The posterior rings of the lower lumbo-sacral vertebrae are subject to stress fractures at any part - pedicle, pars, or lamina. The site of fracture is apparently determined by the axis of weight bearing. The three illustrative clinical examples cited include a jogger with a laminar fracture, a ballet dancer with pedicle fractures, and a nine-year-old boy with fractures of pars and lamina. Chronic low back pain is the typical complaint with stress fractures of the lower lumbo-sacral spine. Special imaging techniques are usually needed to demonstrate these lesions, including vertebral arch view

An increasing scoliosis was documented in seven elderly women. The average curve at the most recent examination was 43 degrees (range 26 degrees-78 degrees). Previous films, from 5 to 26 years before, demonstrated an average increase of 2.3 degrees/year (range 1 degree-4.8 degrees/year). There were three lumbar and four thoracolumbar curves. Three curves were to the right and four were to the left. Only one patient had osteoporotic vertebral body crush fractures. The common underlying mechanism in the progression of senile scoliosis appears to be asymmetric loading of the spine which can be ca

Fifty patients (14-55 years of age) with unstable thoracolumbar fractures were studied: 24 patients treated conservatively 1971-1977 and 26 patients treated surgically with Harrington instrumentation 1977-1981. The treatment groups were comparable in all respects. Radiologic evaluation showed that Harrington distraction rods restored the fractured vertebra almost to its original shape, and the gibbus and scoliosis were significantly reduced. However, at the follow-up examination at least 2 years after the injury, the gibbus angle had recurred almost to the value at admission in patients with t

Three cases of stress fractures affecting the rigid spine of ankylosing spondylitis are reported. Even without the typical destructive features of the Romanus lesion, symptoms may be very prolonged and disabling and the diagnosis difficult. Internal fixation produces immediate pain relief and rapid fracture union.

Eight mechanically failed Harrington rods have been retrieved and examined clinically, metallurgically, and biomechanically to characterize the mode of instrument failure and to determine how future failure rates might be minimized. The results of the study indicated fatigue to be the mode of mechanical failure in all cases, however, in only one case was any significant metallurgical defect observed. Failure occurred at the ratchet-shaft junction in seven of eight cases with the remaining failure occurring at the midshaft region. This occurred in the rod with the metallurgical defect. The poss

The purposes of using Harrington instrumentation for the treatment of thoracolumbar fractures are to reduce the fracture, decompress the spinal canal, create stability at the fracture site, and shorten the hospitalization period. However, technical problems or the injudicious use of Harrington-instrumentation systems can also complicate the management of these fractures. We have studied forty patients (forty-five Harrington-instrumentation stabilization procedures) who had significant complications. Twenty-six of the thirty patients who were followed for more than two years required additional

Fourty-five children presenting with an isthmic lysis, complicated or not with spondylolisthesis were studied. The authors suggest that the isthmic lysis could be a stress-induced lesion, a "fatigue fracture" which may heal if treated early, before spondylolisthesis occurs. The main point is therefore an early diagnosis.

Insufficiency stress fractures may occur in the sacrum after radiation therapy or secondary to postmenopausal or steroid-induced osteoporosis. These fractures are often either overlooked or confused both clinically and radiographically with metastatic disease. Findings on plain films and conventional tomograms are often subtle. Radionuclide bone scans show a characteristic distribution of increased uptake. Computed tomography is the definitive technique for demonstrating the fractures.

Because the etiology of spondylolysis is not well understood, the authors performed an analysis of loads and stresses in human lumbar vertebrae to determine whether purely mechanical factors are likely to cause spondylolytic fractures in a normal spine. To perform these studies, modeling methods were applied. A mechanical system was developed to study muscle forces and reactions in joints of the lumbar spine. Next an optimization approach was applied to find loads on vertebrae and muscle forces. Finally photoelastic experiments were performed to find effective stresses and stress concentration

In the second part of this biomechanical study of thoracolumbar spinal fractures with bone in the neural canal contract force on the spinal cord--meningeal complex was measured with relation to varying fracture wedge depth, flexion angulations, and distraction and shortening of the fracture model motion segment. The removal of progressively deeper vertebral body wedges did not change the measured contact force/depth of penetration characteristics, however, the resistance to flexion angulation was decreased. Distraction of the motion segment an average of 5.2 mm increased significantly the cont

