Tuberculous spinal epidural abscess: a cautionary case report of permanent paraplegia due to delayed diagnosis and management.

BACKGROUND: Spinal epidural abscess caused by tuberculosis is a rare but serious complication of spinal tuberculosis. Its atypical presentation predisposes to diagnostic delay and a poor prognosis. This case not only documents the clinical and radiologic evolution of tuberculous spinal epidural abscess but also suggests that delays in diagnosis and treatment may jointly contribute to serious neurological outcomes. CASE PRESENTATION: A 39-year-old man initially underwent acupotomy for recurrent low back pain with recent exacerbation. Subsequent imaging revealed a paraspinal abscess within the

A Case of Acute Spontaneous Spinal Epidural Hematoma Caused by Repeated Bleeding Thoracic Radiculomeningeal Artery.

Spontaneous spinal epidural hematoma (SSEH) presents with back pain followed by myelopathy. Venous plexus disruption is common, and arterial bleeding is extremely rare. Here, we report a case of acute spontaneous spinal epidural hematoma that showed spot sign on computed tomographic angiography and was treated. A 48-year-old woman with no medical history experienced unprovoked back pain and visited a local hospital, where she was placed under observation. Numbness gradually developed in both legs, and the patient visited our emergency room 5 hours after the onset of symptoms. Magnetic resonanc

Current progress and controversies in thoracic ossification of the ligamentum flavum: pathogenesis, diagnosis and surgical management.

Thoracic ossification of ligamentum flavum (TOLF) is the primary pathological driver of thoracic spinal stenosis (TSS), characterized by insidious clinical onset, high surgical complexity, and a elevated risk of permanent neurological deficits. This systematic review synthesizes the epidemiological landscape and genetic, mechanical, and inflammatory pathogeneses ofTOLF. We provide a comprehensive evaluation of current radiological classification systems and critically appraise different decompressive procedures-ranging from classic open laminectomies to emerging minimally invasive biportal and

Targeting the gut microbiota to promote motor rehabilitation after spinal cord injury: mechanisms, evidence, and translational challenges.

Spinal cord injury (SCI) is a devastating central nervous system trauma that frequently results in permanent motor dysfunction. Over the past decade the gut microbiota has emerged as a potentially modifiable contributor to secondary injury and neural repair, giving rise to the concept of the gut-spinal cord axis. In this narrative review we synthesize current understanding of how SCI-associated gut dysbiosis may relate to the recovery of motor function, focusing on immune modulation, microbial metabolites such as short-chain fatty acids and bile acids, and gut-central nervous system signaling.

Clinical Application and Safety of Intrathecal Adjuvants in Single-Shot Thoracic Segmental Spinal Anesthesia: A Narrative Review.

Thoracic segmental spinal anesthesia (TSSA) is an emerging regional anesthesia (RA) technique. TSSA with local anesthetic (LA) alone is associated with a relatively short duration of surgical anesthesia and postoperative analgesia, which may limit its clinical utility. Intrathecal adjuvants have been widely used in lumbar spinal anesthesia (LSA) to prolong block duration and improve block quality. However, their role in TSSA remains poorly defined. This review summarizes commonly used intrathecal adjuvants during TSSA, their doses, adverse effects, and safety considerations. A comprehensive li

Assessing the Categories and Factors Influencing Treatment Decision Conflict in Patients With Lumbar Disc Herniation: A Latent Profile Analysis.

BACKGROUND: Lumbar disc herniation (LDH) represents a prevalent degenerative spinal disorder, characterized by severe pain and functional limitations that significantly diminish patients' quality of life. Despite the availability of conservative and surgical treatment options, patients frequently encounter considerable decisional conflict (DC). Current research often neglects the inherent heterogeneity within this patient population. To ensure that the research questions and outcome measures reflect patients' real-world priorities, we involved patients with LDH in the study design phase. This

Symptom duration and postoperative outcomes in degenerative cervical myelopathy: a systematic review and meta-analysis.

BACKGROUND: Surgical decompression is regarded as standard treatment for degenerative cervical myelopathy (DCM). However, the timing of surgery remains a critical yet unresolved issue in clinical practice. Therefore, this meta-analysis aimed to evaluate the impact of symptom duration on surgical outcomes and determine the optimal timing for surgery. METHODS: A systematic search was conducted on 16th November 2025 using PubMed, Ovid, Scopus, and Web of Science databases to identify eligible articles. Studies comprising patients with DCM undergoing surgery, assessing outcome by mJOA score at a

Pyogenic Sacroiliitis in Older Adults Presenting With Massive Retroperitoneal Abscess: A Case Report.

Pyogenic sacroiliitis (PSI) is an uncommon form of septic arthritis and is frequently diagnosed late in older adults because initial symptoms are nonspecific and imaging findings may be misattributed to other sources of infection. We report two older patients with PSI complicated by massive retroperitoneal and gluteal abscesses, highlighting the importance of early recognition and, when indicated, timely surgical source control. Case 1 was an 83-year-old woman in whom a conspicuous retroperitoneal abscess and abnormal urinalysis led to suspicion of an intra-abdominal or urinary tract source, d

Association between ankylosing spondylitis and hearing loss: A Mendelian randomization study.

