Of 10 patients with spondylolisthesis of the fifth lumbar vertebra, 8 had characteristic tight hamstrings syndrome, and 4 showed a Grade IV slip. Sciatic pain was present in all cases. The cause of spondylolysis and spondylolisthesis remains unknown. There is increasing evidence that the defect in the pars interarticularis is due to fatigue fracture rather than being of congenital origin. We describe the youngest patient on record with spondylolysis and spondylolisthesis in whom roentgenograms that showed no abnormality had previously been taken. This case supports the hypothesis that spondylolysis and spondylolisthesis are acquired and not congenital, even when discovered in a very young child.

An unusual vertical fracture through the superior articular process of L5 is described in a professional skier. This fracture, not associated with a pars interarticularis defect, is felt to be stress related. Standard oblique radiographs failed to demonstrate the defect but it was seen clearly on steep oblique views. The fracture was the casue of chronic backache to the patient. Followup radiographs after 14 months demonstrated a nonunion of the fracture.

Basing on roentgenological observation of the course of the disease, and on postmortem examination, of a patient with polysegmental Andersson lesions (known as spondylodiscitis) in ankylosing spondylitis, the article reports on the histological and often also clinico-roentgenological possibility of differentiating between an inflammatory and non-inflammatory type of this destructive lesion. The authors stress the frequency of metastases of the cervico-occipital junction unrecognized in medical literature: 30 cases have been diagnosed within four months. They report their different patterns and complications and insist on the prefractured pattern with osteolysis of the body of C-2 which can be shown only by polytomography. Flexion-extension motion and compression-distraction forces across the cervical spine were evaluated in 6 patients in halo vests and in 1 patient in a halo cast. Motion in the vest was evaluated in supine and upright positions and averaged 31% of normal motion at the levels tested. Compression-distraction forces showed great individual variation and frequent compression. The average distraction force varied in different positions by nearly 20 pounds in the vest and over 30 pounds in the cast. Methods of improving cervical spine stability in halo orthoses are discussed.

Previous reports and a study of the mechanism of injury in 25 cases of fracture of the odontoid suggest that the fracture usually results from a high velocity force directed to the head, producing flexion, extension, or rotation of the upper cervical spine. The fatigue strength of vertebral posterior elements was determined by applying repetitive forces to the inferior joints between the articular processes of human rhesus-monkey vertebrae. The results indicate that in vivo stresses resulting from unusual activities for sustained periods can induce fatigue fracture of the lumbar vertebrae. Comparison of these data with those of other investigators suggests the use of ultimate strength as a normalizing factor to correlate fatigue-strength data obtained under different test conditions.

Four failed polycentric total knee prostheses were evaluated by clinical examination of the patients and surface analysis of the removed prosthetic components.
