This report discusses the first known reported case of bilateral pedicle stress fractures. The importance of early detection and appropriate therapy to promote early healing is emphasized. Bone scintigraphy with single photon emission CT is favored for initial evaluation with CT recommended to further evaluate areas of focal abnormal scintigraphic activity.

Bone mineral density (BMD) was studied in young exercising amenorrheic girls to determine if density was compromised and the change related to injury. Ninety-eight volunteers (professional ballet dancers and controls) were studied in a cross-sectional study. Ibuprofen is a widely used cyclo-oxygenase inhibitor in clinical practice. It has been demonstrated by others to have an inhibitory effect on fracture repair in animals. Stress fractures in athletes usually heal well with merely rest and conservative treatment. It has been estimated that in Finland 400-500 athletic stress fractures occur annually per year. Vertebral insufficiency fractures may result from excessive loading of normal and routine loading of osteoporotic spines. Fracture occurs when the mechanical load exceeds the vertebral compressive strength, i.e., the maximum load a vertebra can tolerate. Isthmic spondylolysis is a fatigue fracture which mostly affects athletes. It is acquired between the ages of 5 and 15 years and is directly related to the type of sport practised. Ethnic and racial factors are also involved. It is of interest to identify the first symptoms of isthmic damage at the initial stage. The authors reviewed 1 year of experience with planar and single photon emission computed tomographic (SPECT) bone scintigraphy in 162 young patients with symptoms of low-back pain possibly related to stress injury to the pars interarticularis or spondylolysis. Planar scintigraphy and SPECT revealed no abnormality in 91 patients (56%). All abnormalities detected on planar images were also detectable with SPECT. Injuries to the cervical spine at the C3-C4 level involving the bony elements, intervertebral disks, and ligamentous structures are rare. We present 25 cases of traumatic C3-C4 injuries sustained by young athletes and documented by the National Football Head and Neck Injury Registry.
