Multidisciplinary care and advanced therapeutics in end-stage heart failure: A case report.

UNLABELLED: The patient was a 72-year-old woman diagnosed with dilated-phase hypertrophic cardiomyopathy at age 60 years. At the age of 67 years, the patient began to experience recurrent heart failure (HF) that required frequent hospital admissions, and at the age of 71 years, home-based dobutamine (DOB) infusions were initiated, which reduced the number of HF-related hospitalizations. Two months prior to the patient's death, she was admitted for a compression fracture of the lumbar spine and congestive HF. Despite continued DOB (3 μg/mL/min) infusions, her health deteriorated progressively b

The distal acromial tip fusion: a novel procedure to mitigate acromial stress fractures in high-risk patients undergoing reverse shoulder arthroplasty.

Acromion and scapular spine fractures (ASFs) pose significant challenges in the management of patients undergoing reverse total shoulder arthroplasty and there is a pressing need for effective preventive measures. Current predictive factors for ASF include female sex, osteoporosis, older age, and anatomical considerations such as glenoid medialization and acromial morphology. However, surgical options to mitigate ASF risk are limited. In this study, we propose a novel technique: incorporation of distal acromial tip fusion (DATF) during primary reverse total shoulder arthroplasty procedures. DA

Risk factors for acromial and scapular fractures following reverse shoulder arthroplasty: a meta-analysis of over 100,000 shoulders.

BACKGROUND: Stress acromial fractures (AF) and scapular spine fractures (SSF) are rare but challenging complications of reverse total shoulder arthroplasty (rTSA). The purpose of our study was to identify risk factors of AF and SSF after rTSA through a detailed systematic review of published literature. METHODS: A systematic review of PubMed, Scopus, Web of Science, and Cochrane Library was conducted from inception to October 2023. We included all peer-reviewed studies investigating the risk of AF and SSF after rTSA, excluding biomechanical studies. A total of 1,047 publications were reviewed

Mechanical Performance of Monoaxial Versus Polyaxial Locked Plates for Bone Fracture Repair: A Review.

Widely used "locking" bone fracture plates include standard monoaxial locking plates (MLPs) that allow a single screw insertion angle relative to the plate, as well as newer polyaxial locking plates (PLPs) that permit various screw insertion angles relative to the plate. This is the first review of all studies that compared the mechanical performance of MLPs vs. PLPs. PubMed and Web of Science were searched using the phrases "locked plate polyaxial" and "locked plate variable." Eligibility criteria were applied: mechanical studies, rather than clinical studies, that directly compared MLPs vs.

[Finite element analysis of skeletal injury mechanisms in adult standing positions with both feet landed on the ground].

OBJECTIVES: The prevalence and complexity of high-fall injuries pose major challenges to injury-mechanism interpretation, which currently relies largely on forensic autopsy. The finite element method (FEM) can elucidate biomechanical mechanisms of skeletal injury and quantify the relationship between fracture risk and fall height, providing evidence for forensic injury analysis and scene reconstruction. METHODS: Using adult upright feet-first landing as a representative scenario, a finite element approach based on the total human model for safety (THUMS) was applied to simulate bilateral feet

CT-Based Characterization of Fracture Patterns in Pediatric Lumbar Spondylolysis.

BACKGROUND: Spondylolysis is a stress fracture of the pars interarticularis. Precise fracture morphology may help dictate management and dictate whether direct repair with an intralaminar screw is possible. The aim of this study was to characterize the specific fracture patterns of the pars interarticularis in pediatric patients with spondylolysis. METHODS: This was a single-center retrospective cohort study. Patients were included if they were aged younger than 21 years with lumbar spondylolysis or Grade 1 isthmic spondylolisthesis on computed tomography (CT) imaging. On sagittal CT, total p

Multilevel posterior neural arch fractures of the lumbar spine: a rare complication due to prior malignant infantile osteopetrosis.

INTRODUCTION: Osteopetrosis is a heterogeneous group of inheritable bone disorderswhich manifest themselves as increased bone density and brittleness that leads to pathologicalfractures and bone deformities. Despite the propensity of patients to present with pathologicalfractures, there have been few reports of the spine being affected until now. CASE PRESENTATION: We present the case of a 23-year-old man who had previously had malignant infantile autosomal recessive osteopetrosis and complained of chronic back pain for several months. We found multilevel (L1-L5) neural arch stress fractures.

Severe Type 1 Thumb Boutonniere Deformity with Interphalangeal Joint Dislocation in a Young Patient: A Case Report.

INTRODUCTION: Ankylosing spondylitis (AS) is a chronic inflammatory disorder that primarily affects the spine and sacroiliac joints, leading to pain, stiffness, and progressive thoracolumbar kyphotic deformity. A key complication in advanced AS is the development of Andersson lesions (AL), degenerative vertebral lesions resulting from the disease's progression. These lesions can cause significant mechanical pain, often mistaken for the chronic discomfort associated with AS. The exact cause of AL remains unclear, with hypotheses ranging from spinal stress fractures to delays in the ankylosing p

Langerhans Cell Histiocytosis Presenting as a Limp in a Child: A Case Report.

