From collapse to comeback: Luteolin rejuvenates nucleus pulposus progenitor cells via SIRT1/ATF5-UPRmt to reverse intervertebral disc degeneration cascade.

BACKGROUND: Intervertebral disc degeneration (IVDD) is a major contributor to low back pain (LBP) and one of the foremost causes of disability worldwide. Oxidative stress-induced senescence of nucleus pulposus progenitor cells (NPPC) and mitochondrial dysfunction are key drivers of IVDD. The mitochondrial unfolded protein response (UPRmt), orchestrated by the Silent Information Regulator 1 (SIRT1)-Activating Transcription Factor 5 (ATF5) axis, plays a pivotal role in maintaining mitochondrial proteostasis. However, its involvement in IVDD remains insufficiently characterized. Luteolin (Lut), a

Cervical Spine Injuries and Head-Supported Mass: A Descriptive Epidemiology Study in Special Operations Combat Soldiers.

INTRODUCTION: Special operations combat soldiers (SOCS) are susceptible to cervical spine (CS) injuries as they undergo tactical training and perform missions in demanding environments with head-supported mass (HSM). This HSM, which includes the baseline protective helmet, communications, specialized night vision technology, and other attachments, can weigh more than 3 kg. The purpose of this study was to describe the injury epidemiology of CS injuries in SOCS including the effect that HSM may have on both CS injuries and pain. MATERIALS AND METHODS: The study population included 284 SOCS and

TDP43 cytoplasmic mislocalization initiates mitochondrial dysfunction and intercellular senescence propagation in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD), a leading cause of low back pain, involves progressive dysfunction of nucleus pulposus (NP) cells and extracellular matrix degradation. The pathological mechanisms underlying IDD remain complex and lack comprehensive elucidation. This study identifies the RNA-binding protein TDP43 as a central driver of IDD pathogenesis through analysis of human clinical specimens and rodent models. We demonstrate that TDP43 expression escalates proportionally with disc degeneration severity and aberrantly accumulates in the mitochondria of degenerative NP cells. This mi

Drug-Induced Cuproptosis Defines the Therapeutic Window of Celecoxib in Intervertebral Disc Degeneration via the HSP90-RBX1 Axis.

Intervertebral disc degeneration (IDD) is a major cause of low back pain, yet the biological effects of commonly used non-steroidal anti-inflammatory drugs (NSAIDs) on disc cells remain poorly understood. Celecoxib is widely prescribed for IDD-related pain, but its direct influence on IDD has not been systematically examined. Here, we identify a concentration-dependent biphasic effect of celecoxib on nucleus pulposus (NP) cells and uncover the mechanism that converts celecoxib from protective to detrimental. Using interleukin-1β-stimulated NP cells and rat IDD models, we show that low-dose cel

From Pixels to Precision: Generative Artificial Intelligence as a Paradigm Shift in Spine Imaging-Technical Foundations, Clinical Applications, and the Path to Safe Clinical Deployment.

Spine imaging represents a complex diagnostic frontier characterized by anatomical variability, motion artifacts, metallic instrumentation interference, and significant inter-reader diagnostic variability (κ=0.20 across institutions). While conventional discriminative artificial intelligence (AI) models achieve >95% accuracy in detecting degenerative changes, they remain limited by data scarcity, heterogeneous protocols, and poor generalizability. In the spine, these limitations are particularly relevant because clinical decisions can often depend on subtle distinctions (such as differentiatin

Contemporary Management of Coccydynia.

Coccydynia is a painful condition of the coccyx that is frequently misdiagnosed and managed inconsistently. This review summarizes and grades the current evidence on diagnostic strategies and treatment options. We systematically searched the literature and included 42 studies covering conservative, interventional, and surgical management. Based on these data, we propose a current best framework for diagnostic evaluation and therapeutic management. Initial assessment should include detailed history and focused examination with palpation for localized coccygeal tenderness and symptom provocation

Spinal cord injury induces acute microbiome shock and system-wide transcriptomic reprogramming.

This study investigates the systemic consequences of spinal cord injury (SCI), with a particular focus on alterations in the gut microbiome and multi-organ transcriptomic responses. We identify a rapid and severe disruption of the gut microbiota-termed "microbiome shock"-that emerges within 12 h post-SCI and persists before gradually resolving by 5 days post-injury. To support further research in this field, we established an open-access resource, the Spinal Cord Injury Gut Microbiome and Multi-Organ Gene Expression Atlas (SCIGAMA).

Age-related differences in surgical outcomes for traumatic central cord syndrome: a multi-institutional causal machine learning analysis.

OBJECTIVE: Traumatic central cord syndrome (TCCS) is the most common incomplete spinal cord injury, yet the optimal management strategy remains controversial, particularly for older adults who often present with worse outcomes. The authors aimed to determine whether surgical intervention confers different benefits across age groups, focusing on 1-year functional and neurological recovery. METHODS: The authors retrospectively analyzed 890 patients with TCCS from the multi-institutional Spinal Cord Injury Model Systems (SCIMS) Database between 2006 and 2021. TCCS was defined as an American Spin

Disruption of hippocampal synaptic plasticity by chronic ethanol exposure: A narrative review of neurotoxic adverse outcome pathways.

