Pneumocystis jirovecii, an atypical fungus, is a recognized cause of severe pneumonia in individuals with HIV. However, P jirovecii pneumonia (PJP) is now increasingly encountered among immunocompromised patients without HIV, including those receiving treatment for hematologic or solid organ malignancies, solid organ or bone marrow transplant recipients, patients with autoimmune diseases treated with immunosuppressive therapy, and patients requiring prolonged corticosteroid therapy; many of whom may not receive prophylaxis. This article summarizes characteristic and atypical chest CT findings

BACKGROUND: HIV is associated with a decreased diffusing capacity for carbon monoxide (DLco), decreased spirometry, and an increased risk for developing obstructive lung disease, even in never-smokers. Pneumonia is a risk factor for impaired pulmonary function in people with HIV (PWH), but whether recurrent pneumonia results in a commensurate decline in lung function is unknown.
METHODS: We reviewed the electronic medical records and PFT data for PWH in San Francisco, CA. Participants were categorized by number of pneumonias since HIV diagnosis. We analyzed the association between the number

BACKGROUND: People living with HIV (PLWH) have been reported to have higher unemployment rates than the general population. However, most previous studies were cross-sectional and lacked appropriate controls. To assess the impact of HIV diagnosis on job separation, we conducted a retrospective matched cohort study using a Japanese claims database of employed men. METHODS: We conducted a retrospective matched cohort study of newly diagnosed male PLWH and non-PLWH controls using a Japanese administrative claims database mainly covering employees of larger enterprises. PLWH were matched 1:5 with

BACKGROUND: Immune reconstitution inflammatory syndrome (IRIS) is an exaggerated inflammatory response to previously acquired opportunistic pathogens, causing clinical deterioration in the first months after starting antiretroviral therapy, contributing to morbidity and mortality in people living with HIV (PLHIV). In Mexico, data are limited, but IRIS has been reported in 11.4 - 27% of patients, with Varicella zoster as the most frequently associated pathogen and a mortality rate of 4.9 deaths per 100 person-years.
OBJECTIVE: To describe clinical and etiological characteristics of immune reco

BACKGROUND: Although people living with HIV (PLHIV) increasingly have access to antiretroviral therapy (ART) worldwide, the recurrence of advanced HIV disease (AHD) remains a major concern, exposing them to an enhanced risk of opportunistic infections (OIs) and subsequent death. We aimed here to describe the epidemiology of OIs among patients with AHD in Kinshasa (Democratic Republic of Congo, DRC) to inform future actions against AHD-related deaths. METHODS: A retrospective cohort study was conducted among consecutive AHD patients admitted to Biyela Hospital Center in Kinshasa (DRC)—a periphe

BACKGROUND: Pulmonary infections cause substantial morbidity and mortality among people with HIV. Our objective was to evaluate common urban exposures among those living with HIV which may increase risk of pulmonary infection.
METHODS: We enrolled 75 adult patients infected with HIV and admitted to San Francisco General Hospital into a hospital-based case-control study: 38 cases with pulmonary infection and 37 controls admitted for diagnoses other than pulmonary infection. We administered questionnaires to assess inhaled drug exposures, outdoor and indoor pollution, and occupational exposures

BACKGROUND: Pneumocystis jirovecii is a major cause of morbidity and mortality in HIV. It also poses a significant risk to non-HIV immunocompromised patients, especially those with cancer or recent chemotherapy. We evaluated risk factors, diagnostic markers-including lactate dehydrogenase (LDH)-and mortality predictors in PCR-confirmed pneumocystis pneumonia (PJP).
METHODS: We retrospectively analyzed patients with hypoxemia, pulmonary involvement and/or fever who underwent PJP PCR testing at Koç University Hospital between January 2020 and November 2023. Cases were defined as patients with a

BACKGROUND: HIV infection and AIDS remain serious global health problems. In Thailand, previous studies have revealed that nearly half of newly diagnosed people living with HIV (PLWH) presented with AIDS-defining illnesses (ADIs). However, data regarding the burden of ADIs among PLWH in northern Thailand have been limited.
METHODS: This retrospective cohort study analyzed 777 newly diagnosed PLWH between January 1, 2012, and December 31, 2022 at a tertiary care hospital in northern Thailand.
RESULTS: The study found that 29.0% of patients presented with ADIs at diagnosis, although this preva

BACKGROUND: Increasing case reports and monocentric studies suggest that co-infections may be associated with poorer outcomes in patients with pulmonary cryptococcosis (PC), including both people living with HIV (PLWH) and HIV-negative individuals. However, systematic cohort-based evidence evaluating the prognostic impact of co-infections remains limited.
METHODS: Total of 454 patients with PC (239 PLWH and 215 non-HIV) were included to compare clinical characteristics, CT imaging features, and relevant prognostic factors according to their co-infection status.
RESULTS: Among non-HIV patient

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

INTRODUCTION: Pneumocystis pneumonia (PCP) is a common AIDS-defining opportunistic illness in people with HIV infection, but its incidence has fallen with use of prophylactic treatment. Without treatment, PCP is likely to be fatal in people with AIDS, so placebo-controlled studies would be considered unethical.
METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the following clinical questions: What are the effects of first-line antipneumocystis treatments for Pneumocystis pneumonia in people infected with HIV? What are the effects of adjuvant corticosteroids in people

BACKGROUND: The global burden of the opportunistic fungal disease Pneumocystis jirovecii pneumonia (PJP) remains substantial. Polymerase chain reaction (PCR) on nasopharyngeal swabs (NPS) has high specificity and may be a viable alternative to the gold standard diagnostic of PCR on invasively collected lower respiratory tract specimens, but has low sensitivity. Sensitivity may be improved by incorporating NPS PCR results into machine learning models.
METHODS: Three supervised multivariable diagnostic models (random forest, logistic regression and extreme gradient boosting) were constructed an

BACKGROUND: HIV-related opportunistic infections like Pneumocystis jirovecii Pneumonia (PCP) remain a major contributor to child morbidity and mortality globally. PCP accounts for over 60% of AIDS in the first year of life and is responsible for a third of AIDS in children globally. Cotrimoxazole prophylaxis, which is an intervention directed towards tackling this burden, has not attained remarkable coverage despite advocacy towards scale-up. This work was therefore aimed at evaluating the efficacy of cotrimoxazole in the prevention of PCP among children exposed to and infected with HIV by car

INTRODUCTION: Pneumocystis pneumonia (PCP) is a common and serious complication of HIV/AIDS, with a higher prevalence in patients not receiving antiretroviral therapy. Due to the high mortality rate of PCP, accurate prediction of its case fatality rate is very important for clinical treatment. We aimed to develop a risk model for the near-term prognosis of people with HIV/AIDS and PCP and verify its effectiveness.
METHODS: This single-center, retrospective observational study was conducted at Beijing Youan Hospital from January 2012 to October 2022. 972 AIDS patients with Pneumocystis pneumon
