Interaction of calcium nutrition and physical activity on bone mass in young women.

We have examined the relationships among calcium intake, mechanical stress (activity) and axial (lumbar) and peripheral (midradius) bone mass in a population of 60 young normal, healthy eumenorrheic women (age 25-34 years), selected on the basis of weight for height (within 20% of expected). In this population, vertebral bone mineral density was significantly related (r = 0.41, p less than 0.005) to activity pattern but not to calcium intake. Elimination of the effects of activity, however, disclosed a significant correlation between total calcium intake and vertebral bone mass (r = 0.36, p le

[Mineral content of the spongiosa of the lumbar spine determined by computer tomography. Normal values for healthy perimenopausal women and their relation to mechanical stress of the vertebral column].

Healthy perimenopausal women (152) were examined by QCT to quantify the mineral content of the axial skeleton. The density was measured in the spongiosa of the L2-4, using single energy technique and the Cann-Genant calibration phantom. The women, aged 37 to 64, had no history of pathological fractures, ovariectomy, metabolic disease, or hormone therapy. The age-related mean values were 25 mg K2HP04/cm3 lower than the normal values reported by Genant et al. for 203 healthy American women. The 95% confidence interval was distinctly narrower (+/- 40 mg/cm3) than the American confidence interval

[Trends and results of treatment of spinal fractures].

As long as simple and efficient operative techniques for stabilizing of injured segments of the vertebral column were missing naturally conservative methods dominated. The fixateur interne and the transpedicular spongiosa bone graft, however, provide operative techniques of high efficiency and technical simplicity, which make a revision of old indicational rules- operation versus conservative treatment - necessary. The example of a stable spine fracture points out that the calculated final result has to be clearly better if risk and effort of an operation should be advantageous for the patient

Acute traumatic spondylolysis. Report of a case and review of the literature.

Acute traumatic bilateral spondylolysis and spondylolisthesis was observed in a 34-year-old man. The literature on pathologic and biomechanical factors leading to acute fracture of the pars interarticularis emphasizes the importance of distinguishing spondylolysis resulting from acute trauma from the more common lytic-fatigue pars defect.

Insufficiency fractures of the sternum: a consequence of thoracic kyphosis?

Osteoporosis often leads to thoracic compression fractures and progressive thoracic kyphosis. The kyphosis creates a deforming stress on the sternum that may eventuate in an insufficiency-type stress fracture. The findings in four women with osteoporosis, thoracic kyphosis, and insufficiency fractures of the sternum are reported.

Bone scintigraphy in the assessment of spondylolysis in patients attending a sports injury clinic.

Thirty-three patients attending the Nottingham Sports Injury Clinic in whom a clinical diagnosis of a stress fracture of the pars interarticularis was made were evaluated with both radiography and bone scintigraphy. This was done in an effort to distinguish a recent stress fracture which would produce a positive scintigram from an established 'incidental' spondylolysis giving a normal scintigram. Nine patients demonstrated abnormal radiography with a normal scintigram. A further nine patients had a positive scintigram. Six were associated with a spondylolysis indicating a recent stress fractur

A biomechanical evaluation of cervical spinal stabilization methods in a bovine model. Static and cyclical loading.

A bovine model was developed for biomechanical evaluation of surgical procedures stabilizing traumatic cervical injuries disrupting the anterior and posterior spinal column. Cervical spinal segments and C4-5 functional spinal units were tested statically, and C4-5 functional spinal units were tested cyclically in evaluation of 1) the intact cervical spine, 2) Rogers' wiring method, 3) Bohlman's triple-wire technique, 4) sublaminar wiring, 5) anterior cervical plate instrumentation, and 6) posterior hook plate stabilization. Anterior cervical plate instrumentation proved inadequate, and was the

Insufficiency fractures of the pelvis that simulate metastatic disease.