Observational studies have suggested a potential link between ankylosing spondylitis (AS) and hearing loss. However, this association remains controversial due to potential drug-induced ototoxicity from treatments. Therefore, we employed a Mendelian randomization (MR) approach to evaluate the association between AS and hearing loss from a genetic perspective. This MR study utilized summary data from genome-wide association studies on AS and hearing loss, including sensorineural hearing loss (SNHL) and conductive hearing loss (CHL). The primary analysis was conducted using the inverse-variance

Diverging diagnostic trajectories of degenerative cervical myelopathy and cervical radiculopathy in South Korea: a nationwide time-series analysis of claims data, 2010-2024.

BACKGROUND: Degenerative cervical myelopathy (DCM) is the leading non-traumatic cause of spinal cord dysfunction in adults, and its burden is projected to grow as populations age. Cervical radiculopathy shares the same degenerative substrate but follows a separate diagnostic pathway. Long-term national trends in the two conditions, and their relation to surgical practice, are unquantified in an East Asian system with universal coverage. METHODS: This nationwide repeated cross-sectional ecological analysis used aggregate open data from the Korean Health Insurance Review and Assessment Service,

Age-related spinal degenerative diseases: a quantitative analysis of the dynamic evolution, knowledge structure, and research frontiers in neurological damage.

BACKGROUND AND PURPOSE: Aging is the primary risk factor for the four major spinal degenerative diseases of lumbar disc herniation (LDH), cervical spondylosis (CS), lumbar spinal stenosis (LSS) and thoracic disc herniation (TDH). These diseases can lead to severe neurological dysfunction, chronic neuropathic pain and a decline in the quality of life in elderly patients. This article conducts the first simultaneous, comparative bibliometric analysis of these broader age-related spinal degenerative diseases within a unified framework, identifying the mitigation of neurological damage and neuropa

Effects of microtrauma on joint structural changes in the Curdlan-injected SKG mouse model of ankylosing spondylitis.

Although microtrauma is considered a potential environmental factor in the development of ankylosing spondylitis (AS), there is a lack of experimental research regarding its precise contribution to disease pathogenesis. To induce physiological microtrauma, the right ankles of SKG mice were immobilized for a week using surgical plaster and a pipette tip to stimulate compensatory mechanical overload in the contralateral limb. Following the removal of the immobilization, the mice were intraperitoneally injected with 3 mg of Curdlan. In this model, the non-immobilized left ankle served as the micr

Third lumbar transverse process syndrome with acute abdominal pain as the main clinical manifestation: A case report.

RATIONALE: Third lumbar transverse process syndrome is an uncommon musculoskeletal cause of referred abdominal pain. Because acute abdominal pain is appropriately evaluated first for visceral, vascular, urinary, and surgical emergencies, a spinal or paraspinal pain generator may be overlooked. PATIENT CONCERNS: A 66-year-old man presented with persistent left lower-quadrant abdominal pain radiating to the left lower back. Baseline pain intensity was 7/10 on a VAS, and the pain was aggravated by postural change, standing, and walking. DIAGNOSES: Serial laboratory tests, endoscopy, ultrasonogr

Intraosseous Lidocaine Trigger Point Injection for Vertebrogenic Pain: From Molecular Principles to Clinical Applications: A Narrative Review.

Intraosseous lidocaine trigger point injection (LTPI) has been utilized for an extended period in the management of chronic vertebrogenic pain; yet, its molecular mechanisms and clinical efficacy require critical appraisal. This narrative review highlights the milestones of osteoreception and elucidates the molecular mechanisms driving intraosseous receptor irritation and its pivotal role in chronic pain development. Pathological vertebral bone remodeling triggers a receptor activator of nuclear factor-kappa B ligand/osteoprotegerin (RANKL/OPG) imbalance, accelerating hyperactive osteoclastoge

Thoracolumbar Fascia Remodeling in Lumbar Spinal Stenosis: Translational Orthopedic Perspectives.

Lumbar spinal stenosis (LSS) is a major cause of pain and disability, yet its pathophysiology remains incompletely understood. Although spinal decompression is the standard surgical treatment for LSS with disabling pain or with progressive neurological symptoms, clinical outcomes are often suboptimal, with reoperation required in 5-23% of cases. While degenerative changes in the vertebral canal have been extensively investigated, the potential contribution of the thoracolumbar fascia (TLF) has never been explored at the molecular level. We hypothesized that TLF undergoes extracellular matrix r

Stereotactic body radiation therapy in elderly patients with spinal metastases.

PURPOSE: Elderly patients with spinal metastases are often considered poor candidates for aggressive local therapy, due in part to medical comorbidity and limited evidence to guide treatment selection. This study evaluated outcomes following spine stereotactic body radiation therapy (SBRT) in this patient population and examined predictors of local failure (LF) and vertebral compression fracture (VCF). METHODS: A prospectively maintained database of consecutive patients aged ≥ 70 years with available imaging who underwent spine SBRT at a quaternary referral center from 2002 to 2024 was analyz

Endoscopic Decompression for Lateral Recess Stenosis Due to Vertebral Compression Fracture: A Case Report.

STUDY DESIGN: Case presentation. OBJECTIVE: The interlaminar endoscopic lateral recess decompression (IE-LRD) represents a novel surgical alternative to consider in carefully selected cases, particularly when patients do not respond to conservative treatment. BACKGROUND: Lumbar spinal stenosis (LSS) constitutes one of the most prevalent spinal conditions in the elderly population around the world. A myriad of causes have been elucidated, and although infrequent, vertebral compression fractures (VCF) may also cause LSS. Conservative treatment might be the first option; however, patients often
