Spondylolysis of the sacrum in Alaskan and Canadian Inuit skeletons.

Spondylolysis of the lower back, particularly that involving the isthmus between the superior and inferior articular processes (pars interarticularis), is generally attributed to stress fracturing caused by movement of the affected vertebra relative to the vertebra below. The finding of isthmic spondylolysis in the first vertebra of a fused sacrum is thus unusual and requires explanation. Although unrepresented in the clinical literature, sacral spondylolysis has been reported for archaeological specimens and appears to be especially prevalent in North American Inuit. A study of 373 Inuit sacr

Direct pediculo-body fixation in cases of spondylolisthesis with advanced intervertebral disc degeneration.

In inveterate cases of grade 2-3 spondylolisthesis (degenerative or spondylolytic), segmental mobility may be reduced by radiologically confirmed disc resorption. Fusion may be indicated in patients with persistent pain. A simple technique for fusion without reduction of the spondylolisthesis is presented. Fixation of the segment is achieved by two cancellous bone screws inserted bilaterally through the pedicles of the lower vertebra into the body of the upper slipped, vertebra. The cases of 16 patients with an average follow-up of 31 months (range 24-27 months) treated with this direct pedicu

MR-myelography: a comparison with conventional myelography.

After creating a MR pulse sequence, called MR-myelography, the purpose of the study was to evaluate the value of this sequence in comparison to conventional myelography. Twenty consecutive patients were examined with conventional and MR-myelography concerning the clinical questions of chronic lumbago, spinal canal stenosis, herniated disc, spondylolisthesis and instability. Six observers assessed both examinations independently for subjective criteria such as pictorial quality, depiction of the dural sac, the nerve roots and their sheaths, artifacts, and the usefulness of single MR-myelography

The cervical spine in athetoid cerebral palsy. A radiological study of 180 patients.

We have reviewed the cervical spine radiographs of 180 patients with athetoid cerebral palsy and compared them with those of 417 control subjects. Disc degeneration occurred earlier and progressed more rapidly in the patients, with advanced disc degeneration in 51%, eight times the frequency in normal subjects. At the C3/4 and C4/5 levels, there was listhetic instability in 17% and 27% of the patients, respectively, again six and eight times more frequently than in the control subjects. Angular instability was seen, particularly at the C3/4, C4/5 and C5/6 levels. We found a significantly highe

[Helical CT for lumbosacral spinal].

PURPOSE: The aim of this study was to investigate the efficacy of helical CT for lumbosacral pathology. MATERIALS AND METHODS: We performed helical CT with multiplanar reconstruction, including the formation of oblique transaxial and coronal images, in 62 patients with various lumboscral disorders, including 32 non-enhanced CT and 36 CT after myelography. We correlated the appearance of the stenotic spinal canal and neoplastic disease with the findings on MRI obtained at nearly the same time. RESULTS: We obtained helical CT images in all cases in about 30 seconds. The diagnostic ability of h

False localizing signs in upper cervical spinal cord compression.

Proprioceptive loss, paresthesias, and atrophy of the hands can occur with disorders afflicting the upper cervical spinal cord. The diagnosis might be erroneous, because compression in this region might produce signs and symptoms that seem to originate in the lower cervical cord. This article reviews the clinical presentation and radiographic data of a consecutive series of 11 patients who presented between 1992 and 1994 with an extradural lesion above the C4 level. Each patient had a characteristic syndrome of finger and hand dysesthesia, hand atrophy, and occipital or cervical pain. These co

Controversy in spine care. Is fusion necessary after anterior cervical discectomy?

Anterior cervical discectomy is an effective and reliable treatment for nerve root or cord compression caused by disc herniation or spondylosis. Although physicians have traditionally included fusion as a part of this procedure, recent experience has suggested that this may not be necessary. Dr. Volker Sonntag and Dr. Peter Klara express opposing views on the need for fusion after discectomy and support their perspectives with clinical experience and a review of the pathoanatomy of disc disease. Dr. Sonntag believes that the majority of patients are well served with discectomy alone, avoiding

Anterior cervical fusion: outcome analysis of patients fused with and without anterior cervical plates.

The purpose of this study was to assess the role of the anterior cervical plate in the treatment of cervical spondylosis. Forty-three patients surgically treated for cervical spondylosis were reviewed. The technique for discectomy and fusion was the same for both groups (Smith-Robinson with autologous iliac crest bone graft). Group I consisted of 25 consecutive patients treated with anterior cervical discectomy, autograft fusion, and anterior cervical plate fixation (Morscher titanium hollow screw plate system). Group II consisted of 18 consecutive patients treated without plate fixation. The

Cervical spinal epidural haematoma: the double jeopardy.

