a stand-alone procedure, with lateral plate and screw fixation, or with supplemental posterior instrumented fusion. In vitro biomechanical data suggest that lateral plate fixation limits segmental motion. The aim of this study was to evaluate the impact of lateral screw and plate fixation on the rate and grade of long-term radiographic implant subsidence after LLIF. METHODS: A review of patients who underwent LLIF for degenerative lumbar spondylosis from 2014 to 2022 at two quaternary referral centers was conducted. All pat

Intraoperative neuromonitoring use in posterior endoscopic cervical decompression: can it reduce complications?

PURPOSE: This study aimed to address the challenges and explore potential solutions for improving IONM utilization. Posterior endoscopic cervical decompression (PECD) relieves pressure on the exiting nerve root and spinal cord, potentially leading to improvements in patient symptoms. The routine use of intraoperative neuromonitoring (IONM) currently remains debatable, despite its ability to provide additional real-time neurological observation beyond standard practice. METHODS: In this retrospective cohort study, we investigated the clinical data of 140 patients with single-segment cervical s
