Surgical indications for lumbar spine disorders should not be determined solely based on degenerative or stenotic magnetic resonance imaging findings. This article discusses how orthopedic spine surgeons evaluate lumbar pathology in collaboration with neurologists. The central issue is whether the patient's symptoms, neurological findings, and imaging abnormalities can be used to identify the responsible lesion that is likely to improve after surgery. Differential diagnoses, including cervical or thoracic myelopathy, peripheral neuropathy, motor neuron disease, vascular claudication, hip disea

Lhermitte's sign (LS) is characterised by a brief electric shock-like sensation elicited by neck flexion that radiates down the spine and may extend into the posterior aspects of the legs. It is a clinical indicator of cervical or upper thoracic dorsal column dysfunction, most commonly due to demyelination or intrinsic spinal cord pathology. It may occur after head and neck (chemo)radiotherapy. We describe two patients with human papillomavirus-associated oropharyngeal squamous cell carcinoma who developed LS following chemoradiotherapy. One patient developed protracted LS lasting 20 months be

Non-traumatic spinal cord injury (NTSCI) may result from degenerative spinal conditions, including spinal stenosis and multilevel disc herniation, and can cause substantial motor, sensory, and functional impairment. We present a 33-year-old man with NTSCI secondary to severe thoracic spinal stenosis and multilevel disc herniation. Before the initial surgical intervention, the available neurological examination documented 5/5 upper-extremity strength and 4/5 strength in the bilateral lower extremities. Sensation was impaired below a thoracic level, and hyperreflexia and other findings consisten

BACKGROUND Kidney transplantation offers clear survival and quality-of-life advantages over dialysis, but severe pulmonary disease can complicate perioperative planning. In patients with substantially reduced respiratory reserve after pneumonectomy, general anesthesia may be considered high risk, creating uncertainty about transplant feasibility. CASE REPORT A 33-year-old man with end-stage kidney disease secondary to biopsy-proven AA amyloidosis was evaluated for preemptive living-donor kidney transplantation. He had undergone left pneumonectomy in 2015 because of bronchiectasis. Pulmonary fu

Multilevel degenerative spinal stenosis (DSS) is surgically challenging due to longer operative times, greater blood loss, and risk of instability. The Slalom technique, a motion-preserving, minimally invasive approach, has shown promise; however, no randomized controlled trials currently exist to validate its efficacy against standard techniques. This meta-analysis evaluated its operative outcomes in multilevel DSS. Following PRISMA guidelines and PROSPERO registration (CRD420251065160), a comprehensive literature search was performed across PubMed, Embase, and the Cochrane Library through Ja

Diagnostic lumbar puncture is a core procedure in the diagnosis and management of infectious diseases. However, in patients with complex anatomical conditions, such as obesity and spinal deformities, the traditional blind puncture method is often challenging to perform, resulting in a substantially higher failure rates and increased complication risks. Bedside ultrasound guidance provides a precise, visual solution to these challenges and represents an emerging tool for improving clinical practice. This narrative review aimed to comprehensively examine the clinical value of ultrasound-guided l

BACKGROUND: Telomere length (TL) is a marker of cumulative biological aging, but whether it causally contributes to degenerative spine disease or reflects upstream metabolic and inflammatory burden remains unclear.
METHODS: We performed a population-based study integrating two-sample Mendelian randomization (MR) with longitudinal UK Biobank analyses. MR used genetic instruments for leukocyte TL, body mass index (BMI), and C-reactive protein (CRP) against five degenerative spine GWAS outcomes. We estimated direct TL effects after adjustment for BMI and CRP. Observational analyses included 265,

The spinal cord is a critical central nervous system structure facilitating neural transmission and reflex activity. Spinal cord disorders manifest as myelopathy with diverse etiologies, necessitating imaging for accurate diagnosis. MR imaging is the gold standard modality due to superior soft tissue contrast and multiplanar capability, enabling detailed assessment of intrinsic and extrinsic pathologies. Conventional sequences, including T1-weighted, T2-weighted, short tau inversion recovery, and postcontrast imaging, remain foundational for detecting structural abnormalities. Emerging advance

This article reviews common MR imaging artifacts encountered in spine imaging and emphasizes their clinical relevance, as artifacts may degrade image quality or mimic spinal pathology, postoperative complications, or cord lesions. It outlines practical clues for distinguishing artifactual findings, including extension beyond anatomic boundaries, appearance limited to specific planes or sequences, and absence of expected associated disease. Artifacts are organized by mechanism: motion-related artifacts such as bulk motion and cerebrospinal fluid pulsation with flow-related ghosting; field inhom

MR imaging of the human spinal cord presents unique challenges due to its small cross-sectional dimensions, complex microstructure, surrounding cerebrospinal fluid and other tissues, and susceptibility to motion and field inhomogeneities. Advanced imaging modalities have emerged to address these limitations by providing objective biomarkers of tissue integrity, metabolic status, and functional connectivity. This article synthesizes current evidence, outlines technical considerations and clinical applications of advanced spinal cord imaging, and provides a translational roadmap for integrating

Myelographic imaging evaluates the spinal canal, nerve roots, and subarachnoid space using intrathecal contrast with fluoroscopy and computed tomography (CT), or noninvasively with MR-myelography. Although MR imaging has largely replaced conventional myelography owing to its superior soft tissue contrast and noninvasive nature, CT myelography remains essential when MR imaging is contraindicated, limited by artifact, or inconclusive. It is particularly valuable for assessing nerve root compression, spinal stenosis, cerebrospinal fluid leaks, postoperative anatomy, and complex lesions. Myelograp

Myelopathy refers to a pathologic process involving the spinal cord that encompasses a heterogeneous spectrum of clinical presentations with distinct etiologies, prognoses, and management strategies. As symptoms are often nonspecific and clinical localization can be challenging, imaging-particularly MR imaging-plays a pivotal role in diagnosis. A structured, pattern-based imaging approach is essential for accurate interpretation, integrating symptom acuity and physical examination findings with lesion localization, extent, signal characteristics, enhancement patterns, temporal evolution, and a

In this article, we focus on imaging of degenerative cervical myelopathy, the most common form of spinal cord dysfunction in adults arising from its progressive compression by age-related degenerative changes in the spine. Imaging plays a key role in confirming the underlying cause of symptoms and guiding neurosurgical intervention. Emerging imaging methods promise earlier diagnosis, better prognostication, and more accurate identification of candidates for surgical intervention. Differences in imaging findings for the less prevalent degenerative thoracic myelopathy are also discussed.

Primary infectious myelitis is a rare but critical diagnosis with potential for severe morbidity and permanent neurologic deficit if not detected and treated early. This article introduces an image-guided framework that organizes spinal cord infections into 5 distinct transverse magnetic resonance imaging patterns: extramedullary, centromedullary, eccentric tract-specific, frontal horn, and irregular or mass-like. This article recognizes the substantial variability in imaging manifestations of various infections in the spinal cord, and the proposed pattern-based approach is not intended to ser

Ankylosing Spondylitis(AS) is a chronic inflammatory disease primarily affecting the spine and sacroiliac joints, with a higher prevalence in young males. Without timely diagnosis and treatment, it can progress to spinal ankylosis, joint function loss, and disability. The exact pathogenesis remains incompletely understood, leaving some patients unresponsive to current treatments and facing poor outcomes. In recent years, the role of NOD-like receptor family pyrin domain containing 3 (NLRP3) inflammasome in the pathogenesis of AS has attracted much attention. Upon activation, the NLRP3 inflamma
