This report describes a rare and challenging case of severe lumbar spinal canal stenosis resulting from extensive, concurrent lumbar ossification of the posterior longitudinal ligament (OPLL) and ossification of the ligamentum flavum (OLF). A 44-year-old female patient who initially presented with numbness and gait instability due to thoracic spinal cord compression was successfully managed by thoracic decompression and fusion. However, she subsequently developed lower back pain, buttock pain, and numbness along both legs due to severe multilevel lumbar stenosis from L2 to L5. Surgical managem

This is the first review of mechanical testing or computational modeling articles that analyzed the biomechanical influence of parametrically changing scapula fracture implant (SFI) variables to optimize or characterize performance. PubMed and Embase were explored for articles using inclusion and exclusion criteria: (i) SFI articles that permutated implant variables were included, but articles using only one SFI configuration were excluded; (ii) SFIs comprising of plates (with screws), non-plate isolated "lag" screws, or cerclage wires were included, but anchors/sutures, bone cements, bone gra

BACKGROUND: Persistent somatic symptoms are common in patients with post COVID-19 condition (PCC). Yet, somatic symptom disorder (SSD) which captures maladaptive symptom-related thoughts, feelings, and behaviors has not been investigated in individuals with PCC using clinical interviews. This study examined SSD frequency and associated biopsychosocial factors in a randomized controlled trial (RCT) sample of patients with PCC.
METHODS: Baseline data from adults with PCC enrolled in a RCT were analyzed. SSD was assessed using a structured DSM-5-based clinical interview. Self-report measures ass

Degenerative cervical myelopathy (DCM), the most common non-traumatic spinal cord injury, is a prevalent, progressive, and highly disabling neurological disease, causing heterogeneous impairment ranging from sensory loss and impaired hand dexterity to gait, balance, and autonomic dysfunction. Disability often persists despite surgical decompression, and untreated disease can progress to irreversible neurological injury, including tetraplegia. Current clinical management is largely reactive, with diagnosis and intervention typically only after neurological deterioration has become clinically ap

BACKGROUND AND PURPOSE: Cholinergic premotor inputs to motoneurons support high levels of motor output when required. However, the possible role of these inputs in pathologically elevated muscle tone has not been characterised. To address this, we examined the modulatory effect of spinal acetylcholine (ACh) on experimental spastic paralysis.
EXPERIMENTAL APPROACH: Rats were injected with tetanus neurotoxin (TeNT, 33-100 pg, im) into gastrocnemius to induce sustained rigid hind-paw extension and resistance to passive ankle flexion. The effects of spinal ACh on experimental hypertonia were prob

Foraging strategy frequently drives morphological diversification in birds. The plunge-diving behavior of certain species makes the functional and ecological trade-offs of variation in cervical vertebrae an especially intriguing question. This study compares cervical vertebrae across five plunge-diving species and two non-plunge-diving species, using morphometric analysis of micro-CT scans to test whether diving birds share specialized cervical vertebrae morphologies distinct from non-divers. Plunge-divers repeatedly experience extreme impact forces during headfirst water entry, requiring the

INTRODUCTION: Work-related musculoskeletal disorders (WMSDs) are the most commonly reported health problem among workers in Europe. Manual handling of loads and awkward postures are cited as important risk factors for WMSDs. Exoskeletons, wearable devices that support and stabilize the locomotor system, are emerging as a lifting aid that could reduce muscular effort and potentially lessen the impact and the prevalence of WMSDs among manual workers. This systematic review and meta-analysis aims to summarize the current scientific evidence on passive exoskeletons designed to support the lower ba

OBJECTIVE: Axial spondyloarthritis (axSpA) is a chronic inflammatory autoimmune disease associated with persistent systemic inflammation and endothelial dysfunction. Angiogenic T cells (Tang cells), defined as CD3+CD31+CD184+ T cells, are specialized immune cells involved in vascular repair and endothelial homeostasis, but their clinical relevance in axSpA remains unclear. This study aimed to characterize circulating Tang cells in axSpA and evaluate their potential as immune-vascular biomarkers of disease activity.
METHODS: Peripheral blood samples were obtained from patients with axSpA strat

We report a 77-year-old woman with papillary renal cell carcinoma showing no increased [ 68 Ga]Ga-FAPI uptake in the primary tumor, while intense sacral FAPI uptake mimicked bone metastasis. MRI findings and 2-year follow-up supported a sacral insufficiency fracture without metastatic disease. This case highlights fracture-related FAPI uptake as an important oncologic imaging pitfall.

Dysferlinopathy (Limb Girdle Muscular Dystrophy R2/LGMD2B) causes progressive weakness of limb-girdle muscles. Although gait abnormalities are common, the contribution of spinal muscles to disease symptoms remains poorly understood. Using cross-sectional MRI in patients and longitudinal analyses in a dysferlinopathic mouse model, we identify early-onset, progressive adipogenic degeneration of spinal muscles as a previously unrecognized hallmark of dysferlinopathy. Multiple spinal muscles, including the iliocostalis, longissimus, and spinalis, are affected, with degeneration preceding pathology

Transthyretin (TTR) amyloidosis, particularly the wild-type form (ATTRwt), is an age-related systemic disease characterized by the deposition of misfolded TTR in tendons and ligaments. Patients therefore frequently present initially to orthopaedic or hand surgeons with carpal tunnel syndrome (CTS) or lumbar spinal stenosis (LSS) rather than to cardiologists or neurologists. These musculoskeletal manifestations may precede ATTR cardiomyopathy (ATTR-CM) by several years to more than a decade, serving as "red flags" that provide an opportunity for early diagnosis, particularly now that disease-mo

Accurate lesion localization is a fundamental component of neurological diagnosis. This review describes how neurological examination can be used to differentiate cervical spinal cord lesions, cervical radiculopathy, brachial plexopathy, and peripheral nerve disorders in patients presenting with upper-limb sensory and motor symptoms. Particular attention is given to the distribution of sensory impairment, tendon reflex abnormalities, and patterns of muscle weakness. The characteristic clinical features of cervical spondylotic myelopathy, cervical radiculopathy, brachial plexus disorders, and e

In patients presenting with numbness, sensory disturbances, or motor deficits in the upper extremities, it is important to differentiate between cervical spinal cord or nerve root disorders and peripheral neuropathies. This article discusses the key points for distinguishing C8 spinal cord segment and nerve root disorders from peripheral neuropathies, particularly ulnar neuropathy and posterior interosseous nerve palsy. A comprehensive evaluation of the patterns of muscle weakness, distribution of sensory disturbances, and electrophysiological findings is essential, with careful consideration

Lower-limb numbness, pain, and weakness are common clinical complaints that require differentiation between cauda equina or radicular lesions associated with lumbar spinal disease and peripheral nerve disorders. Because degenerative findings on lumbar imaging are common, particularly among older adults, the responsible lesion should not be determined based on imaging findings alone. In this context, neurological examination contributes to anatomical localization and is central to the differential diagnosis. Particular emphasis is placed on interpreting muscle weakness and sensory disturbance a
