Association of frailty indices with post-operative outcomes in shoulder surgery: a systematic review.

HYPOTHESIS AND/OR BACKGROUND: Frailty is a multidimensional state of diminished physiologic reserve that increases vulnerability to surgical stress and adverse post-operative outcomes. Although frailty indices have been associated with adverse outcomes across a variety of medical and surgical specialties, their role in shoulder surgery remains unclear. METHODS: This systematic review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. PubMed, Embase, Scopus, Web of Science, and the Cochrane Library were searched for studies evaluating frailty in pa

Epidemiology, Work-Up, and Management of Common Track and Field Injuries.

PURPOSE OF REVIEW : Track and field is one of the most widely played sports worldwide, encompassing a variety of events across different age groups. Injury is extremely common, with data suggesting that up to 2/3 of track and field athletes will sustain an injury in any given season. In this review, we provide an overview of common track and field injuries, including their incidence, workup, management considerations, and return-to-play protocols. RECENT FINDINGS : Track and field athletes experience a wide range of injuries, depending on the event in which they participate, as well as predisp

Finite element analysis of sacral fixation strategies for fragility fractures of the pelvis.

This study aimed to evaluate the biomechanical performance of different sacroiliac screw fixation strategies for posterior pelvic ring injuries in older adults with fragility fractures of the pelvis. A finite element model was created using the pelvis of an older woman with combined anterior and posterior ring injuries, simulating a unilateral pubic rami fracture and a Denis zone I sacral fracture. A subcutaneous internal fixator (INFIX) system was used to support the anterior pelvic ring. Percutaneous sacroiliac screws of different lengths and fixation levels were used to create six posterior

Pelvic insufficiency fractures in cervical cancer patients treated with definitive chemoradiation: impact of dosimetric and clinical factors.

BACKGROUND: The aim of this study was to investigate the correlation between clinical and dosimetric factors and the incidence of pelvic insufficiency fractures (PIFs) in patients with cervical cancer treated with radical radiotherapy (RT), with or without chemotherapy, and to identify dose constraints that may reduce this risk. METHODOLOGY: We reviewed data from cervical cancer patients treated with pelvic external-beam RT at our institution between 2020 and 2023. For each patient, pelvic bone marrow (BM) was contoured and divided into three subsites: lumbosacral spine (LSBM), ilium (IBM), a

Risk factors for acromion and scapular spine fractures after reverse total shoulder arthroplasty: a time-to-event analysis.

BACKGROUND: Acromion and scapular spine fractures are recognized complications following reverse total shoulder arthroplasty (rTSA). While prior studies have identified select risk factors, formal time-to-event analyses evaluating fracture hazard remain limited. METHODS: This retrospective database study utilized the TriNetX US Collaborative Network. Data were queried in February 2026 and included data collected from February 2006 to February 2026. Any patient undergoing rTSA with at least 1 year of longitudinal health record data were included in the analysis. Kaplan-Meier survival analysis

C6 Pedicle Fatigue Fracture Nonunion Causing Unilateral Cervical Radiculopathy Through Hypertrophic Foraminal Stenosis: A Case Report.

CASE: A 56-year-old woman presented with progressive weakness of the left shoulder abduction and elbow flexion. She had cervical dystonia. Imaging revealed bilateral chronic C6 pedicle fatigue fracture nonunion, with left hypertrophic changes causing C5 to 6 foraminal stenosis and C5 to C6 radiculopathy. Posterior C4 to C7 fixation with left foraminotomies was performed. Motor function improved, and follow-up computed tomography (CT) confirmed healing of the nonunion. CONCLUSION: Cervical pedicle fatigue fracture nonunion is a rare but underrecognized cause of radiculopathy when hypertrophic

Computationally efficient modeling of cervical spinal cord injury without vertebral fracture.

Finite element (FE) models are used to study spinal cord biomechanics and injury mechanisms. However, most existing models incorporate full vertebral geometry, substantially increasing computational cost and limiting the ability to perform large-scale parametric studies or apply modeling in clinically feasible timelines. In central cord syndrome, movement of the vertebrae is important while vertebral deformation contributes minimally to cord biomechanics. This study tested whether modeling the spinal canal geometry alone (versus full vertebral geometry) is sufficient to capture spinal cord str

Spontaneous L4/L5 Dislocation Associated With Diffuse Idiopathic Skeletal Hyperostosis in a Young Patient With Severe Obesity: A Case Report.

Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by extensive ossification of the spinal ligaments and entheses, resulting in a rigid spine that behaves biomechanically like a long bone. Although spinal fractures and dislocations after minor trauma are well recognized in ankylosed spines, non-traumatic spontaneous instability is rare. We report a rare case of spontaneous L4/L5 dislocation in a young patient with severe obesity and DISH. A 40-year-old man presented with progressive low back pain, bilateral lower extremity weakness, and bladder-bowel dysfunction. He had initially

A rare and complex case report of superimposed clostridial spondylodiscitis and epidural abscess associated with spinal lymphoma of the thoracic spine.

