Early mobilization after reverse total shoulder arthroplasty is not associated with increased risk of acromial and scapular stress fractures: a retrospective cohort study.

BACKGROUND: Acromial and scapular spine stress fractures (ASFs) are recognized complications following reverse total shoulder arthroplasty (rTSA) and may negatively affect clinical outcomes. The influence of post-operative immobilization duration and timing of rehabilitation on ASF risk remains unclear. This study evaluated whether a shorter post-operative sling immobilization protocol was associated with an increased incidence of ASF following primary rTSA. METHODS: A retrospective single-center comparative cohort study was performed including 698 patients who underwent primary rTSA between

Biomechanical analysis of T10-T11 vertebrae under the combined effects of osteoporosis and L4 spondylolisthesis: a finite element analysis based on computed tomography.

AIMS: Osteoporotic vertebral compression fractures are common; however, the biomechanical link between L4 spondylolisthesis and non-adjacent thoracic fractures remains unclear. This study used 3D finite element (FE) analysis to assess the biomechanical effects of L4 spondylolisthesis and osteoporosis on T10-T11. METHODS: Four FE models of T10-L5 were reconstructed from CT images of a healthy adult male: normal (A), isolated spondylolisthesis (B), isolated osteoporosis (C), and combined (D). A 500-N axial load and 10-N·m moments simulated six motions. Maximum von Mises stresses of whole verteb

Mechanical Testing and Computational Modeling of Scapula Fracture Implants: A Review.

This is the first review of mechanical testing or computational modeling articles that analyzed the biomechanical influence of parametrically changing scapula fracture implant (SFI) variables to optimize or characterize performance. PubMed and Embase were explored for articles using inclusion and exclusion criteria: (i) SFI articles that permutated implant variables were included, but articles using only one SFI configuration were excluded; (ii) SFIs comprising of plates (with screws), non-plate isolated "lag" screws, or cerclage wires were included, but anchors/sutures, bone cements, bone gra

FAPI-negative Papillary Renal Cell Carcinoma With Markedly FAPI-avid Sacral Insufficiency Fracture.

We report a 77-year-old woman with papillary renal cell carcinoma showing no increased [ 68 Ga]Ga-FAPI uptake in the primary tumor, while intense sacral FAPI uptake mimicked bone metastasis. MRI findings and 2-year follow-up supported a sacral insufficiency fracture without metastatic disease. This case highlights fracture-related FAPI uptake as an important oncologic imaging pitfall.

Conservative management of a cervically fractured zygomatic implant using a novel titanium device: a technical report with biomechanical validation and 24-month follow-up.

Fracture of a zygomatic implant is an uncommon but clinically challenging complication. Conventional management generally involves implant removal or alternative anchorage strategies, both of which may require extensive surgery, increase patient morbidity, and compromise an existing implant-supported rehabilitation. This technical report describes the development, biomechanical validation, and first clinical application of a novel prosthetic rescue device for the conservative management of cervical fractures of zygomatic implants while preserving the osseointegrated apical segment. Burosumab-responsive micro tumor-induced osteomalacia caused by a 6-mm phosphaturic mesenchymal tumor of the femoral head: a case report with review of literature on small (≤10 mm) FGF23-producing tumors.

A woman in her thirties presented with progressive bone pain, muscle weakness, recurrent stress fractures, and delayed fracture healing. Laboratory investigations demonstrated hypophosphatemia, renal phosphate wasting, elevated fibroblast growth factor 23 (FGF23), and inappropriately low 1,25-dihydroxyvitamin D, leading to a diagnosis of FGF23-mediated tumor-induced osteomalacia (TIO) approximately one year after symptom onset. Bone scintigraphy demonstrated multiple fractures involving the ribs and pubic bones. [18F]-fluorodeoxyglucose positron emission tomography/computed tomography ([18F]-F

Napari-3D-Counter: A manual cell counter for napari.

We describe the case of a 68-year-old woman with an acute episode of severe low back pain (LBP) resistant to opioids, who had experienced a sacral insufficiency fracture (SIF) two years earlier. At clinical examination, patient reported constant, dull, non-localizable pain at lumbar and sacral level, exacerbated by paravertebral palpation, particularly at L4-L5 and the sacroiliac joint, with a concomitant and remittent neuropathic component, difficult to localize at lumbar and sacral level. The latest magnetic resonance imaging study revealed disc herniations at L3-L4, L4-L5, and L5-S1 levels.

