Stress fractures in athletes. A study of 320 cases.

We analyzed cases of 320 athletes with bone scan-positive stress fractures (M = 145, F = 175) seen over 3.5 years and assessed the results of conservative management. The most common bone injured was the tibia (49.1%), followed by the tarsals (25.3%), metatarsals (8.8%), femur (7.2%), fibula (6.6%), pelvis (1.6%), sesamoids (0.9%), and spine (0.6%). Stress fractures were bilateral in 16.6% of cases. A significant age difference among the sites was found, with femoral and tarsal stress fractures occurring in the oldest, and fibular and tibial stress fractures in the youngest. Running was the mo

Transverse fractures of the spinous process of the 7th cervical vertebra in RDT patients: an Al related disease?

Transverse fractures of the spinous process of the 7th cervical vertebra were noted in 2 patients and of the 1st thoracic vertebra in 1 patient on maintenance hemodialysis. Two patients with aluminum (Al)- and/or iron-related bone disease complained of bone pain, relieved by deferoxamine therapy. In all three cases, the bone fractures had occurred suddenly while the patients were going about their daily work. These observations indicate that Al- or iron-related bone disease with secondary hyperparathyroidism can induce bone fracture by only slight stress in patients maintained on hemodialysis.

The resting spine. A conceptual approach to the avoidance of spinal reinjury during rest.

During periods of rest, the human spine assumes many different postures. This article examines the patterns of stress exerted on the lumbar spine by the more commonplace reclining postures. It proposes a rationale for choosing an optimal resting posture. In the event that the spine is injured, this study suggests that a sleeping posture that will minimize the stress at the level of injury should be adopted.

Ankylosing spondylitis: experience in surgical management of 21 patients.

Between 1976 and 1984, twenty-one patients with ankylosing spondylitis were treated surgically. Eight patients with rigid thoracic kyphosis underwent a two-stage combined procedure. The average correction was 36 degrees. Eight patients underwent a single-stage lumbar osteotomy with Harrington compression instrumentation. The average correction was 31 degrees. Five patients presented with stress fractures and back pain. All underwent combined anterior and posterior surgery. The average correction was 9 degrees. Two patients underwent anterior decompression for progressive paraplegia. Both showe

Fatigue fracture of human lumbar vertebrae.

Fatigue fracture of human lumbar vertebrae under cyclic axial compressive load is investigated between 1 and 5000 load cycles and for load magnitudes between 30 and 70% of the ultimate compressive strength of human lumbar vertebrae. In addition, it is shown that the ultimate compressive strength of human lumbar vertebrae can be predicted from trabecular bone density determined by quantitative computed tomography in combination with a measurement of the area of the vertebral end plate.

Roentgen examination of groin and hip pain in the athlete.

Stress fractures and reactions of the pubic ramus, pubic symphsitis, gracilis syndrome, pelvic avulsion injuries, femoral stress fractures, degenerative osteoarthritis, discogenic pain, and spondylolysis are among the multiple conditions causing groin pain in the athlete. These conditions occasionally have uncertain etiologies and are contributed to by poor training techniques and ill-repaired running shoes, combined with minor congenital anomalies that may have been silent prior to the demands of athletic competition. Roentgen documentation of the specific injury enables early appropriate tre

Spondylolysis in Scheuermann's disease.

Increased lumbar lordosis places increased stress on the pars interarticularis. Fatigue fractures of the pars can result in spondylolysis. There was a 50% incidence of asymptomatic spondylolysis in 18 patients who had Scheuermann's kyphosis and an increased lumbar lordosis. This significant increase further confirms the pathogenesis of spondylolysis. Patients with Scheuermann's disease with low-back pain should be evaluated with oblique radiographs of the lumbar spine to rule out spondylolysis.

Chronic repetitive unrecognized flexion injury of the cervical spine (high jumper's neck).

Unrecognized flexion injuries of the cervical spine may lead to late instability and neurologic damage. These hidden flexion injuries may be from acute or chronic traumatic episodes. Cervical spine instability was seen in an amateur high jumper as a result of chronic repetitive flexion loading of her cervical spine due to incorrect landing technique. The instability from these types of flexion injuries is generally unrecognized on a routine lateral radiograph. The presence of slight anterior subluxation, simple compression fractures, or subtle kyphotic angulation at one cervical level should a

Fracture of the pars interarticularis in adolescent athletes: a clinical-biomechanical analysis.

Stress fracture of the pars interarticularis is an increasing cause of disability in highly competitive adolescent athletes. We have documented this lesion in 14 adolescent athletes engaged in repetitive training and competition exercises involving flexion/extension of the lumbar spine. An in vivo spinal muscle torque study in 11 normal adolescent girls also revealed mean torque values of 22.6 Nm for left lateral flexion and 27.4 Nm for hyperextension. The most common sports resulting in this lesion were gymnastics and hockey. In four patients the lesion was bilateral, and in 10 it was unilate

[Shear fracture stress of the spongiosa and pressure fracture stress of cortical bone of cadaveric vertebrae].

By biomechanic examinations of the vertebrae of human dead bodies the ultimate stress due to shearing (Schubbruchspannung) of the spongy substance could be found out with 1.80 N/mm2 and the ultimate stress due to compression (Druckbruchspannung) of the cortical-belonging to the thick of cortical--with 280 N/mm2. Both measuring results are important for the ultimate strength of the connection (Verankerungsfestigkeit) of screws in vertebae.

Nuclear magnetic resonance proton imaging of bone pathology.

Thirty-two patients with diversified pathology were examined with a supraconductive NMR imager using spin echo with different TR and TE to obtain T1 and T2 weighted images. They included 20 tumors (12 primary, eight metastasis), six osteomyelitis, three fractures, two osteonecrosis, and one diffuse metabolic (Gaucher) disease. In all cases except for the stress fractures, the bone pathology was clearly visualized in spite of the normal lack of signal from the compact cortical bone. Nuclear magnetic resonance (NMR) imaging proved to be at least as sensitive as radionuclide scintigraphy but much

Injury of the annulus fibrosus and disc protrusions. An in vitro investigation on human lumbar discs.

Discs of 25 specimens of human lumbar motion segments were subjected to an internal division of the annulus fibrosus, sparing only a peripheral layer 1 mm thick. Thus an attempt was made to simulate an internal disruption of the annulus caused by a traumatic episode or a degenerative process. The disc bulge that developed at the site of the injury was observed under axial loads in the physiologic load range, after axial compression fracture and after intradiscal injection. Under 1,000-N load, the bulge amounted to less than 0.5 mm; typically it increased to less than 1.0 mm after fracture. An

Traumatic atlantooccipital dislocation.

A young adult sustained a traumatic atlantooccipital dislocation together with fracture-dislocation at C.4/5 level and had total neurologic deficit below C4 segment. He retained full consciousness but required respiratory support. He developed a stress ulcer with hemorrhage and evidence of "shock lung." He responded to intensive care. Surgery on the 11th day secured reduction and internal fixation at both levels. Death due to respiratory and cardiac arrest followed on the 14th day. Postmortem examination revealed edema of the brain and brainstem.

The optimum spine.

System theory is used to describe the mechanism of the lumbar spine. The role of the spine in vertebrate evolution is presented. The importance of the intervertebral joint for the survival of the species is shown to be crucial. The mechanical behavior of the joint is derived, and from this the corresponding spinal motion and muscular responses is calculated. It is shown that physiologic behavior implies that the stress at the intervertebral joints is equalized and minimized. From this simple condition, the motion of the spine in the sagittal plane is calculated. From the analysis of sagittal p
