This study was undertaken to determine and compare indications and relative benefits of various surgical approaches in 170 patients (average age 55 years) with far-lateral herniated lumbar discs, identified by magnetic resonance (MR) imaging and computerized tomography (CT) and operated on between 1984 and 1994. Essentially three surgical procedures were performed: complete facetectomy in 73 patients, laminotomy with medial facetectomy in 39 patients, and intertransverse discectomy (also known as ITT) in 58 patients. Follow-up periods averaged 5 years (range 0.5-10 years). Outcomes were scored

Between July 1990 and January 1994, 160 patients underwent transpedicular reduction and stabilization using an AO internal fixator. In this series, most of the patients (101 cases) had degenerative spondylolisthesis. The follow-up period ranged from 12 to 36 months. One hundred and thirty one patients (82%) had a good or excellent postoperative outcome. Sixteen (10%) had fair outcomes and 13 (8%) had poor outcomes. There were 26 complications (16%) secondary to this treatment. Improper screw placement was found in five patients, wound infection in four patients and dural tear with cerebrospina

Intraspinal synovial cysts are an uncommon but well recognised cause of backache and sciatica, and should be considered in patients, in particular with degenerative spondylolisthesis, who are symptomatic. MRI is the initial investigation of choice. If there is any doubt as to the diagnosis, CT with or without facet joint arthrography is helpful.

This study presents the estimates of heritability for spondylosis deformans in the boxer based on 353 offspring from 24 randomly selected sires, each with at least three radiographically investigated offspring. The estimated heritability (h2) for maximum degree of osteophyte development was high, both when estimated by paternal half-sib correlation (0.42) and by the regression of offspring based on the parents (0.62). The heritability for the number of affected discs estimated by paternal half-sib correlation was also high (0.47). The estimate of heritability for the number of affected discs b

OBJECTIVE: In a search for ancillary MR findings for the diagnosis of spondylolysis, we performed a retrospective study to characterize changes in MR signal intensity of marrow within lumbar pedicles at the level of a spondylolytic defect. These reactive marrow changes were classified according to the anatomic-pathologic scheme developed for degenerative disk disease by Modic et al.
MATERIALS AND METHODS: Two neuroradiologists retrospectively reviewed MR images of 60 patients with lumbar spondylolysis confirmed by conventional radiography or CT. The MR signal of each pedicle at the level of a

Acute spondylolytic spondylolisthesis was diagnosed after major trauma in five patients. The level of injury was between the third and fourth lumbar vertebrae in one patient and between the fifth lumbar and first sacral vertebrae in four. The initial spondylolisthesis was grade I in four patients and grade III in one. Four of the patients were initially managed non-operatively. The deformity did not progress in a five-year-old boy with grade-I spondylolisthesis who had been managed with immobilization in a body cast. The deformity progressed in two of the adolescents who had been managed non-o

Forty-six patients with the Diapason transpedicular screw system for lumbar and lumbosacral fusions were analysed clinically. No implant breakage was observed, but one rod migrated. Additional sacral screws different in the length of the threaded portion are necessary in this system. Metallosis was noted in a few cases, but posed no significant clinical problems. This system, which is simple in structure, easy to use, and applicable to magnetic resonance scanning after surgery, is considered to be a useful instrumentation.

The patient is a 63-year-old Chinese man who presented with tetraparesis and urinary incontinence. The initial diagnosis was cord compression from cervical spondylosis. The patient relapsed 3 months after cervical laminectomy. The transverse myelitis picture, left optic atrophy and suggestive brainstem evoked potentials led to treatment of a presumptive demyelinating process. The presence of vitiligo, however, led to detection of high titers of antinuclear antibodies (ANA) and presence of anti-nonhistone antibodies. The patient was then diagnosed to have a lupus (SLE)-like disease, which has n

OBJECTIVE: To discuss an interesting case depicting multiple levels of cervical spondylolysis. Differential diagnosis and a brief review of the literature are discussed.
CLINICAL FEATURES: A 24-yr-old man suffered from constant neck discomfort of approximately 4 months' duration. After physical examination, X-rays were performed and revealed multiple congenital anomalies. The patient's symptoms are considered independent of the radiographic findings.
INTERVENTION AND OUTCOME: This patient was successfully treated with conservative chiropractic management and experienced complete resolution o

We investigated 185 adolescents under the age of 19 years with spondylolysis. All but five were active in sport. The pars defect was classified into early, progressive and terminal stages. Of the 346 pars defects in 185 patients, 39.6% were early, 29.5% progressive and 30.9% in the terminal stages. Conservative management produced healing in 73.0% of the early, 38.5% of the progressive and none of the terminal defects. These results suggest that spondylolysis is caused by repetitive microtrauma during growth and can be successfully treated conservatively if treatment is started in the early st

STUDY DESIGN: This study investigated the clinical usefulness of motor evoked potentials and a silent period after motor evoked potentials produced by transcranial magnetic stimulation of the brain.
OBJECTIVE: The results were correlated with the clinical state of the patients with myelopathy, whereas no abnormality of the conduction time was observed in the patients with spinal canal stenosis.
SUMMARY OF BACKGROUND DATA: Magnetic stimulation has been widely used for examination of the descending excitatory motor pathways in the central nervous system, but little attention has been paid to c

PURPOSE: To document the occurrence of isolated dorsal subluxation of posterior elements in cases of lumbar spondylolysis without spondylolisthesis both quantitatively (using spinal canal measurements) and qualitatively (by visual inspection) on sagittal MR images.
METHODS: Retrospective analysis identified 63 patients with lumbar spondylolysis (confirmed by CT or conventional radiography) who had undergone MR imaging. From these we identified 12 patients with pars interarticularis defects but no evidence of spondylolisthesis. Measurements of anteroposterior spinal canal diameters were perfor

STUDY DESIGN: Tissue from the pars defects of six adult patients with symptomatic spondylolysis and spondylolisthesis was obtained at surgery. A histologic study was conducted to identify and characterize neural elements in this tissue.
OBJECTIVES: To determine if nociceptive nerve endings were present within the pars defect of patients with symptomatic spondylolysis.
SUMMARY OF BACKGROUND DATA: The origin of back pain in patients with spondylolysis remains uncertain. The defect in the pars interarticularis has been implicated as a possible pain source.
METHODS: The soft tissue from the par
