Finite element modeling and experimental validation of wedge fractures in the thoracic spine.

PURPOSE: Vertebral compression fractures (VCFs) are the most common fractures in patients with osteoporosis, contributing to approximately 700,000 spinal fractures annually. Wedge fractures, characterized by anterior vertebral body collapse, are the most prevalent type of VCFs and a significant cause of spinal deformity, such as thoracic kyphosis. This study aimed to develop and validate a finite element model (FEM) of wedge fractures to understand their biomechanics and clinical implications, and help future studies to elucidate spine fractures in osteoporotic patients. METHODS: CT-based fin

Intraoperative three-dimensional imaging-guided vertebral body stenting for cleft-associated very severe osteoporotic vertebral compression fractures: a surgical technique case series.

Very severe osteoporotic vertebral compression fractures (vsOVCF) associated with intravertebral clefts represent a technically challenging subgroup of vertebral augmentation because of severe vertebral collapse, distorted intravertebral anatomy, and increased risk of cement leakage. Conventional fluoroscopy alone may be insufficient for accurate device positioning in these cases. We describe the surgical technique and technical utility of intraoperative three-dimensional (3D) imaging during vertebral body stenting (VBS) in three consecutive patients with cleft-associated vsOVCF without neurol

Surgical approaches, techniques and knowledge gaps in the management of osteoporotic vertebral fractures in the thoracic and lumbar spine: A Narrative review.

Osteoporotic vertebral fractures (OVF) represent a growing clinical challenge due to population aging and the increasing prevalence of osteoporosis. Surgical management of OVF differs substantially from that of traumatic vertebral fractures in younger patients and requires careful consideration of fracture morphology, stability, and patient-related factors. A wide range of surgical techniques is currently available, yet clear treatment algorithms remain limited. A non-systematic narrative review of the literature was performed using PubMed, Scopus, and Google Scholar, supplemented by manual re

Non-osteoporotic L4 vertebral compression fracture after low-voltage electrical injury without secondary trauma: A rare case report.

Electrical injury is a common occupational hazard; however, vertebral compression fracture caused by low-voltage electric shock in the absence of secondary fall- or impact-related trauma is extremely rare. We report the case of a man in his early 40s who developed severe lower back pain and inability to walk immediately after a 220-volt alternating-current electrical injury at work. He remained conscious and sustained no fall or blunt trauma. Imaging examinations, including radiography, computed tomography, and magnetic resonance imaging, revealed an acute L4 vertebral compression fracture. Du

Segment-specific prediction of vertebral collapse severity in osteoporotic fractures.

BACKGROUND: Osteoporotic vertebral compression fractures (OVCFs) predominantly cluster at the thoracolumbar junction (T11-L1). Recent studies suggest that both bone quality and muscle health significantly contribute to vertebral collapse. However, among numerous multidimensional imaging metrics, which one can most accurately predict the severity of vertebral collapse, and whether the precise load-sharing proportions between bone and muscle exhibit distinct segment-specific differences, remain rarely quantified in the clinical literature. PURPOSE: To identify imaging metrics that best predict

Minimal kyphotic progression following pediatric vertebral fractures: implications for reduced radiographic surveillance.

INTRODUCTION: Pediatric spinal fractures are rare injuries (14.24 per 100,000 per year), representing only 0.6-3% of all spinal injuries. While most stable compression fractures are managed conservatively, concerns exist regarding progressive vertebral collapse and kyphotic deformity. Unlike extensive adult literature, pediatric-specific data on radiographic progression following conservative management remain limited, creating uncertainty about the clinical utility of routine follow-up imaging in this radiosensitive population. MATERIALS AND METHODS: Retrospective single-centre cohort study

Outcomes following structural allograft-chain impaction kyphoplasty.

BACKGROUND: Kyphoplasty (KP) commonly uses bone cement to augment vertebral bodies and to reduce pain after a compression fracture or vertebral collapse. However, cement may pose risk of extravasation and potentially compromise future operations on the injured vertebral body. Impaction grafting with allografts is a commonly used orthopedic technique to support weak bones, fill bone voids, and restore normal bone within a region. The goal of this study is to evaluate height restoration, pain relief, and extravasation in a 1-year follow-up of patients who underwent impaction grafting kyphoplasty

CT-Detected Intravertebral Cleft Predicts Radiographic Collapse Progression After Body-Cast Treatment of Acute Osteoporotic Vertebral Fractures.

BACKGROUND: Progressive radiographic collapse after osteoporotic vertebral fractures (OVFs) may lead to kyphotic deformity. Predictors remain inconsistent, partly because prior studies used heterogeneous conservative treatment protocols that may alter collapse risk. QUESTIONS/PURPOSES: We asked (1) how much vertebral compression progressed after a standardized body-cast protocol, (2) which factors were associated with progression, and (3) whether computed tomography (CT)-detected intravertebral cleft (IVC) independently predicted progression. METHODS: We retrospectively reviewed 162 patients

Sustainable high performance computing in the UK: a position paper on public investment, accountability, and the path to net zero.

BACKGROUND: Factors associated with early re-collapse after posterior fixation of thoracolumbar burst fractures have been sparsely examined in previous studies. Early re-collapse is challenging to predict due to multifactorial risk. Therefore, we aimed to investigate factors associated with vertebral re-collapse within the first 3 months following posterior fixation. METHODS: This retrospective study used data from the Swedish Fracture Register (2015-2019) and included patients aged 18 to 66 years with a single-level thoracolumbar burst fracture between T11 and L3 treated with posterior fixat

Single-position 360° correction of severe osteoporotic thoracolumbar kyphosis with neurological recovery.

