[Translated article] Clinical and functional outcomes of unilateral biportal endoscopic decompression in patients with lumbar compression.

INTRODUCTION: Lumbar neural compression is one of the leading causes of pain and disability in older adults. Unilateral Biportal Endoscopic (UBE) decompression has emerged as a minimally invasive alternative to conventional techniques, providing a wider surgical field, enhanced visualization, and preservation of anatomical structures. METHODS: We conducted a retrospective observational study including patients with lumbar spinal stenosis who underwent decompression using the UBE technique between January 2022 and January 2024. Demographic data, surgical details, and clinical and functional ou

False-Negative Ligamentum Teres Tendon Sampling for the Detection of Systemic Amyloidosis.

This case report involves a 60-year-old man with bilateral carpal tunnel syndrome, hip osteoarthritis, and lumbar spinal stenosis who underwent biopsies of the ligamentum teres, ligamentum flavum, and carpal flexor tenosynovium. Amyloid deposits were identified in the ligamentum flavum and flexor tenosynovium, but not the ligamentum teres. Amyloid was typed by mass spectrometry, cardiac scintigraphy was negative, and genetic testing remains pending. This supports that ligamentum flavum and carpal flexor tenosynovium biopsies may have better diagnostic value for the early diagnosis of amyloidos

Guillain-Barré Syndrome in a Patient With Cervical Myelopathy and Lumbar Spinal Stenosis Following a Fall: A Diagnostic Challenge.

Guillain-Barré syndrome (GBS) is an acute autoimmune disease characterized by symmetrical weakness that develops over a few days to four weeks. Its diagnosis can be challenging when symptoms overlap with other conditions, such as lumbar spinal stenosis. We report the case of a 55-year-old man who presented with sudden lower-extremity weakness and bladder incontinence following a fall after alcohol intake. Magnetic resonance imaging revealed cervical and lumbar spinal stenosis with compressive changes, which were initially considered the likely cause of his symptoms. However, the patient subseq

Clinical and radiographic factors associated with residual low back pain after single-level decompression for lumbar spinal stenosis: a multicenter retrospective cohort study.

BACKGROUND CONTEXT: Residual low back pain (LBP) after decompression for lumbar spinal stenosis (LSS) varies considerably and is likely multifactorial. It remains unclear how residual LBP differs by preoperative severity and which factors are independently associated with it. PURPOSE: To examine how postoperative LBP improves across preoperative severity groups and to identify clinical and radiographic factors independently associated with residual LBP. STUDY DESIGN/SETTING: Multicenter retrospective cohort study. PATIENT SAMPLE: A total of 450 patients undergoing primary single-level poste

Association of Spinal Epidural Lipomatosis With Systemic Lipid Metabolism in Patients With Lumbar Spinal Stenosis: A Propensity Score-Matched Study.

BACKGROUND CONTEXT: Spinal epidural lipomatosis (SEL) is often regarded as a local consequence of obesity, yet its relationship with systemic lipid metabolism and symptom severity in lumbar spinal stenosis remains unclear. Apolipoprotein B (ApoB) is central to atherogenic lipid transport and ectopic fat deposition. Examining the association between systemic lipid metabolic abnormalities, including elevated ApoB levels, and epidural fat accumulation may provide insights into the metabolic pathogenesis and clinical relevance of SEL. PURPOSE: To determine whether SEL is associated with systemic

Current progress and controversies in thoracic ossification of the ligamentum flavum: pathogenesis, diagnosis and surgical management.

Thoracic ossification of ligamentum flavum (TOLF) is the primary pathological driver of thoracic spinal stenosis (TSS), characterized by insidious clinical onset, high surgical complexity, and a elevated risk of permanent neurological deficits. This systematic review synthesizes the epidemiological landscape and genetic, mechanical, and inflammatory pathogeneses ofTOLF. We provide a comprehensive evaluation of current radiological classification systems and critically appraise different decompressive procedures-ranging from classic open laminectomies to emerging minimally invasive biportal and

Age-related spinal degenerative diseases: a quantitative analysis of the dynamic evolution, knowledge structure, and research frontiers in neurological damage.

BACKGROUND AND PURPOSE: Aging is the primary risk factor for the four major spinal degenerative diseases of lumbar disc herniation (LDH), cervical spondylosis (CS), lumbar spinal stenosis (LSS) and thoracic disc herniation (TDH). These diseases can lead to severe neurological dysfunction, chronic neuropathic pain and a decline in the quality of life in elderly patients. This article conducts the first simultaneous, comparative bibliometric analysis of these broader age-related spinal degenerative diseases within a unified framework, identifying the mitigation of neurological damage and neuropa

Intraosseous Lidocaine Trigger Point Injection for Vertebrogenic Pain: From Molecular Principles to Clinical Applications: A Narrative Review.

Intraosseous lidocaine trigger point injection (LTPI) has been utilized for an extended period in the management of chronic vertebrogenic pain; yet, its molecular mechanisms and clinical efficacy require critical appraisal. This narrative review highlights the milestones of osteoreception and elucidates the molecular mechanisms driving intraosseous receptor irritation and its pivotal role in chronic pain development. Pathological vertebral bone remodeling triggers a receptor activator of nuclear factor-kappa B ligand/osteoprotegerin (RANKL/OPG) imbalance, accelerating hyperactive osteoclastoge

Thoracolumbar Fascia Remodeling in Lumbar Spinal Stenosis: Translational Orthopedic Perspectives.