We report a case of multifocal Langerhans cell histiocytosis (LCH) in a previously healthy four-year-old male child who initially presented to the chiropractic clinic with a three-week history of progressive left-sided limping and nocturnal leg pain. Upon evaluation, the patient demonstrated an antalgic gait, point tenderness over the left femur and tibia, and constitutional symptoms including low-grade fevers, fatigue, and recent weight loss. Radiographic evaluation revealed multiple lesions in the left femur, tibia, and pelvis, with subsequent MRI confirming infiltrative lesions and a pathol

Management of Bone Stress Injuries: A Holistic Approach.

Stress fractures are fatigue fractures of bone that result from an overuse mechanism. These injuries present most commonly in the legs and feet of distance runners and military personnel but can also occur in the upper extremity and the spine depending on the causative activity. Nutritional, hormonal, and biomechanical factors influence the development of bone stress injuries, and no two bone stress injuries behave identically. A thorough history, detailed physical examination, and proper imaging are required to diagnose and classify these injuries. Management of stress injuries requires nutri

Innovative Dynamic Ultrasound Diagnosis of First Rib Stress Fracture in an Adolescent Athlete-A Case Report.

Background: First rib stress fractures (FRSFs) are exceptionally rare in skeletally immature athletes and are frequently overlooked because their symptoms mimic more common scapular conditions such as scapular dyskinesis or thoracic outlet syndrome. Early and accurate identification is critical to avoid delayed union, prolonged disability, and misdirected management. Case Presentation: We report a 12-year-old elite baseball pitcher with progressive scapular winging and audible snapping during pitching. Unlike typical posterior-type fractures near the costotransverse joint, imaging revealed a c

Delayed Neurological Deficits Caused by Epidural Leakage of Intravertebral Contents in a Patient With Osteoporotic Vertebral Fracture.

Delayed neurological deficits (DND) after osteoporotic vertebral fractures (OVF) are a rare but serious complication. In patients with Parkinson's disease (PD), involuntary movements such as dyskinesia may increase biomechanical stress, potentially exacerbating spinal instability. We report the case of a 74-year-old woman with PD and a history of T12 OVF, who presented with progressive bilateral thigh pain and gait disturbance despite conservative treatment and bed rest. Imaging revealed a posterior epidural mass at T12 with an atypically high CT attenuation (300-340 hu), inconsistent with hem

A Comprehensive Analysis of Vertebroplasty and Kyphoplasty for Osteoporotic Vertebral Compression Fractures and Postoperative Sagittal Balance.

INTRODUCTION: Appropriate indications for endoscopic sinus surgery (ESS) for chronic rhinosinusitis (CRS) are currently poorly defined. The lack of clear surgical indications for ESS likely contributes to the large geographic variation in surgical rates and contributes to reduced quality of care. The objective of this study was to define appropriateness criteria for ESS during management of adult patients with uncomplicated CRS. METHODS: The RAND/UCLA appropriateness methodology was performed. An international, multi-disciplinary panel of 10 experts in CRS was formed and completed two rounds

Localisation and outcome of presumed thoracolumbar vertebral stress fracture in 35 UK Thoroughbred racehorses using nuclear scintigraphy.

BACKGROUND: Stress fracture is an important cause of mortality and morbidity in racing Thoroughbreds. Nuclear scintigraphy is the imaging modality of choice for investigating unlocalised stress-related bone remodelling in horses. Stress fractures of the caudal lumbar vertebral column have been previously described in both racing Quarter horses and racing Thoroughbreds post-mortem. There is a lack of literature describing the imaging appearance of vertebral stress fractures in racehorses ante-mortem. OBJECTIVES: To describe: (1) ante-mortem scintigraphic appearance of presumed thoracolumbar ve

Long term risk of scapular fracture following reverse shoulder arthroplasty.

PURPOSE: Acromial stress fractures (ASF) and scapular spine fractures (SSF) are devastating complications of reverse shoulder arthroplasty (RSA). There is limited literature on the impact of age and incidence of these fractures at long term follow-up. We aimed to identify incidence, timing, and age of occurrence in a single centre over a 20-year period. METHODS: Retrospective consecutive case series of all primary RSA performed between 1999 and 2018 at a single centre. Cases had a minimum of 4-year follow-up. Notes and radiographs were reviewed to identify scapular fractures. Associated demog

Essential oils as promising treatments for treating Candida albicans infections: research progress, mechanisms, and clinical applications.

Study Design Retrospective case series. Objective Ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH) are two related diseases that significantly increase the risk of unstable spinal fractures from seemingly trivial trauma. Given the older age and higher surgical risk profile of most of these patients, minimally invasive (MIS) approaches to the treatment of such fractures may reduce operative risk and physiologic stress. Methods Eleven consecutive patients with hyperextension thoracolumbar injuries and a diagnosis of AS or DISH admitted to a single level I trauma ce

N-cadherin Alleviates Apoptosis and Senescence of Nucleus Pulposus Cells via Suppressing ROS-dependent ERS in the Hyper-osmolarity Microenvironment.

The in-situ osmolarity is an important physicochemical factor that regulates cell fate of nucleus pulposus cells (NPCs). Our previous studies demonstrated that reduced N-cadherin (NCDH) expression in nucleus pulposus cells is associated with cellular damage under hyper-osmolarity microenvironment. This study was aimed at exploring the impacts of NCDH on senescence and apoptosis of NPCs, as well as the potential molecular mechanism. By comparing NPCs from patients with lumbar fractures and lumbar disc herniation, we identified a correlation between decreased NCDH expression and increased endopl