Fourteen cervical spine motion segments consisting of two adjacent vertebral bodies and their connecting ligaments were tested in shear. Five had intact facet joints, five had bilateral facetectomy of 50% or less, and four had bilateral 70% facetectomy. Three to 5 mm of root could be exposed in the specimens with 50% facetectomy, and 8 to 10 mm in those with 70% facetectomy. Anterior-posterior shear tests were run alternately in compression and distraction. Facetectomy was found to have no effect on compression and distraction stiffness. Failure in the 70% facetectomized specimens was due to f

Retrosomatic spondylolysis is a very rare type of neural arch defects. In typical cases the defect is a vertical cleft of the pedicle, which is thought to be the result of two ossification centers. The authors present a case of retrosomatic spondylolysis with a segmentation defect of the corresponding vertebral arch structures. The follow-up of this child from age 7 to 12 seems to be proof for this defect to be a fatigue fracture quite similar to those in isthmic spondylolysis.

Insufficiency fractures in the supraacetabular region were identified in five women, aged 55-83 years. Factors contributing to the diminished resistance of their bones included postmenopausal osteoporosis, steroid therapy, radiation therapy, and rheumatoid arthritis. The supraacetabular fractures were seen on routine radiographs as hazy bands of sclerosis located immediately above and parallel to the acetabular roof. All five patients had additional fractures in the spine or pelvis. Supraacetabular insufficiency fractures may be an unsuspected cause of hip pain, especially in older women.

A case of malignant lymphoma in the skull after head injury associated with whole bone metastasis is reported. The patient was a 66-year-old man who was admitted to Almeida Memorial Hospital because of headache and general fatigue 2 months after head injury. After admission tumors appear in the frontal and occipital region and grew rapidly. Plain craniogram revealed large map-like bone destructions and multiple punched out lesions. Bone scintigram with 99mTc-MDP revealed multiple accumulations of RI in the skull, vertebrae, ribs and pelvis. CT scan revealed destructive, markedly enhancing bone

The assessment of bone status is so far usually performed by means of the relatively simple methods of radiographs and radiogrammetry, which are not always able to give a correct information about bone mineral content (B.M.C.) especially in cases of cortical osteoporosis. Among the other non-invasive methods, single- and dual-photon absorptiometry are of particular interest for their relatively low cost, precision, accuracy and repetitibility, giving the patient a very slight dose of radioactivity. Dual-photon device, in particular, offers the possibility to investigate B.M.C. in the bones sur

Effective management of mechanical back pain in the athlete requires an understanding of the significance of the nature and the location of the pain and which injuries are related to the age or sport of the athlete. Muscle strains and interspinous ligament sprains occur at any age and in all sports. However, the adolescent may have an iliac crest apophysitis rather than a muscle strain, and the gymnast or diver may have interspinous process bursitis or a stress fracture of the pars interarticularis rather than a ligament sprain. Similarly, the swimmer with upper thoracic pain, training in the

This manuscript is directed to review the epidemiology of spinal trauma, the anatomy of the vertebral column, spinal ligaments, muscles, motion of the spine and spinal cord. Because little information is available on the material properties of ligaments and the components of the vertebral column, this material is also included. A review of the experimental spinal cord injury models is given because of the concerted interest in this area today. Laboratory studies conducted in animals to develop typical spinal cord injury models and the corresponding alterations in perfusion and metabolic pathwa

Seven cases of cervical spondylolysis are reported one of them with cervical myelopathy, and the literature is reviewed. The particular anatomical features of the articular processes observed in 4 cases, as well as the many characteristics common to cervical and lumbar spondylolysis, suggest that both diseases might be caused by acquired fatigue fractures of the isthmus due to abnormal stress in patients predisposed by neural rich malformations.

Spondylolysis occurring after a spinal fusion is considered to result from operative damage to the pars interarticularis on both sides. Fourteen cases are reported, and compared with the 23 cases which have previously been published. The defects are usually recognised within five years of fusion, and usually occur immediately above the fusion mass. Other contributory causes may be: fatigue fracture from concentration of stress; damage and altered function of the posterior ligament complex; and degenerative disc disease immediately above or below the fusion. Fusion technique is critical, since