Spontaneous spinal epidural haematoma is an unusual but well recognized cause of compressive myelopathy or cauda equina syndrome. Radicular pain is one of the earliest symptoms and a hallmark of spontaneous spinal epidural haematoma, as in the case of cervical spondylosis and disc prolapse. Should an epidural haematoma be located in the cervical spine, the resultant cervical radicular pain may sometimes be erroneously attributed to a cardiac cause, especially in the setting of pre-existing cardiac disease. The error in diagnosis can lead to another pitfall, the addition of heparin. If the etio

Imaging in degenerative disease of the cervical spine.

Standard radiography of the cervical spine is still the first imaging procedure applied in degenerative disease of the cervical spine. Spondylosis deformans, osteochondrosis and uncovertebral arthrosis are well depicted by such images. Whether or not additional imaging is needed depends on the clinical symptoms. Special projections (anteroposterior oblique radiographs, lateral radiographs in flexion and extension) may be the next step. For many abnormalities, mainly of the intervertebral discs and the myelon, MR imaging has become the standard procedure, largely because of its multiplanar capa

Radiologic interpretation of vacuum phenomena.

"Vacuum" phenomena may occur within peripheral joints or in the spine. These phenomena are related to the accumulation of gas, principally nitrogen, under physiologic or pathologic circumstances. Within the spine, vacuums may occur within the intervertebral discs and are useful indicators of intervertebral osteochondrosis , spondylosis deformans, Schmorl's nodes, and limbus vertebrae. In addition, intraosseous vacuum may occur within the vertebral bodies usually related to osteonecrosis. Under physiologic conditions, naturally occurring peripheral vacuums cannot exist in the presence of joint

Evaluating the child with back pain.

The most common causes of serious low back pain in children include spondylolysis, Scheuermann disease and musculoligamentous injury. Questions should be asked about the mechanism of onset and exacerbating factors, and the frequency, duration and severity of the pain. The examination should check gait and alignment, flexibility, strength and reflexes, and localize and evaluate the pain. Warning signs of serious problems include constant pain in a child younger than 11 years of age that lasts for several weeks or occurs spontaneously at night, repeatedly interferes with school, play or sports,

[Traumatic spinal complications of cervical arthrosis].

OBJECTIVES: Cervical spondylotic myelopathy is usually a chronic and progressive disease. In a few cases, however, dramatic tetraplegia sometimes occurs after even minor injury. METHODS: We report seven patients (6 males, 1 female, aged from 41 to 63), who suffered from acute myelopathy after an injury. RESULTS: In 5 cases, the injury revealed the cervical spondylotic myelopathy. A hyperextension of the cervical spine was found in 5 cases. The injury was severe in only one case, but there was no bony abnormalities, except arthrosis. The most common cause was a fall. In contrast, severe tetra

Cervical and lumbar MRI in asymptomatic older male lifelong athletes: frequency of degenerative findings.

OBJECTIVE: The athletic activity of the adult U.S. population has increased markedly in the last 20 years. To evaluate the possible long-term effects of such activity on the cervical and lumbar spine, we studied a group of asymptomatic currently very active lifelong male athletes over age 40 (41-69 years old, av. age 53). MATERIALS AND METHODS: Nineteen active, lifelong male athletes were studied with MRI and the results compared with previous imaging studies of other populations. An athletic history and a spine history were also taken. RESULTS: Evidence of asymptomatic degenerative spine di

Cervical spondylosis. An update.

Cervical spondylosis is caused by degenerative disc disease and usually produces intermittent neck pain in middle-aged and elderly patients. This pain usually responds to activity modification, neck immobilization, isometric exercises, and medication. Neurologic symptoms occur infrequently, usually in patients with congenital spinal stenosis. For these patients, magnetic resonance imaging is the preferred initial diagnostic study. Because involvement of neurologic structures on imaging studies may be asymptomatic, consultation with a neurologist is advised to rule out other neurologic diseases

Chronic neck pain.

The disorders most commonly seen in rheumatologic practice are acute to chronic cervical disc-related strains, radiculopathies, and degenerative spondylosis. Inflammatory disorders including ankylosing spondylitis and rheumatoid arthritis, although generally rare, are not uncommonly seen in a rheumatologic practice. The pertinent anatomic and pathologic features of cervical disorders are examined in the context of their implications for the hierarchy of specific therapeutic interventions. The basis for the usually favorable prognosis for these conditions, particularly in response to a rational