Chronic is a major neurotoxicant that disrupts hippocampal synaptic plasticity, leading to persistent cognitive deficits. This narrative review maps the adverse outcome pathways (AOPs) through which ethanol impairs synaptic function, primarily via interconnected cascades: TLR4/NF-κB-mediated neuroinflammation (triggering microglial activation and pro-inflammatory cytokines TNF-α, IL-1β), CYP2E1-driven oxidative stress (generating ROS/RNS, 4-HNE, causing protein carbonylation and mitochondrial dysfunction), and glutamate excitotoxicity (mediated by GluN2B-NMDAR subunit shifts, Ca²⁺ overload, an

Tazarotene ameliorates steroid-induced osteonecrosis of the femoral head by activating autophagy.

BACKGROUND: Steroid-induced osteonecrosis of the femoral head (SIONFH) is a debilitating condition characterized by lipid accumulation and bone structural collapse. A key pathogenic mechanism involves downregulated osteogenesis and upregulated adipogenesis of bone marrow mesenchymal stem cells (BMSCs). Autophagy has been implicated in lipid metabolism and bone homeostasis. This study investigated whether activating autophagy could ameliorate SIONFH. METHODS: Lipid droplet based high-throughput screening of an active compound library was performed on dexamethasone (DEX)-treated rat BMSCs. Furt

Soft substrate priming erases fibrotic mechanical memory in mesenchymal stromal cells via YAP lysosomal degradation to improve therapeutic efficacy for spinal cord injury.

Mesenchymal stromal cells (MSCs) hold great promise for spinal cord injury (SCI) repair owing to their potent immunomodulatory and neuroprotective properties, yet their therapeutic efficacy collapses in the chronic phase. We identify a critical, overlooked barrier: the pathological stiffening of the fibrotic scar microenvironment hijacks the mechanotransduction machinery of transplanted MSCs, driving them toward a maladaptive fibroblast-like phenotype that compromises their immunomodulatory and regenerative functions, ultimately blunting their overall therapeutic potential. Here, we reveal tha

Abl kinase activation promotes axon initial segment disassembly and protein sorting defects in Alzheimer's disease.

BACKGROUND: Axon initial segment (AIS) dysfunction disrupts neuronal compartmentalization, which leads to pathological processes like Tau missorting in Alzheimer's disease (AD). However, the molecular mechanisms that destabilize the AIS scaffold are incompletely understood. Our group has previously shown that the Abl1 non-receptor tyrosine kinase is aberrantly activated in AD mouse models and promotes dendritic spine collapse, Tau hyperphosphorylation, and neuronal apoptosis. Given the important role of Abl1 in AD and its emerging significance in Tau pathology, we examined how it contributes t

Fixed-Angle Static Versus Variable-Angle Dynamic Plates for Multilevel Anterior Cervical Discectomy and Fusion.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: This study compared multilevel ACDF using fixed-angle static versus dynamic plates. We hypothesized that static plates would better maintain lordosis and fusion construct height, with similar functional outcomes and nonunion rates, compared with dynamic plates. SUMMARY OF BACKGROUND DATA: Dynamic plates allow for translational collapse, theoretically improving fusion rates; conversely, fixed-angle static plates limit toggle at the screw-plate interface and may better maintain lordosis and construct height. Existing research comparing tradi

Total joint replacement of the lumbar spine: surgical technique and procedural details.

Surgical arthrodesis of the degenerative lumbar spine eliminates natural mobility across the motion segment and permanently alters the stress distribution resulting in accelerated degeneration at adjacent levels. This article and accompanying video describe the surgical details of an alternative motion preservation procedure: total joint replacement (TJR) of the lumbar spine. This procedure is indicated for patients who exhibit radiographic evidence of severe degeneration of the three-joint complex (intervertebral disc and facet joints). The TJR procedure involves a bilateral transforaminal lu

Risk factors and management of early pain non-response after vertebroplasty in osteoporotic vertebral compression fractures.

BACKGROUND: Percutaneous vertebroplasty is widely used to treat painful osteoporotic vertebral compression fractures (OVCFs), providing rapid pain relief in most patients. However, a subset of patients experiences early pain non-response, and the predictors and optimal management of this condition remain incompletely understood. This study aimed to identify risk factors associated with early pain non-response after vertebroplasty and to describe a practical management approach. METHODS: We retrospectively reviewed patients with OVCFs who underwent vertebroplasty at a tertiary referral center

[Forensic assessment of injuries to the subaxial part of the cervical spine column].

An analysis of the treatment outcomes of 65 patients with non-fatal subaxial cervical spine (CS) injuries was conducted. Injuries were classified according to the four-column biomechanical classification model for injuries to these structures (STSG, 2006). It was established that the most frequently injured structures are the anterior column of the C4-C7 vertebrae, where complicated compression, burst fractures, fracture-dislocations, and subluxations are most often observed. Uncomplicated fractures of the transverse processes, arches, and articular processes are commonly observed in the later