A woman in her seventh decade with severe osteoporosis developed progressive thoracolumbar kyphosis (86°) and new upper motor neuron signs. Imaging revealed vertebral collapse at T11-T12 with partial correction in the prone position. A single-position, prone-only 360° correction was performed using posterior instrumentation and a minimally invasive Extreme Lateral Interbody Fusion (XLIF) corpectomy. Intraoperative navigation and pleural management enabled safe anterior access without repositioning. Postoperatively, kyphosis improved to 24°, neurological function remained intact and mobilisatio

Natural polymer-derived hydrogel as an emerging hemostatic strategy.

Rapid and effective bleeding control is critical for preventing circulatory shock and improving patient survival. Compared with conventional hemostatic materials such as gauze and patches, hydrogels have gained increasing attention in hemorrhage management owing to their excellent biocompatibility, high water retention, and superior wound sealing capabilities. Among them, natural polymer-derived hydrogels are particularly promising for clinical applications, as their inherent biological components enable the formation of extracellular matrix-mimicking networks that promote tissue integration a

Clinical Reasoning: A 59-Year-Old Man With Delayed Electric Shock-Like Sensations After a Fall.

We present a 59-year-old man who developed delayed-onset electric shock-like sensations triggered by neck flexion approximately 3-4 weeks after a workplace fall. Immediately after the fall, he experienced a brief episode of transient global sensory loss that resolved within minutes, followed by headache and posterior neck pain. Approximately 10 minutes later, he developed bilateral upper extremity paresthesias in the C6 dermatomal distribution that gradually resolved over several hours. Several weeks later, he developed reproducible electric shock-like sensations radiating from the neck throug

Awake Tracheostomy in the Sitting Position for a Predicted Difficult Airway in Ankylosing Spondylitis With Inhalation Injury: A Case Report.

BACKGROUND: Airway management in patients with inhalation injury is particularly challenging because of rapidly progressive airway edema. This difficulty is further compounded in patients with ankylosing spondylitis, in whom cervical spine rigidity and deformity severely restrict neck mobility and may prevent standard positioning for airway interventions. In such situations, conventional airway management techniques may fail, necessitating alternative strategies. CASE PRESENTATION: We report the case of a 48-year-old man with inhalation injury following a flame burn who also had long-standing

Intrabody Cage Augmentation in Kümmell Disease and Osteoporotic Burst Fractures: Technical Insights and Narrative Review of Current Evidence.

Intrabody cage augmentation has emerged as a minimally invasive technique for anterior column reconstruction in Kümmell disease and osteoporotic burst fractures. These osteoporotic conditions lead to progressive vertebral collapse, kyphosis, and instability. While cement augmentation provides rapid pain relief, it often fails to reliably restore sagittal balance or ensure biological integration in advanced stages of collapse. Although conventional anterior corpectomy with long-segment posterior fusion can achieve satisfactory deformity correction, these procedures are associated with substanti

Early mechanical outcomes of two expandable interbody cages in minimally invasive TLIF: a comparative single-surgeon study.

BACKGROUND: Expandable transforaminal lumbar interbody fusion (TLIF) cages permit low-profile insertion followed by in situ restoration of disc height and segmental lordosis. Mechanical failure, including secondary collapse of the expansion mechanism and endplate subsidence, has been reported with some expandable designs. We compared early secondary cage collapse and, secondarily, subsidence between the Catalyft PL and X-PAC expandable TLIF cages in a consecutive single-surgeon minimally invasive surgery (MIS) cohort. METHODS: We retrospectively reviewed 168 consecutive patients (224 cages) u

Effect of the Reverse Cotton Osteotomy on First Metatarsal Position: A Cadaveric Study.

BACKGROUND: Flatfoot deformities are characterized by collapse of the medial arch due to tendinous and ligamentous dysfunction. The degree of deformity can be evaluated radiographically by measuring the talo-first metatarsal angle (Meary angle). These deformities have traditionally been corrected using a medial cuneiform dorsal opening wedge (Cotton) osteotomy; however, this procedure requires bone grafting to fill the osteotomy site and results in lengthening of the medial column. To address these concerns, the plantar closing wedge of the medial cuneiform bone, known as the reverse Cotton os

Early postoperative L1 pelvic angle and T10 pelvic angle as predictors of proximal junctional kyphosis in adult spinal deformity.

BACKGROUND: Excessive posterior inclination of the thoracolumbar spine increases stress concentration at the upper instrumented vertebra (UIV) and adjacent motion segments, potentially accelerating junctional collapse or kyphotic failure. This study evaluated whether early postoperative regional parameters - specifically T10 pelvic angle (T10PA) and L1 pelvic angle (L1PA) - predict the development of proximal junctional kyphosis (PJK) following long-segment fusion for adult spinal deformity (ASD). METHODS: A retrospective review of 100 patients undergoing long-segment posterior fusion to the

The pelvic femoral angle in flexed-seated position analysis predicts impingement risk of total hip arthroplasty: Defining hip joint hypermobility and true hip hyperflexion.

BACKGROUND: Although total hip arthroplasty (THA) is highly successful, standard cup orientation does not prevent all impingement-related complications. Functional planning integrates spinopelvic motion, but flexed-seated definitions of hip hypermobility remain unclear. We aimed to define clinically relevant flexed-seated femur-pelvis motion thresholds and test their association with simulated impingement and anteversion safe-zone constraints. HYPOTHESIS: The existence of a pathological hip mobility threshold, independent of other risk factors for impingement with standard cup positioning, wo