BACKGROUND: Progressive back pain in elderly individuals may be caused by spine infections, metastatic cancers, or hematologic malignancies. Spine infections, such as spondylodiscitis, often result from hematogenous spread by staphylococci, streptococci, or gram-negative bacilli, whereas Clostridium species is a rare cause of spondylodiscitis. Lymphoma involving the spine can mimic infectious spondylodiscitis and lead to irreversible neurological damage. We report an elderly Thai woman with clostridial spondylodiscitis and lymphoma coexisting at the thoracic spine, highlighting the importance

Clinical and 18F-FDG PET/CT Imaging Characteristics of Post-radiotherapy Sacral Insufficiency Fractures in Cervical Cancer Patients.

INTRODUCTION: Sacral Insufficiency Fractures (SIFs) are a common yet frequently misdiagnosed late complication following pelvic radiotherapy for cervical cancer. Accurate differentiation from bone metastases is crucial to avoid unnecessary interventions. While MRI is sensitive for early marrow edema, integrated 18F-FDG PET/CT offers a unique simultaneous assessment of bone metabolism and systemic tumor status. However, comprehensive studies detailing the qualitative and quantitative PET/CT characteristics of post-radiotherapy SIFs are lacking. This study aims to systematically define these fea

Hybrid III lumbar spinal column injury risk curves from vertical impact.

Musculoskeletal injuries from underbody blasts due to the detonation of improvised explosive devices (IED) impart dynamic loading to the human vertebral column via the pelvis region, and the primary insult from the IED is absorbed first by the lumbar spine before it traverses to the thoracic and cervical spines. Injuries from these scenarios have been documented in many clinical and retrospective studies. Although the dynamic response index continues to be used to predict lumbar spine injuries, it is based on the accelerations of the seat pan. A need exists to develop lumbar spine injury crite

The Importance of Instrumentation Length in Ankylosing Spinal Disorders and Thoracolumbar Fractures.

Background/Objectives: Ankylosing Spinal Disorders (ASDs) encompass a heterogeneous group of rheumatic diseases characterized by progressive ankylosis of the axial skeleton, including Ankylosing Spondylitis (AS), Diffuse Idiopathic Skeletal Hyperostosis (DISH), and Non-Radiographic Axial Spondyloarthritis (nr-AxSpA). Spinal ankylosis profoundly alters the biomechanical properties of the vertebral column, transforming it into a rigid long-bone equivalent and dramatically increasing fracture risk even after low-energy trauma. Once a fracture occurs, the long lever arm created by the ankylosed se

Subcutaneous gluteal migration of hardware after spine fusion: A case report.

BACKGROUND: Rod fracture is a well-recognized late mechanical complication following long-segment posterior instrumentation in adult spinal deformity. However, rod fracture with distant migration is rare. Here, a 52-year-old female presented 15 years following a T10-sacrum fusion with a subcutaneous gluteal mass representing migration of a fractured rod fragment. CASE DESCRIPTION: A 52-year-old female had a posterior instrumented fusion from T10-sacrum performed 15 years earlier for thoracolumbar scoliosis. She presented with progressive pain and a palpable mass in the left gluteal region (fi

Age-Related Transitions in Low Back Pain Pathology: A Life-Course Narrative Review.

INTRODUCTION: Low back pain (LBP) represents the primary global cause of years lived with disability, significantly compromising health-related quality of life and imposing a substantial economic burden on healthcare systems. This narrative review conceptualizes LBP as a dynamic life-course process rather than a static diagnostic entity. METHOD: A comprehensive search of PubMed, Scopus, and Web of Science was conducted using MeSH and free-text terms (e.g., "Low Back Pain," "Aging," spine diseases) for clinical trials, cohorts, and reviews published from inception through to January 2026. Pott's disease as the initial presentation of tuberculosis: diagnostic challenges and radiological findings in district-level care-a case report.

BACKGROUND: Spinal tuberculosis (Pott's disease) is a rare form of extra-pulmonary tuberculosis, often presenting with nonspecific symptoms and posing diagnostic challenges, especially in resource-limited settings. While microbiological confirmation is the gold standard, it is often unavailable in district hospitals. CASE PRESENTATION: I report the case of a previously healthy 28-year-old Black female individual who presented with a 3-week history of progressive lower back pain, fatigue, and functional impairment but without pulmonary symptoms or known tuberculosis contacts. Modified spinopelvic fixation with a novel three-dimensional-printed element following total sacrectomy for patients with sacral tumours.

OBJECTIVE: Total sacrectomy is now a standard curative procedure for primary malignant sacral tumours. Reconstruction, however, remains demanding because of the complex regional anatomy and unique biomechanical environment. Several spinopelvic reconstruction techniques have been reported, all yielding satisfactory functional outcomes. This study aims to evaluate both the clinical outcomes and the biomechanical behavior of a modified spinopelvic fixation construct, which incorporates a novel three-dimensional-printed component following total sacrectomy. METHODS: We conducted a retrospective c

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities.
