Magnetic resonance appearance of sacral insufficiency fractures.

Insufficiency fractures of the sacrum are a commonly recognized form of stress fracture typically occurring in elderly patients. As such patients usually present with low back pain, MR imaging is often performed initially as a means of evaluation. We present 5 patients with sacral insufficiency fractures imaged with MR. Metastatic disease was a leading clinical suspicion as all patients were elderly and three had known primary neoplasms. T1-weighted sequences demonstrated bands of decreased signal intensity, usually paralleling the sacral aspect of the sacroiliac joints and occasionally occurr

Treatment of the vertebral crush fracture syndrome with enteric-coated sodium fluoride tablets and calcium supplements.

A cohort of 101 patients were treated with enteric-coated sodium fluoride tablets and calcium supplements. Vitamin D was also given in supra-physiologic doses in 70% of the cases. Lumbar bone mineral density (BMD), as measured by dual-photon absorptiometry, increased in a linear fashion up to four years, irrespective of the value of initial BMD and of the underlying condition, be it involutional osteoporosis (the vast majority), glucocorticoid osteoporosis, or even osteogenesis imperfecta. Estrogen replacement therapy (ERT) seemed to promote the fluoride-induced increase in lumbar BMD, as did

Treatment of vertebral osteoporosis with disodium monofluorophosphate: comparison with sodium fluoride.

Eighty one women with vertebral osteoporosis were treated for up to 2 years with fluoride administered either as monofluorophosphate (MFP, 200 mg/day, i.e., 26.4 mg fluoride-ion) or sodium fluoride (NaF, 50 mg/day, i.e., 22.6 mg fluoride-ion). All patients received calcium supplementation (1 g of Ca2+/day) taken apart from NaF and in the same tablet for MFP. Despite almost similar fluoride dosage of both regimens, the early increase in the bone mineral density (BMD) of the lumbar spine was higher with MFP than with NaF, reaching 11% and 4%, respectively, at 1 year (p = 0.007), and 21% and 6%,

Risk factors for the development of stress fractures during fluoride therapy for osteoporosis.

We attempted to identify risk factors for the development of lower limb stress fractures during fluoride therapy for osteoporosis (OP). We compared 18 patients who developed 41 such fractures (26 periarticular, 6 femoral neck, 5 long bone shaft, 1 greater trochanter and 3 pubic rami fractures) during fluoride therapy, with 24 similarly treated patients who did not develop stress fractures. Treatment consisted of sodium fluoride 0.99 mg/kg per day, elemental calcium 1 g/day, and vitamin D. We obtained a previous fracture history, annual radiographs of the spine (fractures), hands (metacarpal co

The causes of bone scintigram hot spots in fluoride-treated osteoporotic patients.

We previously described new bone formation in fluoride-treated osteoporotic patients. Since then, several investigators have contended that fluoride-induced "hot spots," as seen on bone scintigrams, represent stress fractures. To further evaluate this issue we analyzed scintigrams, radiographs, and quantitative computer tomography (CT) scans of the spine and femoral condyles from 129 patients, obtained before and during therapy. Hot spots, new or of increased intensity, were more than twice as common in the weight-bearing peripheral skeleton than in nonweight-bearing sites (p less than 0.001).

Operative treatment of spine fractures in children.

Spinal column fractures in children are unusual. This article presents concise methods of evaluation and management of these injuries with regard to their alignment, stability, and possible spinal canal compromise. The role of radiographic imaging is defined and illustrated by example. The author recommends the techniques outlined in this chapter for stabilization of those spinal column injuries in children when surgery is thought to be indicated.

Nutrition and the incidence of stress fractures in ballet dancers.

The effects of nutrition on the incidence of stress fractures among classical ballet dancers were studied. Ten dancers with stress fractures were compared with a group of dancers without stress fractures and a group of nondancing control subjects. Subject pairs were matched for age, weight, and height. Specific nutrient intake and eating patterns were thus isolated to determine if dietary patterns could account for the incidence of stress fractures among these dancers. The majority (80%) of the 10 dancers with recent stress fractures had weights less than 75% of ideal (p less than 0.05) and sh

Effect of exercise on bone: permissive influence of estrogen and calcium.

Estrogen deficiency in postmenopausal women is associated with low lumbar bone mineral density and an increased incidence of fractures of the vertebrae and proximal femur. Estrogen deficiency in premenopausal women with secondary amenorrhea related to athletic training or anorexia nervosa is also associated with decreased lumbar bone mineral density. The purpose of this review is to present four concepts related to the adaptations of bone to physical exercise, as a basis to explain the loss of bone mass in women with athletic amenorrhea. These concepts are based on Lanyon's theory of a Minimum

Bone density of élite female athletes with stress fractures.

To investigate whether stress fractures occurring in élite female athletes are related to reductions in bone mineral density (BMD), we measured BMD in nine athletes with such fractures and nine athletes without fractures who were matched for age, weight, height and sport. BMD was measured in three regions: upper limbs (distal radius), axial skeleton (lumbar spine) and lower limbs (femoral neck) by photon absorptiometry. The number of menses per year was significantly less (P less than 0.04) and the age of menarche was significantly delayed (mean +/- SD; 16.1 +/- 0.4 v. 14.4 +/- 1.5 years, P le

[Current status of surgical treatment of the unstable spine with special reference to neurosurgical indications].

The presently available surgical techniques will practically always allow stabilization of the vertebral column in case of injuries indicating the need for surgery. Patients can be subjected to early functional treatment and introduced to rehabilitation training without requiring the plaster casts that used to be major obstacles to mobilization. Excellent results can be obtained in injuries of the thoracic and lumbar vertebral column if surgery is performed early, using less invasive dorsal stabilization techniques. Dorsal intervention is also possible in polytraumatic and intensive-care patie

Dynamic analysis of the Harrington system using a spinal simulator.

The authors constructed a spinal simulator for the thoracolumbar spine with mechanical properties similar to those of cadaver specimens, and studied the effectiveness of the Harrington system in unstable fractures of the thoracolumbar spine. Distraction and compression rods were applied under various conditions. External bending moment was applied to the model to measure the internal bending moment on the vertebral body and ligaments. Optimal spinal stability was obtained when the distraction system was combined with the compression system with hooks at laminas three levels above and below the

Spinal pedicle finder for transpedicular screw fixation--design and early clinical result.

Spinal transpedicular fixation has gained widespread popularity in the past 5 years. In biomechanical studies, the deeply-inserted transpedicular screws withstood the largest number of cycles in the cephalad-caudad and medial-lateral direction before failure. However, in clinical practice, the risk of screw placement which is too far medially or too far laterally do exist. The optimization of increasing screw depth to avoid complication is of significant clinical importance. A Spinal Pedicle Finder (S.P.F) has been designed for transpedicular screws and a prototype has been completed. It is co