Insufficiency fractures of the pelvis, which almost always occur in elderly women with osteoporosis, are often misinterpreted as metastatic disease. The initial symptom of such fractures is severe pain unassociated with an obvious history of trauma. The typical sites of involvement are the sacrum, the iliac bones, and the pubis. The plain film appearance of the sacral and iliac fractures is usually subtle and easily overlooked, and bone scans will show the abnormal areas more readily. The existence of multiple fractures not only in the pelvis but also in the vertebrae and ribs should suggest t

[Stress stable spondylodesis in injuries of the lower cervical spine].

In view of the high incidence of late instability in the conservative treatment of cervical spine fractures, operative stabilization should be performed as soon as possible. Besides immediate decompression of the spinal cord, rapid mobilisation of the patient without external fixation is possible. We report on the results of operative treatment in 97 patients with injuries of the lower cervical spine. The indication to spondylodesis as well as the operative methods and their complications are discussed.

The use of single photon emission computed tomography (SPECT) in the diagnosis of low-back pain in young patients.

Single photon emission computed tomography (SPECT) is a method of scintigraphy that provides sectional and multi-planar imaging that should improve the sensitivity for diagnosis in remote areas of the spine. To test this hypothesis, the authors reviewed 15 patients with low-back pain and compared the results of the radiographs, planar bone scans (PBS), and SPECT. Four of 15 had normal examinations with all three methods of imaging. The rest had a positive test in at least one of the three methods. Single photon emission computed tomography was positive in eleven, PBS in six, and the radiograph

Biomechanical evaluation of spinal fixation devices: I. A conceptual framework.

In the field of spinal fixation devices, there is a profusion of new instrumentations. Often, the biomechanical evaluation is done in a nonstandardized manner, which makes it difficult to compare the results of one researcher with those of another, for the same device or for different devices. There is a need for a conceptual framework under which guidelines may be suggested for the evaluation of these devices in some uniform and comprehensive manner. There are three basic biomechanical tests: strength, fatigue, and stability. The strength test evaluates the failure load of the device, determi

Sclerosis of a rib associated with a healed fracture of a contiguous thoracic vertebra.

A patient who suffered a fracture of the body of the 10th dorsal vertebra as a result of an automobile accident was examined seven years later. He complained of local pain over the gibbus site. All movements in the back and the joints of the extremities were within normal limits. Radiographs disclosed a healed compressed fracture of the body of the 10th thoracic vertebra with bony bridges contiguous with the 9th and 11th thoracic vertebrae. An unusual finding was the presence of a uniform sclerosis of a single rib at the level of the fracture site. Other bones of the rib cage, as well as the l

Neck injuries in ice hockey: a recent, unsolved problem with many contributing factors.

Previously unrecognized, major neck injuries in hockey have become a common problem in Canada and they have increased markedly in the 1980s. At the present time, the incidence remains high and has shown no sign of diminishing. The etiology of the problem appears to be multifactorial. Several epidemiologic factors have been identified and a reporting system established so that prevention programs can be monitored. It is hoped that greater awareness of the risk factors among players, coaches, leagues, referees, and parents will be an effective prophylactic measure in itself. Prevention must invo

Mechanical consequences of rod contouring and residual scoliosis in sublaminar segmental instrumentation.

The mechanical performance of contoured Luque rods in a neuromuscular model of spine deformity was examined to define an upper limit of deformity above which rod stresses would exceed the endurance limit for 316L stainless steel and therefore predict fatigue failure. Bovine constructs varying from 0-120 degrees scoliosis were loaded axially, with strain recordings obtained at the apex of the curve. Relatively low loads produced enough tensile stress to contemplate implant fatigue in all except the nondeformed (0 degrees) construct. Construct stiffness was found to decrease rapidly in spines wi

Mechanical behavior of the human lumbar spine. II. Fatigue strength during dynamic compressive loading.

Seventeen cadaveric human lumbar motion segments from eight spines were cyclically loaded in vitro under axial compression. Loading frequency and magnitude were chosen to simulate rigorous activity within an in vivo physiological level. The load magnitude was determined as a percentage of the ultimate compressive load, the latter estimated from the bone mineral content (BMC) of lumbar vertebrae determined by dual-photon absorptiometry. Following testing, the degree of macroscopic disc degeneration was assessed and the type of fracture in each specimen was determined from serial sagittal sectio
