We report a case of high-risk prostate cancer (pT3bN1Gleason score 4+5), in a 63-year-old male who underwent robot-assisted radical prostatectomy with extended pelvic lymph node dissection in May 2019, followed by adjuvant combined androgen blockade (CAB) therapy. In June 2022, the case progressed to castration-resistant prostate cancer (CRPC), and enzalutamide was initiated. Bone metastases to the sacrum and seventh thoracic vertebra were identified concurrently and treated with radiotherapy. In January 2024, the patient presented with lower back pain and left thigh pain. Computed tomography

PURPOSE: Acromial and scapular spine fractures after reverse shoulder arthroplasty (RSA) can impair post-operative outcomes; however, quantitative assessment of pre-fracture cortical changes remains challenging. This study evaluated longitudinal changes in inferior acromial cortical thickness after RSA using a computed tomography-based (CT-based) acromial coronal view.
METHODS: This multicentre retrospective cohort study included 44 patients who underwent RSA. The acromion was divided into three zones: I (lateral acromion), II (acromial base), and III (scapular spine). Inferior cortical thick

BACKGROUND: Scapulothoracic posture is known to influence shoulder mechanics, with kyphotic (type C) alignment associated with altered scapular orientation and muscle balance. However, the biomechanical consequences of scapular posture on individual muscle loading and bone strain remain poorly defined, limiting the ability to tailor surgical planning and rehabilitation to a patient's baseline alignment.
QUESTIONS/PURPOSES: (1) Does a type C posture alter deltoid recruitment and rotator cuff loading patterns compared with a type A upright posture across dynamic shoulder motions? (2) Does type

Spinal lesions in patients with breast cancer are often presumed metastatic; however, benign insufficiency fractures can closely mimic metastatic disease, creating a diagnostic challenge. We report the case of a 52-year-old female with estrogen receptor-positive, progesterone receptor-positive, and HER2-negative (ER/PR+, HER2-) metastatic breast cancer to the liver who presented with severe back pain and peripheral numbness. Imaging revealed bilateral pedicular insufficiency fractures at the level of L3 and L4 vertebral bodies. The pedicular fractures demonstrated imaging features favoring ins

Lumbar bone stress injuries are a frequent cause of low back pain in children and adolescent athletes. They are approximately two to three times more prevalent in symptomatic athletes than in the general population. The key challenge is the early identification of an active, potentially healable lesion. MRI is the imaging modality of choice. Advanced "CT-like" bone MRI sequences allow for the early detection of defects and healing assessment without ionizing radiation, thereby limiting the need for CT. Management remains mainly conservative and aims at bony healing. It relies on respecting pai

This study aims to assess the biomechanical impact of bone cement perfusion on osteoporotic vertebral compression fractures across various distribution sites through finite element simulation. The finite element model of the L1-L3 vertebral body was constructed and its efficacy was validated. Simulated compression fractures of the L2 vertebral body resulted in the distribution of cement in the anterior, middle, and posterior thirds of the L2 vertebral body. Physiological loading and boundary conditions of axial compression experienced by the spine in the upright position were established, and

BACKGROUND: Sacroiliac joint screw (SIJS) fixation is widely regarded as the gold standard for treating unstable posterior pelvic ring injuries. However, the technique presents notable challenges, including a high risk of neurovascular injury and a steep learning curve. This study aimed to introduce a novel Posterior Internal Fixation (P-INFIX) system and evaluate its biomechanical performance in the treatment of Denis zone I sacral fractures using finite element analysis.
METHODS: A three-dimensional pelvic model-cortical bone, cancellous bone, and cartilage-was reconstructed from computed t

Diabetes mellitus severely impair fracture healing due to a pathological microenvironment characterized by persistent hyperglycemia, excessive reactive oxygen species (ROS), tissue hypoxia, and chronic inflammation, which together suppress angiogenesis and osteogenesis. Most current biomaterial strategies target only a single pathological factor and therefore fail to disrupt the interconnected metabolic cascade that perpetuates tissue dysfunction. Here, we developed a pathology-driven catalytic nanozyme-integrated adhesive hydrogel (Gel-FQG) to simultaneously regulate hyperglycemia, oxidative

BACKGROUND AND OBJECTIVES: The Dietary Oxidative Balance Score (DOBS) and Dietary Inflammatory Index (DII) reflect dietary oxidative and inflammatory status. This study investigated their associations with the prevalence of wrist, hip, and spine fractures.
METHODS AND STUDY DESIGN: DOBS and DII were scored based on 16 and 28 dietary factors, respectively. Multivariate logistic regression models and restricted cubic spline (RCS) models were used to assess the associations of DOBS and DII with the prevalence of wrist, hip, and spine fractures. Temporal trends of DOBS and DII and their associati

