Use of Cryoneurolysis Therapy for the Management of Idiopathic Trigeminal Neuralgia.

INTRODUCTION: Trigeminal neuralgia is a debilitating facial pain condition characterized by recurrent, electric shock-like episodes within the trigeminal nerve distribution. While first-line treatment involves pharmacologic therapy, treatment options are limited for patients who are refractory or poor surgical candidates. CASE PRESENTATION: We present a 75-year-old female with long-standing idiopathic trigeminal neuralgia refractory to combination anticonvulsant therapy and nerve block, with notable medication-related side effects. MANAGEMENT AND OUTCOMES: The patient underwent percutaneous

A rare and complex case report of superimposed clostridial spondylodiscitis and epidural abscess associated with spinal lymphoma of the thoracic spine.

BACKGROUND: Progressive back pain in elderly individuals may be caused by spine infections, metastatic cancers, or hematologic malignancies. Spine infections, such as spondylodiscitis, often result from hematogenous spread by staphylococci, streptococci, or gram-negative bacilli, whereas Clostridium species is a rare cause of spondylodiscitis. Lymphoma involving the spine can mimic infectious spondylodiscitis and lead to irreversible neurological damage. We report an elderly Thai woman with clostridial spondylodiscitis and lymphoma coexisting at the thoracic spine, highlighting the importance

Navigating the Diagnosis and Management of Papillary Muscle Rupture Following Inferior Myocardial Infarction.

Papillary muscle rupture (PMR) is often underdiagnosed or diagnosed late due to its low incidence and nonspecific symptoms in acutely presenting, critically ill patients. This report presents a case of a 74-year-old male who developed acute mitral regurgitation secondary to PMR in the context of an initially conservatively managed inferior wall infarction following recent spine surgery. The discussion highlights the most appropriate diagnostic approach for critically ill patients presenting with acute hypoxic respiratory failure in the cardiac ICU. It also examines optimal management strategie

Direct spinal cord injury during cervical epidural and nerve root blocks: A case series and pathophysiological reevaluation.

Cervical epidural and nerve root blocks effectively manage radicular pain but pose risks of severe neurologic complications, including acute myelopathy. Although vascular embolization of particulate steroids is often implicated in these events, direct mechanical spinal cord injury remains an underrecognized etiology. Identifying the true pathophysiologic mechanism is essential to prevent permanent neurological deficits. We retrospectively reviewed four patients who developed acute spinal cord injuries immediately following cervical nerve blocks or targeted acupuncture. We analyzed clinical pre

Technical considerations for time-efficient four-level biportal endoscopic lumbar interbody fusion in a frail elderly patient with severe lumbar deformity: a case report.

BACKGROUND: Biportal endoscopic lumbar interbody fusion (BE-LIF) has recently emerged as a minimally invasive alternative to conventional open lumbar fusion surgery. However, long-segment endoscopic fusion for multilevel degenerative lumbar stenosis with associated degenerative scoliosis remains technically challenging because prolonged operative time and perioperative morbidity may substantially increase in frail elderly patients. CASE DESCRIPTION: An 86-year-old man presented with severe back pain, bilateral radiating leg pain, progressive bilateral lower-extremity weakness, and left foot d

Challenges in Diagnosis and Management of Spondylodiscitis of L-5/S-1 Due to Propionibacterium acnes in a Patient with a Twenty-Year History of Tetraplegia.

Objectives: To present novel strategies in the diagnosis and management of spondylodiscitis in a patient with tetraplegia. Case Presentation: A 44-year-old man presented with increased spasms despite having a SynchroMed II Medtronic for intrathecal infusion of baclofen. The C-reactive protein level was raised. Infection was suspected, but there were no localising signs. Positron Emission Tomography (PET) was performed to identify the focus of infection. PET showed an increased Fluorodeoxyglucose uptake in the L-5 vertebral body and the associated ill-defined soft tissue anteriorly. There was s

Surgical Management of Pott's Paraplegia in a 27-Week Pregnant Woman with Continuation of Pregnancy: A Case Report.

INTRODUCTION: Spinal tuberculosis is primarily managed with antitubercular therapy, while surgical intervention is reserved for cases with neurological compromise or spinal instability. The occurrence of Pott's paraplegia during pregnancy is rare and presents unique diagnostic and therapeutic challenges due to concerns regarding fetal safety, imaging limitations, and surgical risks. Management during the second trimester is particularly complex, as options, such as medical termination or delivery are not feasible. Only a limited number of case reports describing Pott's paraplegia during pregna

Beyond Kyphosis: A Case of Tuberculous Spondylodiscitis With L3 Vertebral Dislocation.

