Endovascular management of iliac vessel injury complicating lumbar disc surgery: a case report and review of literature.

BACKGROUND: Lumbar disc surgery is one of the most commonly performed spinal procedures and is generally considered safe. However, rare complications such as major vascular injury can be life-threatening and are often difficult to recognize intraoperatively. These injuries most commonly involve the iliac vessels because of their close anatomical relationship to the L4-L5 disc space. Early diagnosis is challenging, and delayed recognition is associated with significant morbidity and mortality. With advances in endovascular techniques, minimally invasive management has emerged as an effective al

Hemothorax from a Thoracic Chalk-Stick Fracture in Ankylosing Spondylitis: A Case Report.

INTRODUCTION: Chalk-stick fractures are transverse spinal injuries seen in patients with ankylosing spondylitis due to chronic inflammation and spinal rigidity. These fractures may result from minor trauma and are associated with potentially fatal complications. While spinal fractures in ankylosing spondylitis are well recognized, thoracic chalk-stick fractures complicated by hemothorax from vascular injury remain exceedingly rare. We present a case of an elderly male with ankylosing spondylitis who sustained a thoracic chalk-stick fracture following a ground-level fall, complicated by hemotho

Synergistic protein-reinforced DNA hydrogels with tunable biomechanics for mechanoresponsive drug release.

DNA hydrogels are widely explored in biomedical research for their programmability and soft tissue-mimicking mechanics. However, their application in load-bearing implants is restricted by insufficient mechanical robustness, especially for mimicking the biomechanics of the nucleus pulposus, which is a naturally occurring functional tissue essential for spinal flexibility and shock absorption in the intervertebral disk. Here, we report a novel single-pot, two-step fabrication strategy in which long DNA strands produced by rolling circle amplification form an initial viscoelastic network that is

Cement-augmented endplate-penetrating pedicle screws for osteoporotic vertebral collapse: a technical note.

Osteoporotic vertebral collapse with a retropulsed posterior wall fragment may present with delayed neurological deficits. Although this condition is managed with short-segment posterior fixation, pedicle screw (PS) pullout is a major concern in patients with severe osteoporosis. Herein, we describe a preliminary salvage technique combining endplate-penetrating hollow PSs and cement augmentation to minimize intradiscal cement leakage. Two elderly women with osteoporotic vertebral fractures at L1/L2 with paraparesis underwent partial laminectomy at the collapsed level and short-segment posterio

Disc degeneration did not lead to an increased incidence of adjacent vertebral fractures in wedge-type OVCF patients.

BACKGROUND: Stress concentration is a primary pathogenesis of adjacent vertebral fracture (AVF) following percutaneous vertebroplasty (PVP) for osteoporotic vertebral compression fractures (OVCF). Disc degeneration (DD) is highly prevalent in the elderly, including OVCF patients. As a key biomechanical function of the intervertebral disc (IVD) is to ensure uniform load distribution, the progression of DD may impair this capacity, leading to localized stress concentrations. This study aims to investigate the impact of DD on AVF risk and elucidate its underlying biomechanical mechanisms. METHOD

Benchmarking risk prediction tools in spine surgery: a national registry analysis of albumin, frailty, and surgical calculators.

OBJECTIVE: Preoperative risk-stratification tools, including frailty, nutritional, and surgical risk metrics, are used to predict complications after spine surgery. The relative performance of these tools across complication types and surgical subgroups is not well characterized. This study aimed to compare the predictive performance of 5 risk metrics, American College of Surgeons Surgical Risk Calculator (ACS SRC), serum albumin, Risk Analysis Index (RAI), modified 5-item frailty index (mFI-5), and Geriatric Nutritional Risk Index (GNRI), for perioperative complications. METHODS: The authors

Oral Food Supplement with Bio-Activated Silicium and Vitamins D3 and K2 in the Conservative Management of Osteoporotic Vertebral Compression Fractures.

Background: Osteoporotic vertebral compression fractures (OVCFs) are the most prevalent manifestation of osteoporotic skeletal disease, associated with severe pain, functional decline, and elevated risk of subsequent fractures. Conservative management remains the first-line approach for stable fractures, yet pain control is often suboptimal, and vertebral collapse progresses in up to 37% of patients. Bio-activated orthosilicic acid combined with vitamins D3 and K2 (BioSi-DK) may support fracture healing through complementary mechanisms acting on osteoblast differentiation, collagen synthesis,

Multicentre prospective randomised controlled non-inferiority trial on the efficacy and safety of minimally invasive SAIF vertebral reconstruction technique versus spinal fixation in unstable osteoporotic vertebral compression fractures: a protocol.

INTRODUCTION: Osteoporotic vertebral fractures (OVFs) significantly impact morbidity, mortality, function and quality of life. Stable fractures with controllable pain can be managed conservatively. Those that remain painful can be treated with minimally invasive measures of vertebral augmentation (VA). In contrast, severe and unstable OVFs risk collapse, progressive kyphosis and neurological damage. Conventional VA is considered insufficient for these unstable fractures, making surgical stabilisation necessary to restore spinal load-bearing capacity, and allow fast and painless mobilisation. H

Beyond biochemical cascades: the biophysical execution of disulfidptosis via actin network collapse.

The discovery of disulfidptosis has identified a distinct form of regulated cell death in which metabolic redox failure is translated into biophysical disruption of the actin cytoskeleton. This review synthesizes current evidence on the molecular machinery of actin remodeling under disulfide stress, with particular emphasis on the Rac1-WRC-Arp2/3 signaling axis and other actin-regulatory nodes. Mechanistically, disulfidptosis can be conceptualized as a redox-to-mechanics transition. In SLC7A11-high cells, glucose deprivation limits pentose phosphate pathway-derived NADPH production and weakens

A Fatal Case of Spondylodiscitis Complicated by Aortic Fistulization and Mycotic Pseudoaneurysm Formation.

Spondylodiscitis, a pyogenic infection of the intervertebral disc and adjacent vertebral bodies, is an uncommon but serious condition that may progress to life-threatening vascular complications when the infective process erodes into adjacent great vessels. We report the case of a 60-year-old woman who presented with a three-week history of severe, progressive low back pain, high-grade fever, and chills. Laboratory workup revealed marked leukocytosis and a markedly elevated C-reactive protein level, and blood cultures yielded Staphylococcus aureus (S. aureus). MRI of the thoracolumbar spine de

MAP-Derived Shock Index for Point-of-Care Physiological Risk Stratification After CT-Confirmed Cervical Spine Fracture: Development and Internal Validation of a Prognostic Model for In-Hospital Mortality.

Background/Objectives: Early mortality risk stratification after CT-confirmed cervical spine fractures remains clinically challenging in patients with major trauma. Imaging establishes a structural diagnosis, but it does not fully capture early physiological deterioration. This study aimed to develop and internally validate a prognostic model for in-hospital mortality and evaluate whether a MAP-derived Shock Index provides incremental prognostic information beyond routinely available clinical variables and the Injury Severity Score (ISS). Methods: This retrospective single-center cohort study

Diagnostic decision-making, vertebral augmentation, and osteoporosis therapy in vertebral compression fractures: a narrative review.

Vertebral compression fractures (VCFs) represent the most prevalent osteoporotic fracture and constitute a growing cause of morbidity, mortality, and healthcare utilization worldwide. Although often considered benign, VCFs are associated with chronic pain, progressive spinal deformity, functional impairment, decreased pulmonary capacity, and increased short- and long-term mortality, especially in older adults. Despite their high prevalence, key challenges persist regarding appropriate diagnostic evaluation, optimal treatment selection, and integration of osteoporosis management following fract