Lumbar spinal stenosis (LSS) is a major cause of pain and disability, yet its pathophysiology remains incompletely understood. Although spinal decompression is the standard surgical treatment for LSS with disabling pain or with progressive neurological symptoms, clinical outcomes are often suboptimal, with reoperation required in 5-23% of cases. While degenerative changes in the vertebral canal have been extensively investigated, the potential contribution of the thoracolumbar fascia (TLF) has never been explored at the molecular level. We hypothesized that TLF undergoes extracellular matrix r

[Single-portal endoscopic decompression for degenerative lumbosacral spinal stenosis: a prospective single-center study].

BACKGROUND: Degenerative lumbosacral spinal stenosis is one of the leading causes of chronic pain and disability in elderly patients. Single-portal endoscopic interlaminar bilateral decompression is a promising minimally invasive method. However, prospective data are limited. MATERIAL AND METHODS: A single-center prospective study included 50 patients with central single-level lumbosacral degenerative stenosis who underwent single-portal endoscopic interlaminar bilateral decompression through a unilateral approach. VAS, ODI, MacNab and GAS scores were assessed. We also analyzed surgery time a

Endoscopic Decompression for Lateral Recess Stenosis Due to Vertebral Compression Fracture: A Case Report.

STUDY DESIGN: Case presentation. OBJECTIVE: The interlaminar endoscopic lateral recess decompression (IE-LRD) represents a novel surgical alternative to consider in carefully selected cases, particularly when patients do not respond to conservative treatment. BACKGROUND: Lumbar spinal stenosis (LSS) constitutes one of the most prevalent spinal conditions in the elderly population around the world. A myriad of causes have been elucidated, and although infrequent, vertebral compression fractures (VCF) may also cause LSS. Conservative treatment might be the first option; however, patients often

Diffuse Idiopathic Skeletal Hyperostosis (DISH): Potential Links Between Gut Microbiota and Spinal Degenerative Diseases.

Diffuse Idiopathic Skeletal Hyperostosis (DISH) is a systemic, non-inflammatory skeletal disorder characterized by progressive calcification and ossification of spinal ligaments and entheses. Its strong associations with aging, obesity, type 2 diabetes, and metabolic syndrome raise the possibility that metabolic and inflammatory pathways may contribute to disease development. The gut microbiota participates in nutrient metabolism, short-chain fatty acid production, bile-acid transformation, intestinal-barrier maintenance, and immune regulation; these functions provide a plausible framework for

Retrospective Analysis of Unilateral Biportal Endoscopy Versus Open Laminotomy for Lumbar Spinal Stenosis.

Lumbar spinal stenosis is a common degenerative condition that causes pain, disability, and reduces quality of life, especially in older adults. Techniques such as Unilateral Biportal Endoscopy (UBE) and Open Unilateral Laminotomy with Bilateral Decompression (ULBD) aim to preserve soft tissues while achieving adequate decompression of neural tissue. The study included 194 patients with symptomatic degenerative lumbar spinal stenosis, of whom 177 patients underwent UBE and 17 patients underwent Open ULBD. Clinical outcomes were measured with the visual analog scale (VAS) for pain, the Oswestry

Placental alkaline phosphatase promotes lipid accumulation and fibrosis in ligamentum flavum hypertrophy and represents a potential target for statin repurposing.

BACKGROUND AND OBJECTIVE: Lumbar spinal stenosis is a prevalent degenerative disorder driven primarily by ligamentum flavum hypertrophy. Emerging evidence links ectopic lipid accumulation to fibrotic remodeling and suggests a potential contribution of immune dysregulation. However, the molecular targets connecting lipid accumulation with ligamentum flavum fibrosis remain incompletely defined. We sought to evaluate the underlying mechanisms of lipid-lowering drug targets in lipid-associated ligamentum flavum fibrosis. METHODS: We applied two-sample Mendelian randomization (MR), summary-data-ba

Case Series: MILD® Procedure as a Minimally Invasive Option for Lumbar Synovial Cyst Decompression.

BACKGROUND: Lumbar synovial cysts are benign, fluid-filled lesions arising from the facet joints, often associated with degenerative changes and segmental instability. These cysts can cause neurogenic claudication and radiculopathy, significantly limiting mobility and quality of life. While traditional treatments include conservative management, injections, aspiration, and open surgery, each has notable limitations. The Minimally Invasive Lumbar Decompression (MILD®) procedure, originally designed for lumbar stenosis, may offer a less invasive alternative for synovial cyst resection, especiall

Closing the diagnostic gap in transthyretin cardiac amyloidosis: red flags, modern imaging, and emerging therapies.

Transthyretin cardiac amyloidosis (ATTR-CM) is an increasingly recognized, treatable, and still underdiagnosed cause of heart failure with preserved ejection fraction (HFpEF) and unexplained left ventricular hypertrophy in older adults, with a typical diagnostic delay of several years. We conducted a narrative review with a systematic literature search of PubMed and Embase (January 2000 to June 2026) using database-specific controlled vocabulary and free-text terms related to ATTR cardiomyopathy, clinical red flags, bone scintigraphy, monoclonal protein testing, cardiac magnetic resonance, taf