Spinal tuberculosis (TB) accounts for the majority of skeletal TB cases and remains a significant cause of morbidity in developing nations. While thoracic involvement is most common, lumbar vertebral dislocation represents an exceedingly rare and severe complication. This case involved a 22-year-old female who presented with a six-month history of severe back pain followed by the sudden onset of complete paraplegia with bowel and bladder incontinence. Despite initiation of antitubercular therapy (ATT) at another facility, her condition progressively worsened. Neurological examination revealed

Association between hypertension and sepsis-related complications: a retrospective nationwide inpatient sample database cross-sectional study.

BACKGROUND: Sepsis remains a significant global health challenge, with high morbidity and mortality rates. The interplay between hypertension, a prevalent comorbidity, and sepsis outcomes has been the subject of ongoing debate. This study aimed to investigate the incidence of sepsis-related complications in individuals with and without hypertension via a large nationwide inpatient database. METHODS: A retrospective observational analysis was conducted via the National Inpatient Sample (NIS) database from 2010 to 2019. The study population comprised adults (≥ 18 years) diagnosed with sepsis an

Axial gout diagnosed by dual-energy CT in a young normouricaemic man with recurrent nephrolithiasis: a case-based review.

Axial gout is an underrecognized manifestation of monosodium urate (MSU) crystal deposition and frequently mimics inflammatory, infectious, or mechanical spinal disorders, particularly without overt hyperuricaemia. We report a 29-year-old man with recurrent low back pain for over two years and recurrent nephrolithiasis who had undergone three extracorporeal shock wave lithotripsy sessions with only transient relief. Admission laboratory investigations showed normal serum urate (262 µmol/L, ~ 4.4 mg/dL) but markedly elevated C‑reactive protein (119 mg/L) and erythrocyte sedimentation rate (53 m

Fatal Injuries Caused by Falling Trees: Correlation Between Postmortem Computed Tomography and Forensic Autopsy Findings in Eight Cases.

Falling trees and large branches can result in severe blunt trauma; however, detailed correlations between postmortem computed tomography (PMCT) and forensic autopsy findings in such deaths remain limited. We retrospectively analysed eight consecutive deaths occurring between 2012 and 2025 in which impact or compression by a tree trunk or large branch constituted the primary traumatic event and in which both PMCT and complete forensic autopsy were performed. Thin-slice PMCT of the head and trunk was obtained using standard brain, soft-tissue, lung, and bone windows, supplemented by a gas-optim

Successful surgical repair of blunt traumatic cardiac rupture with V-A ECMO initiated during open cardiac massage: a case report.

Blunt traumatic cardiac rupture is a rare and frequently fatal injury. The role of veno-arterial extracorporeal membrane oxygenation (V-A ECMO) during active surgical resuscitation remains uncertain. A 55-year-old man was transported to our hybrid emergency room in profound shock after a motorcycle collision. Focused Assessment with Sonography for Trauma demonstrated pericardial effusion with cardiac tamponade. Emergency resuscitative thoracotomy and pericardiotomy revealed a right ventricular rupture. Despite manual compression of the rupture site and open cardiac massage, the patient develop

Upright MDCT evaluation of gravity-induced bronchial collapse and paradoxical lung volume reduction in severe thoracic lordoscoliosis: A case report.

A 21-year-old male with severe thoracic lordoscoliosis presented with progressive dyspnea, which worsened in the upright position. Because standing whole-spine radiographs showed postural exacerbation of lordoscoliosis, upright multidetector computed tomography (MDCT) was performed in addition to conventional supine imaging to assess dynamic anatomical changes under gravitational influence. Preoperative upright MDCT revealed significant dynamic bronchial collapse. The right main bronchus diameter decreased from 3.23 mm in the supine position to 1.15 mm in the upright position, representing a 6

Neuroschistosomiasis Mimicking Spinal Tuberculosis: A Case Report.

Schistosomiasis is a chronic parasitic disease acquired through freshwater exposure, in which cercariae penetrate the skin and mature into adult worms. Disease manifestations largely result from granulomatous inflammation and fibrosis surrounding retained eggs. A 65-year-old widow presented with six months of lower back pain, weight loss, paraplegia (power 1/5), sphincter dysfunction, a T12 sensory level, and T9-T10 gibbus. She had received empirical antitubercular therapy for two months without improvement. Thoracolumbar MRI demonstrated vertebral and disc collapse at T10-T11 and L1-L2, with

Concomitant osteoporotic vertebral compression fracture and infectious spondylitis in a patient with herpes zoster: A case report.

RATIONALE: The incidences of herpes zoster, osteoporotic vertebral compression fractures, and infectious spondylitis is increasing worldwide owing to the aging population. Given the potential for multiple comorbid conditions with overlapping clinical presentations, thorough history-taking and physical examination are essential for accurate diagnoses. However, the coexistence of herpes zoster, osteoporotic vertebral compression fractures, and infectious spondylitis in a single patient is exceedingly rare, making timely diagnosis even more challenging. PATIENT CONCERNS: A 64-year-old woman with
