Risk of Pneumocystis jirovecii Pneumonia in Patients With HIV in Taiwan: Evidence from a Cross-sectional Study.

BACKGROUND/AIM: Studies have demonstrated that patients with HIV are at a higher risk of Pneumocystis jirovecii pneumonia (PJP). Epidemiological knowledge of the risk of PJP among patients with HIV infection is lacking. This study aimed to assess the risk of PJP among patients with HIV. PATIENTS AND METHODS: This cross-sectional study was conducted using the National Health Insurance Research Database of Taiwan. The participants were 18,929 patients with new-onset HIV infection from 2002 to 2015. Each patient was matched with four HIV-negative patients for age, sex, insured salary, urbanizati

Association of time-to-treatment with prognosis in pneumocystis pneumonia among immunocompromised patients without HIV infection: a multi-center, retrospective observational cohort study.

BACKGROUND: Pneumocystis jirovecii pneumonia (PCP) in non-human immunodeficiency virus (HIV) patients is associated with high morbidity and mortality. Although prior studies have linked delayed treatment to worse outcomes, they are often limited by small sample sizes and inadequate adjustment for confounders. Therefore, we evaluated whether early treatment after hospital admission improves mortality in non-HIV PCP, adjusting for patient characteristics. METHODS: This multi-center, retrospective, observational cohort study included non-HIV PCP patients treated between January 2006 and March 20

Positivity of polymerase chain reaction and Grocott staining in relation to the duration from therapy initiation to examination in Pneumocystis jirovecii pneumonia.

INTRODUCTION: The diagnosis of pneumocystis pneumonia (PCP) relies on microscopy and/or polymerase chain reaction (PCR) of bronchoalveolar lavage fluid (BALF). Although Pneumocystis jirovecii can be identified even after therapy initiation, the exact timeframe for reliable testing remains unclear. We investigated the relationship between diagnostic examinations for PCP and the duration from therapy initiation to the time of testing. MATERIALS AND METHODS: We retrospectively collected data from 105 patients diagnosed with PCP based on the diagnostic criteria across four institutions from Janua

Pneumocystis Pneumonia-Is it Still a Threat Among People With Human Immunodeficiency Virus (HIV)? A Danish HIV Cohort Study.

BACKGROUND: We aimed to examine pneumocystis pneumonia (PCP) risk and mortality among people with human immunodeficiency virus (HIV), and associations with (1) time after HIV diagnosis, (2) calendar time, and (3) CD4 cell count. METHODS: From the Danish HIV cohort study, we identified all adult people with HIV (PWH) (1995-2021). We estimated incidence rates (IRs) and mortality rates. We used Poisson regression analysis to compute adjusted incidence ratios (IRRs) and mortality rate ratios. PCP risk was assessed according to time after HIV diagnosis, calendar time, and low CD4 cell count. RESU

Causal relationships between immune cell subtypes and risk of Pneumocystis pneumonia and lung cancer: a Mendelian randomization study.

BACKGROUND: Our investigation seeks to elucidate the underlying biological mechanisms by which HIV infection might contribute to lung cancer development, thereby providing valuable insights for developing targeted prevention and treatment strategies for HIV-positive populations. METHODS: We integrated genetic datasets, identified causal SNPs through SMR/HEIDI analysis, and developed predictive models using multiple machine learning approaches with TCGA and GEO cohorts. Survival differences between risk groups were assessed with Kaplan-Meier analysis. We characterized gene functions using GO a

An Observational Analysis of a Large Cohort of Pneumocystis jirovecii Pneumonia.

BACKGROUND: Pneumocystis jirovecii pneumonia (PCP) is increasingly seen in patients who are non-HIV immunocompromised hosts, yet modern risk factors and outcomes are poorly recognized and defined. METHODS: A retrospective review was conducted of adult patients who met criteria for proven or probable PCP from 2017 to 2024 across a large academic multicenter health system. The primary outcome was mortality among cohorts, defined by etiology of immunosuppression through univariate and age-adjusted logistic regression. Secondary analyses included trends in annual diagnoses and testing patterns.
