Short-same segment fixation utilizing differential screw application with sequential reduction for thoracolumbar burst fractures without neurological deficit via Wiltse approach: a technical strategy to enhance reduction and radiological outcomes.

BACKGROUND AND OBJECTIVE: Posterior distraction reduction (DR) technique with polyaxial screw fixation may fail to achieve sufficient central endplate reduction, potentially resulting in residual cavities, postoperative vertebral collapse and hardware failure. The biomechanical benefits of mixed screw types remain insufficiently explored. This study aims to evaluate clinical and radiographic outcomes of a novel transpedicular prying reduction (TPR) technique after axial distraction performed via the Wiltse approach using a sequential monoaxial-monoaxial-polyaxial screw configuration in patient

From 3 to 8 clusters: a machine learning-based classification of operated adult spinal deformity patients.

BACKGROUND CONTEXT: Adult spinal deformity (ASD) is a heterogeneous condition encompassing diverse etiologies, clinical presentations, and surgical challenges. While previous unsupervised learning models have stratified ASD patients into 3 broad phenotypes, these often lack clinical granularity for surgical decision-making. PURPOSE: To refine the classification of operated ASD patients using unsupervised machine learning and assess whether expanding from 3 to 8 clusters improves clinical discrimination and predictive robustness. STUDY DESIGN: Retrospective multicenter study using machine lea

Radiological evaluation and complications of percutaneous vertebroplasty and fenestrated pedicle screw fixation with bone cement augmentation: propensity-score matched cohort study.

BACKGROUND: Osteoporotic vertebral fractures affect many elderly patients, and complications after common treatments such as vertebroplasty (VP) may reduce treatment satisfaction and radiographic stability. This study investigated whether the addition of posterior instrumentation with cement-augmented fenestrated pedicle screws (VP + PI) was associated with improved short-term radiographic outcomes compared with VP alone. METHODS: We retrospectively analyzed 153 patients with osteoporotic vertebral fractures treated between 2017 and 2022. Patients underwent either VP alone or VP + PI. To redu

Acute Disc Herniation at the Proximal Junctional Segment (UIV/UIV + 1) Following Long Segment Spinal Fusion With Cement-Augmented Instrumentation: A Retrospective Study.

BACKGROUND: Prophylactic cement augmentation at the uppermost instrumented vertebra (UIV) and the adjacent supravertebra (UIV + 1) is commonly used during long-segment fusion for adult spinal deformity to reduce the risk of proximal junctional failure. While this strategy may protect against osteoporotic fracture, its impact on adjacent soft tissues-particularly the intervertebral disc-remains poorly characterized. Acute disc herniation (ADH) at the proximal junction represents a rare but potentially catastrophic complication. METHODS: In this retrospective cohort study, 173 patients underwen

Effects of Derotational Breathing Exercises on Thoracic Cage Asymmetry in Adolescents with Thoracic Scoliosis.

Thoracic scoliosis is commonly associated with rib cage deformity and impaired respiratory mechanics. Derotational breathing exercises aim to correct three-dimensional trunk asymmetry by selectively expanding the concave hemithorax. This prospective quasi-experimental study investigated the effects of a 4-week supervised Schroth-based rotational angular breathing (RAB) program on intercostal distance (ICD), vertebral axial rotation, and Cobb angle in 20 female adolescents (age 14.30 ± 1.84 years; mean follow-up 33.45 ± 4.26 days) with thoracic idiopathic scoliosis. Radiographic measurements we

Case report of an occipital neuralgia caused by a vertebral artery loop: surgical strategies from a rare neurovascular conflict.

BACKGROUND: Occipital neuralgia secondary to cervical nerve root compression is most commonly associated with degenerative or post-traumatic foraminal stenosis. In contrast, neurovascular conflict caused by an aberrant vertebral artery loop represents a rare and underrecognized etiology. Optimal surgical management remains variable, with multiple surgical techniques with risks and level of invasiveness. CASE DESCRIPTION: A 74-year-old woman presented with a five-year history of severe, progressively worsening left-sided occipital neuralgia refractory to conservative treatment. The pain was ch

Interaction between FKBP10 and HSP10 activates the TGF-β/Smad pathway to promote the maintenance of osteosarcoma stemness.

Osteosarcoma is a primary malignant bone tumor with high incidence in adolescents. Its characteristics of high recurrence, high metastasis, and cancer stem cell (CSC)-mediated treatment resistance are major obstacles to improving patient prognosis. This study aimed to systematically investigate the role and molecular mechanism of FK506-binding protein 10 (FKBP10) in regulating osteosarcoma stemness and malignant progression. Clinical sample analysis showed that High expression of FKBP10 is negatively correlated with prognosis in osteosarcoma, and positively correlated with the expression of st

Biomechanical comparison of posterior short-segment fixation with or without intermediate screws for thoracolumbar burst fractures under normal and osteoporotic conditions: a finite element analysis.