A biomechanical framework for skeletal injury analysis in upright falls: integrating stress propagation, fracture risk modeling, and injury clustering.

Utilizing the Finite Element Method and the Total HUman Model for Safety model, simulations were conducted of adult subjects landing with both feet on a hard surface from various heights. The biomechanical response was deconstructed using stress analysis, binary logistic regression, and hierarchical clustering based on Jaccard distance. The simulations revealed a stress propagation mechanism and a longitudinal axial distribution of stress concentrations. Fracture risk exhibited two distinct modes: a "stepwise response" in primary load-bearers (e.g., feet, spine) and a "graded response" in seco

Biomechanical comparison of posterior short-segment fixation with or without intermediate screws for thoracolumbar burst fractures under normal and osteoporotic conditions: a finite element analysis.

STUDY DESIGN: Finite element analysis. PURPOSE: To investigate the biomechanical response of posterior short-segment fixation with or without intermediate screws at the index vertebra in osteoporotic thoracolumbar burst fractures using finite element analysis. OVERVIEW OF LITERATURE: Spinal fixation in elderly patients with osteoporotic vertebral fractures is challenging because osteoporosis weakens the screw-bone interface, leading to screw loosening and loss of fracture reduction. Short segment fixation with intermediate screws has been proposed to reduce kyphosis recurrence and implant fa

Anatomical Investigation of the Gluteus Medius Muscle Innervation and Its Topographical Correspondence With Myofascial Trigger Points.

BACKGROUND AND PURPOSE: Hip pain, a common complaint among adults that often causes functional disability, can be caused by femoroacetabular impingement, labral injuries, stress fractures of the femoral neck, avascular necrosis of the femoral head, osteoarthritis of the femoroacetabular joint, hip fractures, greater trochanteric pain syndrome, pathology of the lumbar spine and sacroiliac joint, and myofascial pain syndrome (MPS). MPS is characterized by the presence of hyperirritable nodules, known as myofascial trigger points (MTPs), within muscles and fascia. MTPs limit the range of motion o

Spinal Compression Fracture Mechanisms in Mountain Biking: An Accident Reconstruction Approach.

Study DesignBiomechanical Simulation Study.ObjectivesSpinal injuries account for 30-40% of severe mountain biking (MTB) cases and may be mitigated by back protectors. Thoracolumbar compression fractures are linked to rolling impacts during forward falls, yet their biomechanical mechanisms remain poorly understood. It is unclear whether current back protectors, tested under perpendicular conditions (EN1621-2), adequately address the complex loading of real-world crashes. This work aims to investigate the biomechanical mechanisms of thoracolumbar fractures during MTB forward falls.MethodsForty-fo

Standard vs. the alternative centerline technique for baseplate fixation in reverse shoulder arthroplasty: a comparison of anterior shoulder pain.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has increasingly become a reliable intervention to improve function and pain for a variety of indications. Glenoid fixation is critical to the success of rTSA, and traditionally the standard anatomic centerline is used, allowing for central baseplate fixation. However, in cases of severe bone loss, baseplate fixation along the alternative centerline has been described. The baseplate axis is anteverted and inferiorly tilted, allowing for fixation into a column of bone where the base of the scapular spine and coracoid meet, thereby prioritiz

Disc degeneration did not lead to an increased incidence of adjacent vertebral fractures in wedge-type OVCF patients.

BACKGROUND: Stress concentration is a primary pathogenesis of adjacent vertebral fracture (AVF) following percutaneous vertebroplasty (PVP) for osteoporotic vertebral compression fractures (OVCF). Disc degeneration (DD) is highly prevalent in the elderly, including OVCF patients. As a key biomechanical function of the intervertebral disc (IVD) is to ensure uniform load distribution, the progression of DD may impair this capacity, leading to localized stress concentrations. This study aims to investigate the impact of DD on AVF risk and elucidate its underlying biomechanical mechanisms. METHOD