The term "stress fracture" encompasses a spectrum of bone injuries resulting from repetitive submaximal loading that exceeds the bone's capacity for repair. This review explores the pathophysiology and clinical classifications of stress-related skeletal injuries, distinguishing between fatigue fractures-occurring when abnormal stress is applied to healthy bone-and insufficiency fractures, which arise from normal physiological stress on weakened bone. I discuss specific manifestations, including spondylolysis, distal clavicular osteolysis, atypical femur fractures, and pregnancy-associated vert

Osteolytic vertebrae containing central lesions are associated with a high risk of burst fracture, as the presence of such lesions indicates substantial bone destruction. The present study aimed to investigate fracture patterns in a clinically relevant case of a lumbar vertebra with an osteolytic cavity under various loading conditions using a computational modeling approach. Osteolytic damage progression was simulated through a virtual thermal flux approach, while stiffness degradation was represented using a continuum damage mechanics framework. Potential crack locations within the vertebra

Vertebral bone metastases are frequent in oncological patients and lead to a significant risk of vertebral fractures. Computational modelling offers a valuable tool for investigating how metastatic lesions affect the mechanical stability and integrity of the spine. This study presents the development of a Python-based algorithm for a 3D subject-specific parametric finite element model (SS-parFEM) designed to semi-automatically generate simplified spine segments that reproduce deformation patterns similar to those observed experimentally. The model represents a metastatic vertebra including the

Intervertebral disc degeneration (IVDD) is a significant contributor to low back pain and is associated with oxidative stress-induced pyroptosis of nucleus pulposus (NP) cell. Citrulline, a non-essential amino acid integral to the urea cycle and nitric oxide synthesis, plays an essential role in maintaining intracellular redox balance. This study aimed to investigate the protective effects of citrulline on oxidative stress-damaged human NP cells and its underlying molecular mechanism related to cellular pyroptosis. Primary human NP cells were isolated from non-degenerated intervertebral disc t

BACKGROUND: Scapular stress fracture (SSF) is a recognized complication after reverse total shoulder arthroplasty (rTSA), yet its clinical impact remains controversial. This study aimed to determine the incidence of SSF after rTSA; to compare outcomes with a 1:3 propensity score‑matched control group; and to evaluate the influence of fracture location, treatment modality, and radiographic union on clinical outcomes.
METHODS: A total of 551 consecutive primary rTSAs performed between January 2010 and December 2024 at a single tertiary referral center were retrospectively reviewed. Thirty patie

BACKGROUND: For children and adolescents active in organized sports, a relationship between spondylolisthesis and low back pain (LBP) has been postulated. Reported prevalence rates vary widely due to inconsistent definitions, populations, and diagnostic methods. Reliable data are needed to inform preventive strategies and clinical decisions, particularly for adolescents engaged in sports.
PURPOSE: To conduct a systematic review on the prevalence and incidence of radiologically confirmed spondylolisthesis in children and adolescents, considering age, organized sports participation, LBP, and th

Approximately 40% of cancer patients develop spinal metastases, often leading to vertebral fragility fractures. Osteolytic metastases are the most severe, as they significantly alter strain distribution in bone tissue. This pilot study extends a previously validated experimental-computational framework for intact vertebrae to assess the accuracy of personalized finite element models (FEM) in the presence of controlled artificial lytic defects. Three human lumbar vertebrae (L3-L5) were tested in intact and defect conditions. Progressively enlarging hemispherical defects (10-25 mm diameter) were

Postmenopausal osteoporosis (PMO) significantly increases the risk of fragility fractures. The profound estrogen deficiency, compounded by severe oxidative stress, ferroptosis, and an impaired osteoimmune microenvironment, disrupts the vital coupling of angiogenesis and osteogenesis. This disruption frequently leads to delayed fracture union and implant failure. This narrative review aims to consolidate the current literature regarding both conventional pharmacological interventions and specific alternative therapeutic strategies, providing a comprehensive conceptual framework for optimizing b

BACKGROUND: Acromial and scapular spine stress fractures are serious complications following reverse total shoulder arthroplasty (rTSA), and patient-level risk factors may contribute to their development. The Geriatric Nutritional Risk Index (GNRI) is a validated marker of physiologic reserve. The purpose of this study was to evaluate the association between GNRI and post-operative stress fracture after rTSA. We hypothesized that lower GNRI would be associated with increased fracture risk.
METHODS: We performed a retrospective 1:2 age- and sex-matched case-control study of patients undergoing

INTRODUCTION: Overuse-related stress injuries of the thoracic spine are uncommon among adolescent athletes, and apophyseal stress fractures in this region are particularly rare.
CASE PRESENTATION: We report a rare case of an apophyseal stress fracture of the first thoracic (T1) spinous process in a 13-year-old elite baseball pitcher. The patient presented with focal upper thoracic pain without any history of trauma. Magnetic resonance imaging (MRI) revealed an open apophyseal physis with cortical irregularity and bone marrow edema at the T1 spinous process, consistent with an apophyseal stres