STUDY DESIGN: Finite element analysis. PURPOSE: To investigate the biomechanical response of posterior short-segment fixation with or without intermediate screws at the index vertebra in osteoporotic thoracolumbar burst fractures using finite element analysis. OVERVIEW OF LITERATURE: Spinal fixation in elderly patients with osteoporotic vertebral fractures is challenging because osteoporosis weakens the screw-bone interface, leading to screw loosening and loss of fracture reduction. Short segment fixation with intermediate screws has been proposed to reduce kyphosis recurrence and implant fa

Minimally Invasive Management of Collapsed of-4 Osteoporotic Vertebral Fractures: Preliminary Evidence for Vertebroplasty Combined with Pediculoplasty.

BACKGROUND: Osteoporotic vertebral fractures (OVFs) with complete vertebral body collapse (A subtype within the OF4-type category, AO Spine-DGOU classification) represent a severe and unstable subtype of spinal injury in older adults. Owing to technical complexity and concerns over cement leakage, these fractures have traditionally been considered unsuitable for percutaneous vertebroplasty (PVP). However, open surgery carries substantial risks in elderly patients. Evidence regarding the safety and efficacy of PVP combined with pediculoplasty for the specific OF4-type OVFs remains limited. This

Is nail plate fixation the way forward for distal femur fractures with long segmental metaphyseal comminution? - A study of 56 patients.

BACKGROUND: Mechanical instability due to long metaphyseal comminution and medial bone gap after lateral locking plate results in varus collapse and implant failure. We believe that fixation in the form of Nail and plate combination provides better stability and prevents complications. MATERIALS: We performed a retrospective analysis of 56 distal femur fractures with long metaphyseal comminution treated in our institution with nail and plate construct from 2017 to 2023. The mean age was 43.1 years and the mean follow up was 24 Months (4-36 months). Of these 56 patients, 16 were closed injurie

Positional Hemodynamic Instability in a Patient With Pectus Excavatum Undergoing Lumbar Discectomy.

Pectus excavatum (PE) is the most common congenital chest wall deformity, accounting for 90% of cases, and may predispose patients to hemodynamic compromise due to cardiac compression. Such events during spinal surgery, however, are rarely reported. In the widely used genupectoral prone position for spine surgeries, the patient lies prone with the torso supported and the legs flexed, thereby reducing abdominal compression, improving venous drainage, and reducing bleeding. However, the support provided by the thorax in this position may cause significant mediastinal compression in the presence

The Elusive Concept of Stability in Osteoporotic Vertebral Fractures: A Narrative Review.

Osteoporotic vertebral fractures (OVFs) are the most common fragility fractures, representing a substantial burden on healthcare systems worldwide. Although up to 30% of OVFs may be clinically silent, a subset of patients experiences an unfavorable course, developing painful pseudoarthrosis/nonunion, progressive vertebral collapse, and even neurological compromise. While initial OVF management is typically nonoperative, a considerable proportion of patients ultimately require surgical intervention. However, clear and universally accepted surgical indications are lacking, rendering clinical dec

Establishing a standardized biomechanical model for osteoporotic pincer (OF4) Fracture creation in human cadaveric spines: a novel experimental technique.

BACKGROUND: Osteoporotic pincer-type fractures (OF4) represent a distinct and challenging fracture subtype, yet the absence of reproducible experimental models limits scientific opportunities for biomechanical research and the development of tailored treatments. This study describes and validates a standardized method to generate OF4 pincer fractures in human cadaveric spines. METHODS: Twelve osteoporotic bisegmental thoracolumbar specimens underwent controlled fracture induction using a servo-hydraulic testing machine. Radiological assessment confirmed fracture morphology consistent with OF4

Endoscopic cyst fenestration followed by mini-orbitozygomatic craniotomy via eyebrow incision: a novel approach for resection of cystic craniopharyngioma. Patient series.

BACKGROUND: Craniopharyngioma is a rare, noncancerous tumor that develops from Rathke's pouch. Gross-total resection (GTR) is essential to reduce the risk of recurrence. Once the cyst is decompressed, it can be difficult to appreciate how the cyst wall collapses intraoperatively and achieve a GTR. In this case series, the authors suggest endoscopic cyst fenestration (ECF) with outpatient imaging, followed by mini-orbitozygomatic craniotomy via eyebrow incision (EOZ) for resection of cystic craniopharyngioma. OBSERVATIONS: There were 5 patients included in this single-institution retrospective
